Study to the Case Group that Cemental Tear was Doubted Clinically

The cemental tear makes easy bacterial invasion to apical site of the root, and causes collapse of the local and rapid periodontal tissue. This study considered 10 cases of cemental tears detected by our study group.
(1) In all cases, the ages of the patients when the cemental tears were detected were relatively high, with the average of 66.4 years old.
(2) All cases were single root teeth.
(3) Subjects were 8 male and 2 female.
(4) Among the 10 cases, 2 were vital and 8 were nonvital.
(5) Apical cemental tear caused greater loss of periodontal tissue, and cervical cemental tear caused smaller loss of periodontal tissue.
These observations suggest that the causes of cemental tear are associated with traumatic external forces working on teeth and a variety of changes caused by aging of a human being.

A clinical comparison of Implant and Autotransplantation of teeth was reported.
A 29-year-old man case was examined to PDI Dental clinic with the sharp pain of maxillaly 1st pre-molar on left side. The tooth was diagnosed as the preservation impossibility, and was extracted. After about 6 months, IMZ Implant (a diameter 3.3 mm, length 13 mm) was placed. On the other hand, the mandibular right first molar was extracted by root fracture. Though Implant treatment was considered, autotransplantation of teeth was used from the same side's 3rd molar existing soundly. It have good prognosis though two years pass after the surgery.

Orthopantomography has been used for the evaluation of pre-implant surgery. However, because this method can only examine the shape in two-dimension images, it has been difficult to make accurate evaluation of anatomical structures. Three-dimensional information is imperfect in this method as of the upper necessary structure and the alveolar bone extant. Therefore, Authers take pictures of CT (computer tomograph) using customized stent with the X-ray contrast, and examined three-dimensionally. Moreover, 3-D re-constructed image is made on the computer, and simulation with implant placemant. The results suggest that using this method, it could get information before the detailed art, and 3-D simulation can be very useful for dental implant procedure.

The author believes that saving the periodontal ligament is the most important element for a successful autotransplantation. In this study, one case that the problem is caused by the mismatch of donor tooth and recipient site, and the other that saving of the periodontal ligament was attempted by adding orthodontic force to the donor tooth before transplanting are reported.
