Ultrasonography is an inexpensive, convenient, and noninvasive technique that provides real-time imaging with excellent spatial resolution of soft tissue. Ultrasound elastography is a promising method in differentiating malignant from benign in both primary lesion and regional lymph nodes.
In the current TNM classification(8th edition), depth of invasion(DOI) has been incorporated into T staging in oral tongue squamous cell carcinoma(OTSCC). Histopathologically, DOI is defined as the depth of tumor measured by dropping a“plumb line” to the deepest portion of the tumor from the level of the basement membrane of the normal mucosa closest to the tumor. According to previous reports, intraoral ultrasonography has proven to be useful for the measurement of DOI of OTSCC.
Ultrasonography is one of the reliable imaging methods currently available for detecting cervical lymph node metastases in patients with OTSCC. In addition to the minimal axial diameter criteria(exceeding 6-8mm),the ultrasonographic features of metastatic lymph nodes are round shape, loss of echogenic hilum, internal heterogeneity(anechoic and/or hyperechoic area), and peripheral vascularity. Periodical follow-up ultrasonography is useful in patients with OTSCC for the detection of subsequent lymph node metastases after treatment of primary lesion.

We report a case of cervical lymphadenitis caused by a rare nontuberculous mycobacterial infection (NTM infection), which was suspected to be tuberculosis by rapid intraoperative diagnosis. The patient, a 69-year-old man, was referred to our department for a thorough examination of a left-sided submandibular mass measuring 35 × 20 mm, which was pointed out by a local dentist. A needle biopsy was performed, however a diagnosis could not be made. We performed a left-sided submandibular gland tumor resection under general anesthesia. An intraoperative rapid diagnosis of tuberculosis was performed to help determine the extent of the resection. However, Mycobacterium intracellulare was detected in the postoperative bronchial lavage fluid, and cervical lymphadenitis caused by NTM was diagnosed. More than two years have passed since the surgery, and no recurrence has been observed.

We report a case of extraosseous/peripheral ameloblastoma with basal cell type. The patient was an 81-year-old man. At his initial visit, a large tumor measuring 15×10 mm in size was observed in the gingiva of his right maxillary node. CT images revealed no bone resorption around the tumor. A biopsy was carried out and, since it was found to be an extraosseous/peripheral ameloblastoma of the basal cell type according to a histopathological diagnosis, tumor resection was carried out under general anesthesia. The tumor was continuous with the gingival epithelium and the follicular proliferation of basal cell-like cells along with high cylindrical epithelium at the margins of the follicular nests were observed. Seven years and four months have elapsed since surgery, and no recurrence has been observed.

Solitary plasmacytoma is a disease characterized by monoclonal proliferation of plasma cells. It frequently occurs in the spine or femur but rarely in the maxilla. We herein report a case of solitary plasmacytoma of the maxilla that was discovered in association with poor wound healing after tooth extraction and progressed to multiple myeloma. A 72-year-old man was referred to our department for continuous pain after tooth extraction. We found a 15×10 mm mass with a smooth surface in the right upper first molar region. Incisional biopsy resulted in a histopathological diagnosis of plasmacytoma. Since there were no abnormal findings upon general examination in the hematology department, we made a definitive diagnosis of solitary plasmacytoma of the maxilla. The patient received local radiotherapy, and the tumor completely regressed. Although there was no evidence of local recurrence after one year and four months, laboratory analysis indicated remarkably high levels of IgG, and we therefore suspected the tumor would transform into multiple myeloma. After an eight-month hiatus, the patient revisited the hematology department, where PET-CT revealed multiple bone lesions and a diagnosis of multiple myeloma was made. We recommended chemotherapy, however the patient declined and elected for best supportive care.

Most temporomandibular joint(TMJ) disk disorders involve anterior dislocation, while folding of the retrodiscal tissue is extremely rare. We report a case of folding of the retrodiscal tissue of the TMJ. The patient was a 29-year-old female who visited our department in July 2020 with the chief complaint of difficulty closing her mouth. At the time of the initial examination, when the mouth was closed, the mandible deviated to the left and the left molar region was separated from the anterior tooth region. The mandible was manually shifted to the right with a click of the right TMJ, and the intercuspal position was restored. However, the left-sided deviation recurred easily when the patient opened her mouth. The patient was diagnosed with folding of the retrodiscal tissue of the right TMJ from magnetic resonance imaging(MRI). Pumping manipulation was performed. However, the mandible quickly shifted to the left, even though it had been repositioned to the right. Therefore, intermaxillary traction was performed with an intermaxillary elastic. Four days after the operation, the position of the mandible had adjusted, and the intermaxillary traction was released on the 11th day after the operation. One and a half months after the operation, MRI showed a disc adaptation of the right TMJ.
