    Seasonal change of the finger tip pulse wave responsive to language stimulation (instruction) was observed among 28 subjects for fall (September), winter (December), spring (March) and summer (July) in that order. The selected 28 subjects were 21 remote hemiplegic male of 6th and 7th decade (14 right hemiplegics and 7 left hemiplegics) and 7 normal male of the same age group.
    The following findings were obtained;
    1. The fingertip pulse wave among the normals showed marked lowering in fall under language stimulation (instruction) and reading showing W-type pattern.
    2. Among the hemiplegics when given language stimulation the lowering of the pulse wave did not recover in one minute showing tension state longer than the normals. The recovery after reading was as fast as the normals.
    3. The tension caused by language stimulation seemed stronger in right hand than left and seemed to last longer. This tendency was similar in both the normals and hemiplegics.
    4. Right hemiplegics were observed in 2 groups with or without speech disturbance. There was no basic difference and common response pattern as right hemiplegia was observed.
    5. Among the left hemiplegics lowering of the pulse wave under language stimulation was not recognized in fall both on right and left side and there was no lowering on the unaffected side under reading. This response was different from that of the right hemiplegics.
    6. Judging from the fact the hemiplegics showed W-type pattern and the normals did not in spring, it was presumed that altering rhythm of the sympathetic function in a year might be changing among the hemiplegics. However, this might be due to the factor that only the in-patients were selected as subject for this particular study.

  • Feeding Activities and Scissoring Activities

    It was reported by the author that a system named “ADL finger strain-gauge system” is useful for kinesiological analysis of fine finger activities included in usual ADL items and that this system was used for analysis of writing activities. At that study, several results were obtained about handwriting activities.
    Similar study were also obtained in feeding and scissoring activities.
    The study of feeding activities revealed that;
    1. The pressure between each of 3 fingers and the tea-spoon is 51g on an average.
    2. This pressure is about 1/1000 of total hand pressure, 1/200 of pinch-pressure between the thumb and index finger, 1/5 of total writing pressure, and 1/2.5 of finger-pressure during handwriting.
    3. Minimum pressure for spooning is about 23g on an average.
    4. Maximum pressure for spooning is about 100g on an average.
    5. Chop-stick activities was also investigated, but the present method of investigation was found inadequate for this purpose.
    6. and some others.
    In the study of scissoring activities, western type scissors and Japanese ones were compared and similar results with those of feeding activities were obtained.

  • 60 Cases

    60 casee of CP (spastic hemiplegia 3, spastic paraplegia 24, athetoid 7, mixed 3) were given the phenol block with 5% phenol solution to facilitate the training and nursing care. Motor branches of hypertonic muscles, which disturb the ambulation, the ADL, and the nursing care of CP, were searched by teflon coated needle connected with the portable type electrostimulator. At most 4 times of the block had done to one muscle group, when the recurrence had occured after phenol block.
    Spastic pes equinus were treated by motor point blocks (19 times), or tibial nerve blocks (83 times) of the calf muscles. All of the motor point block had recurred within 3 months. However, the tibial nerve block could have long term of effectivenss. More than 6 months of the effective duration has been gotten in 10 time trials of tibial nerve blocks.
    The obturator nerve blocks (42 times) were done to decrease the adductor spasticity, which distorted the gait of CP, and also disturbed the nursing care of severely crippled CP children. 28 times of the trial of obturator nerve blocks could have the long term of effectiveness. 19 times of them has been effective more than 6 months after the blocks.
    4 cases of tension athetosis were treated by the block on thoracodorsal nerve, motor points of triceps brachii, and sciatic nerve to decrease the hypertonic state of the muscles. 1 case of them had the long term of effectiveness, and other 3 cases had the very short term of it.
    Using the thin teflon coated needle to block the nerve trunk, for instance sciatic nerve, the target muscle would be blocked selectively. This comes from author's experience. Phenol block also had done to avoid second operation of Achilles tendon lengthening to correct pes equinus, and adductor tenotomy of the hip.
    16 limbs got the phenol block of calf muscles to correct the recurred pes equinus after Achilles tendon lengthening. Three limbs also got the obturator nerve blocks to correct the recurred adductor spasticity following adductor tenotomy.
