    Afemaleinherfties used Lulicon○R ointment to treat athlete’s foot, and many small blisters developed in the same region on both the hands and legs. Patch testing was performed for the ingredients of Lulicon○R ointment. Based on the positive reaction to luliconazole, the patient was diagnosed with contact dermatitis caused by luliconazole. Luliconazole is an antimycotic agent belonging to vinyl imidazoles, and there is a possibility of cross-reactions with other topical imidazole agents. Due to the increased use of over-the-counter topical antifungal agents, there are more opportunities for sensitization to imidazoles and associated risk of allergic contact dermatitis mediated by their cross-reactions.     A 64-year-old man presented to the otolaryngology department of our hospital with painless swelling on his left neck. Partial biopsy revealed an epithelial malignant tumor and he was referred to our department. Histologically, atypical cells exhibited a cord-like or solid structure, and inltrated into fat and muscle tissue. He was diagnosed with epithelioid angiosarcoma based on immunostaining of CD31 (diffuse positive), CD34(partial positive), factor VIII (negative), and cytokeratin AE1/AE3 (weak, partial positive). PET-CT suggested metastases on the left and right neck, and supraclavicular and left subclavian, axillary and mediastinal lymph nodes. After docetaxel administration, the tumor and lymph node metastases markedly improved. Epithelioid angiosarcoma is a rare, more malignant variant of angiosarcoma, with only 28 cases reported in Japan to the best of our knowledge. Skin Research, 18 : 337-343, 2019

    A 59-year-old woman presented with blisters and erythema on her neck and both palms, which had persisted for 7 months. Physical examination revealed multiple tense blisters and erosions on the trunk, extremities, and buccal mucosa. Histopathological examination demonstrated subepidermal bullae and lymphocytic inltration in the upper dermis. Immunostaining for type IV collagen was positive on the dermal side. Direct immunouorescence of the lesional skin revealed the linear deposition of IgG and C3 at the basement membrane zone. Indirect immunouorescence examination of 1 M NaCl-split normal human skin demonstrated IgG reactivity with the dermal side of the cleft. Enzyme-linked immunosorbent assay detected IgG autoantibodies against type VII collagen. The patient was diagnosed with epidermolysis bullosa acquisita based on the clinical,pathological, and immunological ndings. The skin lesions were well-controlled by prednisolone at 15 mg (0.3 mg/kg)/day. Type IV collagen positivity on the dermal side suggested separation at the lamina lucida due to inammation.     An 84-year-old man presented with painful subcutaneous nodules on both lower legs and feet that had begun to develop a week earlier. Erythema nodosum was suspected and skin biopsy was performed, which revealed the presence of ghost-like cells. Based on this nding, the lesions were diagnosed as subcutaneous nodular fat necrosis. As pancreatic disease was suspected to be involved in this condition, the patient underwent more detailed examinations. Blood examination revealed high serum levels of pancreatic enzymes (serum amylase : 2,244 IU/L, serum lipase : 306 U/L). In addition, abdominal ultrasonography and magnetic resonance cholangiopancreatography (MRCP) revealed cystic lesions in the pancreas and dilatation of the main pancreatic duct. Based on these ndings, the patient was diagnosed with intraductal papillary mucinous neoplasm (IPMN) of the pancreas. Thus, in this case, the diagnosis of subcutaneous nodular fat necrosis led to the diagnosis of IPMN. Subcutaneous nodular fat necrosis sometimes precedes the clinical symptoms of pancreatic diseases. Therefore, early diagnosis of subcutaneous nodular fat necrosis is important for the early detection of pancreatic diseases. Skin Research, 18 : 349-352, 2019

    A 78-year-old man had been receiving an anti-PD-1 antibody therapy every 3 weeks for bladder cancer at our hospital. After completing 17 courses of pembrolizumab therapy, erythema developed across the body with severe itching. On skin biopsy of erythema from the left thigh, prolongation of epidermal rete ridges, acanthosis with inltration of neutrophils, and hypogranulosis in the epidermis were observed. Inammatory cell inltration with eosinophils in the dermis was also noted. Therefore, we diagnosed the patient with psoriasiform dermatitis caused by the administration of pembrolizumab. Due to the spread of erythema, pembrolizumab was discontinued and we started topical therapy with corticosteroids and vitamin D3, but the spread of erythema persisted. When narrow-band UVB (NB-UVB) phototherapy was added, erythema slightly faded one month later. Since then, the skin rash has not recurred even with the continuation of pembrolizumab. NB-UVB phototherapy should be considered as a therapeutic option for patients with psoriasiform dermatitis caused by anti-PD-1 antibody treatment. Skin Research, 18 :353-359, 2019

    A 33-year-old Japanese man went hiking in a mountainous area in France in August. He noticed a tick with itching on his waist and felt slight fatigue several days later. He returned home and visited the Department of Dermatology of Nippon Life Hospital. At the rst visit, a blood sucking tick was found on his waist, but erythema migrans was not found around the bitie site. Laboratory tests revealed a slight increase in serum levels of liver enzymes and c-reactive protein. The tick was removed with the surrounding skin under local anesthesia and the patient was administered 750 mg/day of tetracycline hydrochloride for 14 days. Two months later, the patient consulted the Department of Dermatology of Hyogo College of Medicine for examination of the tick and tick-borne infectious diseases. The tick was identied as an adult Ixodes ricinus female and serum IgM antibodies to Lyme disease were positive. Based on these results, we diagnosed him with Lyme disease caused by the tick bite in France. Early antibiotic therapy may have prevented aggravation of the disease in the present case. Dermatologists in Japan should pay attention to Lyme disease due to Ixodes tick bites in patients who visited Europe or North America. Skin Research, 18 : 360-365, 2019
