We report two cases of apocrine poroma.
Case 1: A 71 year-old male has noticed a 9×4mm red tumor on his right back 6 weeks before the appearance to our clinic. Histological findings are following. The tumor consisting of poroid cells and cuticular cells emanates downward into dermis anastomosing surface epithelium. Many long lumens in various sizes are observed continuous with infundibulum like structures. Decapitation secretion is also seen. Tumor consists of poroid cells and cuticular cells.
Case 2: A 56 year-old male has noticed the pink-red 20mm sized papillomatous tumor on his left thigh seven years ago. Its histologic picture is similar to eccrine poroma, except for the presence of a Y-shaped lumen with eosinophilic cytoplasm-containing intra luminar cells as well as small clusters of sebocytes, which suggests folliculo-sebaceous-apocrine differentiation.

The 69 year-old man presenting with skin metastasis from prostatic carcinoma is reported.
11 years ago, he had a diagnosis of prostatic carcinoma and metastasis to bone and then he had been treated with the hormonal therapy, radiotherapy and chemotherapy.
At first visit, he had presented some red colored firm skin nodules mainly in the scrotum part. Histopathological studies showed dermal nests consisted of atypical cells of non-cutaneous origin, which arranged in a glandular pattern. Immunohistochemical staining of those cells for prostatic specific antigen were positive.
At the time skin metastasis appeared, PSA rose in 980ng/ml and computed tomography showed wall of bladder infiltrated, metastasis to the pelvic and mediastinal lymph nodes and to liver. He died about two months later.
Prostatic carcinoma rarely metastasizes to the skin. But, we propose that this case will increase, because the prevalence rate of prostatic carcinoma is increasing dramatically.

A 72-year-old man presented with a slow-growing, asymptomatic red nodule of a 2-year history on the right side of the scrotum. The lesion was a solitary, soft, and pedunculated nodule with a granular surface. The lesion was excised under local anesthesia. On microscopic examination, the specimen showed papillomatous elongation of the rete ridges and infiltration of form cells in the upper dermis. The dermis contained several dilated vessels. Based on these features, the diagnosis of verruciform xanthoma was made. Compared to other cases of verruciform xanthoma, we observed marked vascular ectasia in the dermis. This observation indicated that high vascular pressure may play a role in the pathogenesis of verruciform xanthoma.

A 47-year-old man had had a lot of asymptomatic vascular spiders on the trunk and arm from 2002 when he had been pointed out alcoholic liver cirrhosis. These lesions showed typical unilateral distribution on the right side of upper chest, arm, shoulder and upper back associated with bilateral palmar erythema and gynecomastia. Laboratory findings, abdominal ultra sonic and abdominal computerized tomography showed alcoholic liver cirrhosis. Histopathological examination of the cutaneous lesion revealed telangiectasia of small vessels with perivascular lymphocytic infiltration in the upper dermis. Immunohistochemical staining for the estrogen receptor on the dilated vessel walls was negative. Both blood estradiol and progesterone level were normal. A diagnosis of unilateral nevoid telangiectasia (UNT) due to alcoholic liver cirrhosis was made from these findings. UNT is considered to be caused by increased estrogen level associated with pregnancy, puberty or liver disease. A nevoid predisposition may be also related to the onset of UNT. Male patients with UNT caused by liver dysfunction have been rarely reported so far.

Tufted hair folliculitis is characterized by scarred erythematous aspect of the scalp on which each follicular orifice has tufted hairs.
A 54-year-old woman who had chronic nephritis came to our hospital complaining of 5cm reddish plaque on the parietal region of her scalp. On the lesion there were dilated follicular orifices in part of which tufted hairs were observed. Hairs on the lesion were easily pulled out. One month administration of roxythromycin was effective against the inflammation of the skin lesion in our case.

A 56-year-old woman visited our hospital with erythematous subcutaneous nodules and plaques on her lower extremities. The histopathologic examination of the erythematous nodule demonstrated lobular panniculitis with granulomatous inflammation and necrotizing vasculitis. A diagnosis of Erythema Induratum of Bazin was made. The tuberculin skin test was strongly positive, but the chest radiograph revealed no abnormalities. QuantiFERON®-TB 2nd Generation (QFT) has recently been developed as a new technique for detecting tuberculosis infection regardless of BCG vaccination. It offers high sensitivity and high specificity. And it can make a diagnosis of latent tuberculosis infection. QFT is useful for the diagnosis of Erythema Induratum of Bazin with tuberculosis infection.

We assessed the efficacy of oral administration of olopatadine hydrochloride (two tablets per day) over 2 weeks in 95 patients (aged over 15 years old) with pruritic dermatoses, based on visual analogue scale (VAS) scores obtained at the start of administration and 2 to 4 weeks after treatment. We also conducted a questionnaire-based survey of the patients to determine their satisfaction with the treatment. The VAS score for pruritus was significantly reduced after treatment, regardless of previous treatment. When the patients were divided into 4 groups based on the change in VAS score, the reduction in VAS score was found to be closely related to the answers on the questionnaire regarding the effectiveness, the satisfaction with the drug and the hope for continuation of the drug.
