    Peroral pancreatoscopy using ultra thin pancreatoscope was performed on 44 patients with pancreatic disease. In 14 patients received surgical operation (13 with pancreatic cancer and one with chronic pancreatitis), endoscopic findings by pancreatoscopy were compared with histopathological findings. Endoscopic findings of irregular elevated lesions and uneven mucosa corresponded to infiltration of cancer cells to the epithelium of the ducts histologically. On the other hand, endoscopic findings of redness with erosive changes corresponded to infiltration of cancer cells around the duct walls or into the epithelium of the main pancreatic duct. Image processing was applied for obtaining clear images of those findings. The results were as follows; 1) band enhancement processing was effective for clarifying the fine structure of the foreground and the outline of the lesions in a distant view, 2) paint out processing was useful for brightening the dark area of the lume in a distant view. Furthermore, usefulness of 3.5 inch MO disk system to record the moving images of pancreatoscopy was studied, which miniaturizes the recording media in comparison to U-matic videotape system. It was suggested that MO disk system may be applied clinically by adoption of proper compressing method and elongation of recording period. His-tological comparison and such improvement will raise the diagnostic ability of pancreatoscopy.

    A clinicopathologic study of 47 lateral spreading tumors of the colorectum was performed in order to decide the clinical management. According to the endoscopic and macroscopic findings, 47 lateral spreading tumors were classified into 32 nodular type tumors and 15 non-nodular type tumors. Nodular type was defined as flat tumor with conglomerated nodular surface and non-nodular type as that with flat-topped surface without nodules. The mean size of nodular type was significantly larger than that of non-nodular type (p< 0.01) . Nodular type was found more often in the rectum and proximal colon than non-nodular type (p< 0.001) . Histologically, carcinoma in adenoma was found in 56% of nodular type tumors, whereas pure carcinoma without adenoma in 60% of the non-nodular type tumors. In cases of carcinoma, non-nodular type showed deep submucosal invasion more frequently than nodular type (36% versus 11%) . These results suggest that both types are characterized by superficial spreading growth in the mucosal layer, but non-nodular type is more aggressive tumor than nodular type. Thus, endoscopic resection is appropri-ate for most cases of nodular type, but some cases of non-nodular type in which endoscopy and radiography indicate the findings suggestive of submucosal invasion require surgical treatment.

    EUS guided FNA cytology was attemted in 27 consecutive patients with pancreatic and peripancreatic mass lesions and submucosal tumors of the gastrointestinal tract using an ultrasound endoscope with a curved linear array ultrasonic transducer. EUS guided FNA cytology was performed in 25 patients. In 20 patients, the material aspirated was enough for analysis. The predictive value of EUS guided FNA cytology for malignancies was as follows. Sensitivity was 67%, specificity 100%, the positive predictive value 100%, the negative predictive value 67%, and the overall accuracy rate was 80%. No complications were encountered. In conclusion, EUS guided FNA cytology elevated the accuracy rate of EUS diagnosis for small lesions of the gastrointestinal tract and is therefore, possible to become a very rational approach for diagnostic purpose, but its introduction on a routine basis will be greatly facilitated with further improvement in the cytology procedure and needle technology.

    A 77-year-old man was admitted to our department in October 1990 with a complaint of exertional dyspnea. He was diagnosed to have folic acid deficiency anemia. The endoscopy of the esophagus and the stomach performed during his hospitalization revealed reflux esophagitis. After discharge, the endocopy of the esophagus and stomach was performed again becuse he complained of heart burn. In the lower esophagus, multiple whitish elevated lesions of approximately 5mm in diameter with granular surface were observed. The histologic study of the polypectomied specimen of elvevated lesions revealed squamous cell papilloma like lesion. We thought that this case was progress case of esophagitis. There is not a report developing esophageal papilloma in short term of 5 months like this case. This case was considered worthy of note in terms of elucidation of the etiology of this disease.

    A 67-year-old woman with liver cirrhosis was referred to our hospital by her neighbor-ing doctor after prophylactic sclerotherapy for esophageal varices had been performed 8 times without effect. A pipeline varix was found next to a cardiac varix by endoscopy. Sclerotherapy was performed using 5 % ethanolamine oleate together with ethanol while vasopressin was being dropped, but there was no effect due to the high blood flow. The same sclerotherapy was repeated using stomach and esophageal balloons to compress the varices from two sides and slow down the blood flow. This was effective in keeping the scierotant in the varices longer and attained reduction of the lesion. Sclerotherapy with stomach balloon appears to be an effective technique for pipeline varices.

    A 58-year-old male was admitted to our hospital because of dysphagia for the past two months. X-ray examination of the upper GI series showed a giant spindle shaped elevated lesion located in middle to lower esophagus. Endoscopic examination of esophagus revealed a reddish tumor covered with necrotized substances. Endoscopic ultrasonographic examination revealed a hypoechoic area under the membrane. We suspected this tumor was carcinosarcoma of esophagus by the findings endoscopic biopsy specimens and perfor-med sub-total esophagectomy. The size of tumor was 17.5 x 7.5 x 4.0 cm. The diagnosis of so-called carcinosarcoma of esophagus was made by histological and immunohisto-chemical findings.

    A 71-year-old woman presenting with epigastric pain, who underwent gastrectomy for duodenal ulcer 30 years ago, was hospitalized because UGI x-ray and endoscopic examina-tions revealed a polypoid lesion in the jejunum. The polypoid lesion with a smooth surface was yellowish, movable and depressed when pushed by a biopsy forceps. Then we removed the polypoid lesion by endoscopy successfully. Histological findings of the specimen revealed typical features of lipoma. Only 3 cases of the lipoma in the jejunum removed endoscopically including this case, have been reported in japan.

    This case is a case report of adenoma of Papilla of Vater. Endoscopic findings of the duodenum shows protruded tumor with granular surface in the area of the papilla of Vater. Biopsy specimen from the tumor revealed tubullovillous adenoma with severe atypia. Hypotonic dudenography shows a tumor with granular surface in diameter of 30 mm in the area of the papilla of Vater. ERP and PTC shows normal findings in the pancretobiliary system. EUS and IDUS revealed a high echoic tumor with pedunculated stalk with no infiltration to the pancreas, no spread of the bile duct and main pancreatic duct. We resected the tumor of papilla of vater with use of cutting current. The postoperative course was unevenful. The pathological diagnosis was tubullovillous adenoma with severe atypia.

    A 47 year-old-male was admitted to the hospital, presenting with bloody stools. Barium enema and colonoscopy showed a flat elevated lesion with an irregular shallow depression in the lower rectum. An early cancer of type ha +IIc was suspected. Microscopic examination of the biopsied specimens from this lesion disclosed marked lymphoid infiltration, posing differential diagnostic problems from low-grade lymphoma. Local resection was performed for the purpose of diagnosis and treatment. The tumor was 7×7×4 mm in diameter. Histologically, the tumor showed atypical lymphoid proliferation in the submucosa with amyloid-like sclerosis. The pathological diagnosis was MALT lymphoma, although the typical lymphoepithelial lesion was not found.

    We experienced a case of common bile duct granuloma with the infiltration of eosinophils. The patient was a 70-year-old man. He, who had taken hemodialysis, complained of general fatigue and itching. Laboratory studies showed liver dysfunction with hyperbilirubinemia. Abdominal US and CT showed gallstone only, but ERC showed elevated lesion of about 10mm in the common bile duct. And EUS showed solid tumor with hypoechoic area in that part. Histological findings of the biopsied specimens showed no malignancy but operation was performed since malignancy could not be ruled out. Pathological examination of the resected specimen revealed granuloma with the dense infiltration of eosiniphils.

    An implantation of biliary tube stents or metallic stents is considered to offer im-proved long term palliation for inoperable cases of malignant biliary obstruction, whereas the obstruction of the devices is often complicated. Recently, Gilles et al. reported percutaneous transhepatic hepaticogastrostomy, by which the obstruction of the stent could be avoided. We performed this therapy in two patients with unresectable malignant biliary obstruction for the first time in Japan with successfull results; the patency of the stent was maintained. This technique seems to become one of the useful palliative therapies for the cases of malignant biliary obstruction.
