Ventilator associated pneumonia (VAP) is a serious problem in a patient on mechanical ventilation. Bacteria contamination of the breathing circuit including a nebulizer and tracheal inflow of water condensed in the circuit would be one of the major risk factors for VAP. It is important to prevent VAP that the breathing circuit and the associated devices are kept clean. In the present study, we investigated the efficacy of a breathing system filter incorporated into the breathing circuit of mechanical ventilator. The breathing system filter DARR Sterivent was used as a microorganism removal filter, which was placed between the expiratory part of the circuit and the moisture trap of Servo Ventilator 900C. Bacterial examination was carried out at the pre-filter portion of the expiratory circuit and at the post-filter portion from the moisture trap to the silicon rubber valve in 17breathing circuits used. As a result, the number of bacteria detected at the post-filter portion apparently less than the pre-filter portion (mean value, 142.6 vs. 1,135.1). However, considerable bacteria were detected at the cup of moisture trap, which was located even at the post-filter portion. In addition to setting the filter, periodic cleaning of the moisture trap would contribute to hygiene of the ventilator respiratory circuit, resulting in reduction of VAP.

An internet-based system was developed for improving care efficiency, which consisits of Internet client computers with liquid crystal tablets, wireless Internet Java mobile phones and a central office desktop server computer. The care items for one month are scheduled by the care manager ; however, the care items are often shanged by request from the client, and the Home Helpers need to know these changes as soon as possible. The liquid crystal tablet is used as an entry device for the client's care requeses. The client computer sends the requests to the server computer in the Home Helper central officd. The server computer automatically creates the request items as a file, and then transfers them to the care manager and Home Helper's mobile phone. The developed system enables Home Helpers to improve their care efficiency. Moreover, care-requiring clients can easily operate the liquid crystal tablet and send their requests to Home Helper and care manager via the sarver computer.
