    BACKGROUND & AIMS:It is considered that the Hb index(IHb)is correlated with themucosal Helnoglobin derlsity. The aim of this study was to investigate the use of the IHb to detect H. pylori infection. METHODS:Firstly, we estimated the best condition of the IHb. Secondaly, we evaluated the relationship between the IHb, histological examination and mucosal blood flow. Considering the basal estimation, we investigated the IHb in 30 patients. RESULTS:We found the most suitable photo-condition was within 10mm distance from gastric mucosa. And the IHb was correlated with the density of the micro-vessels in the gastric mucosa using immunohistochemistry, but IHb did not depend on the mucosal blood flow. In addition, we determined the cut off index of the IHb was 62 for diagnosis of H. pylori infection. We showed the sensitivity was 93.3%, specificity was 80%and that the accuracy was 86.7%. CONCLUSIONS:IHb was simple and available method for the diagnosis of H. pylori infected gastric mucosa.

    A 74-year-old womLan with Basedow's disease, who had been followed up since Jurle, 1999, visited our hospital with a complaint of abdominal discomfort in December. Endoscopic examination revealed severe atrophy in the upper body of the stomach. Laboratory data showed high level of serum gastrin and positive parietal cell autoantibody. The diagnosis of autoimnune gastritis was made. A close association between Hashimoto's thyroiditis and autoimmune gastritis has been reported as thyrogastric disease. We reported a case of thyrogastric disease(variant type)associated with not Hashimoto's thyroditis but Basedow's disease.

    A 49-year-old male complaining mainly of abdominal pain and diarrhea was admitted into our hospital. Abdominal computed tomography and ultrasonography revealed thickening of the intestinal wall of the terminal ileum. Colonoscopy demonstrated an ulcer at the upper lip of the ileocecal valve, and the worm was observed in its vicinity. Therefore, we resected the worm endoscopically using a biopsy forceps. History taking clarified that the patient had eaten mackerel sushi before the onset of the disease. Examination of the resected worm led to the diagnosis of Anisakis simplex, and the level of Anisakis IgE antibody was more than 100UA/ml. The clinical manifestations disappeared after removal of the anisakis. One week later, Colonoscopic exarnination showed healing of the ulcer. Nonetheless, the level of the Anisakis IgE antibody was still elevated even after elapse of six months.

    A 43-year-old man with ulcerative colitis(UC)has been treated with salazosulfapyridine(39/day)for the past 13 years since the initial diagnosis. He had been in a favorable course, However, a follow-up barium enema stu.dy in July, 1998 revealed pneumatosis cy$toides intestinalis(PCI)along the ascendillg colon and hepatic flexure. Only g cases of PCI associated withUC were reported in the last 12 years in Japan. Eight of these g reported cases(89.9%)had PCIlesions along the ascending colon or hepatic flexure. We thought that the right-sided colon isapredilected area in PCI associated with UC. More case studies are required to clarify thisClinical irnplication.

    A 76-year-old man who had been medicated as myelofibrosis was poirlted out the gastroesophageal varices on endoscopic exarmination. He was hospitalized and underwent the endoscopic injection sclerotherapy (EIS) four times The varices dissapeared without complications such as bleeding and stenosis. We suggest that the EIS is a very useful treatment for varices resulting from an unusual mechanism.

    A case of choledochocele associated with common bile duct stones treated by endoscopic sphyncrerotomy is presellted. Patient was a 76-year-old man with past history of acute pancreatitis 22 years before. Upper gastrointestinal endoscopy revealed an elevated lesionsimilar to submucosal tumor in the second portion of the duodenum. Hypotonic duodenography showed a smooth-surfaced elevated lesion, 18×20 rnm in size, on the inner side of the middle second portion of the dtlodenum, which was thought to be an enlarged papilla of Vater. EUS showed a eystie mass in eentact with the eommon bile duet located inside the wall of the duodenum which contailled a stone with varied shapes. Duodenoscopy showed a smoothsurfaced semi-spherically enlarged papilla of Vater, which shape was varied according to contraction and relaxation of the sphincter of the papilla. ERCP showed a cystic dilatation of the terrninal portion of the common bile duct protruding in the duodenal lumen of the duodenurn, contailling a stone. Under the diagnosis of choledochocele associated with common bile duct stones, EST was performed alld stones were removed. ERCP about 2 years and 6 months after EST showed the cystic dilatation of the terminal portion of the common bile duct to have disappeared and neither stolle nor rnalignant change was detected. Biopsy specimell from thecavity of the choledochocele showed normal duodenal mucosa. When an enlarged papilla of Vater is found, choledochocele should be kept in mind as well as tumor of the papilla and common bile duct stones.

    Case l was a 66-year-old woman who cornplained of heartburn. Endoscopic examinationrevealed a DolvDoid lesion at the esophago-gastric junction. In our hosbital, she underwent polypectomy of the polyp and it was entirely removed Pathological examination of tlhe specimen. revealed pyogellic granuloma. Case 2 was a 53-year-old man who complained of heartburn and epigastric pain aftermeals. Endoscopic examination revealed a polypoid lesion, which was covered with a whitecoating on the top, in the middle esophagus. Pathological exarnination of the biopsy specimensrevealed pyogenic granulorna. Twenty-one days later, we tried to perform polypectomy, butthe polyp had completely disappeared. Pyogenic granulorrla, benign and elevated lesion whichhas a tendency to hemorrhage, is often recognized in the oral mucosa and skirL However, thereis little report about pyogenic granuloma localized in the gastrointestinal tract. We report twocases of pyogenic granulorna in the esophagus.

    Background and aims : Endoscopic lithotomy and surgery for the confluence stone are difficult. We studied the efficacy of endoscopic lithotomy for confluence stone. Methods : Endoscopic lithotomy was performed in 15 patients. We also studied the coexisting diseases, the success rate of endoscopic lithotomy, the period of treatment and the frequency of treatment. Result : Stones were detected in the intrahepatic duct in two patients, and the carcinoma of the gallbladder was detected in one. The stones could be removed in all cases. The period of treatment was 12-52 (average 26.8) days and the procedure was performed in 4-9 (average 5.9) sessions. In eleven cases, there were also stones in the gallbladder, which could be removed during the same period in six cases. EHL enables the crushing of huge stones not removed by basket forceps and not crushed by ESWL. Conclusion : We conclude that confluence stone can be removed by selection of appropriate endoscopic lithotomy.
