    Forty cases with tongue-, or floor of the mouth-, or mandibular gingival carcinoma for which bilateral neck dissection was simultaneously performed, were retrospectively examined including T stage and histopathological grade of the primary lesion and clinicopathological features of the metastatic lesion.
    Both ipsilateral- and contralateral sides had pathologically positive nodes in 11 cases (pN (+) (+) ), only the ipsilateral side had the nodes in 14 cases ( pN (+) (-) ), only the contralateral side had the nodes in 2 cases (pN (-) (+) ), and both sides did not have nodes in 13 cases (pN (-) (-) ) . In most of the pN (+) (+) cases, the primary lesion belonged to T3 or T4 and showed high pathological malignancy grading and the number of metastatic lesions in the ipsilateral side was more than 3 and the metastatic lesions were located in level III or IV and extracapsular spread.

    Recently, multiple malignant tumors in patients with oral cancers have increased. About 10% of patients with oral cancer have multiple primary cancers in other organs. We report on a case in which we conducted radical surgical treatment for multiple primary cancers in advanced stage occurring in the lower gingival and stomach.
    An 80-year-old man was referred to our department because of an ulcer of the left lower alveolus. A biopsy showed well-differentiated squamous cell carcinoma. A stomach cancer (poorly-differentiated adenocarcinoma ) was also found by upper gastro-intestinal endoscopy. The stomach cancer was resected by a general surgeon in our hospital. Next, the cancer in the lower gingival was resected after radiation therapy. Histopathologically, the cancer in the lower gingival was pT4N2b and the cancer in the stomach was pT3N0, both diagnosed as being in the advanced stage. There was no evidence of recurrence and metastasis about 2 years after his first visit to our department.

    In our hospital, a combination of intra-arterial infusion chemotherapy and radiotherapy has been performed to preserve functions and morphology in case of oral cancers. In this study, we report a case in which we performed the combined therapy without surgical therapy at the time of primary treatment, and a good clinical course has been observed up to date.
    The patient was a 67-years-old male who had a 58×50 mm tumor of left tongue on thelef t side.
    During hospitalization, we performed seldinger into the femoral artery, then performed chemotherapy with CBDCA (total dose 900 mg), and radiotherapy (Linac, total dose of irradiation : 40 Gy) .
    After treatment, the tumor disappeared, and therapeutic effect was evaluated as complete response based on functional imaging diagnosis using positron emission tomography. The patient is under maintenance therapy, and no recurrence or metastasis have been observed during the 3 years since treatment.
