Among the medicolegal autopsies at Iwate Medical University conducted over the five-year period from 2017 to 2022, it is surveyed the 103 cases of unidentified bodies discovered in the premises of their homes multiple days after their deaths, in order to understand the contemporary social problems associated with solitary deaths. An overwhelming majority of these cases were elderly men. A total of 50 cases were estimated that the time since death is for over one month. In 88 cases, the deceased lived alone, while in 15 cases, they lived with family. The details of the individuals’medical history were unknown in 49 cases, and post-mortem changes made determining the cause of death from the autopsy results difficult in 97 cases. The dental history was also often unknown, making the collection of ante-mortem dental information for the identification exceedingly difficult.

The number of single-person households among the elderly is on the rise, therefore it is in this context that home medical care and welfare activities have been promoted in recent years; however, as the results of this survey suggest that the welfare services failed to reach individuals who refused any social interactions in their old age.
