    Postoperative (30 to 3361 days) clincial observations were made on 200 cases of 20 carat inlay restoration on young permanent first molars.
    The inlay restorations were examined clinically by means of an explorer and colour photography.
    From the results of this experiment, the following conclusions were obtained.
    1) Changes observed in inlay restoration: Discolourlation: 6 cases (3%), surface roughness: 5 cases (2.5%), and dislodgement of inlay: 6 cases (3%).
    2) Changes observed in tooth structure: Tooth fracture: 7 cases (3.5%), recurrent caries: 11 cases (5.5%), and newly formed caries (except recurrent caries): 34 cases (17%).
    3) Since young permanent first molars are highly susceptible to decay, for the purpose of perventing recurrent caries and newly formed caries, new methods of extention for pervention and improvement of cavity form for gold inlay restoration are advisable.

  • II. Evaluation of Acidogenicity of Lactose and Microbial Analysis of the Plaque on the pH-Electrode

    The purpose of this study was to evaluate the acidogenicity of lactose from the pH curve after lactose exposure on a pH-electrode covered with plaque, and to find out the difference between the acidogenicity in children's and in adults' plaque. To analyse statistically the pH-curves, plaque-pH was monitored 5 to 10 times on 3children and 3 adult volunteers respectively, and then 4 parameters (minimum pH value, returning time to pH 6, initial acid production activity and the area between the pH-curve and pH 6 level) were evaluated.
    Furthermore, the plaques accumulated on hydrogen-ion sensitive filed effect transistor (pH-ISEFT) and enamel were analysed bacteriologically.
    The following conclusions were obatined.
    1)Data obtained from the four parameters indicate that acid production from lactose was lower than that from sucrose, but not statistically significant, except for acid production obtained from initital acid production activity. Also, except for initial acid production activity, three parameters showed considerable difference among subjects.
    2)Acidogenicity (especially in minimum pH value and initial acid production activity)increased with age in plaque over six days (2-,4- and 6-days), and a marked increment of acidogenicity was shown between 2-days and 4-days.
    3)With increasing lactose concetration (1,5 and 10%) acidogenicity (in minimum pH value and the area between the pH-curve and pH 6 level) increased, but initial acid production activity did not change.
    4)Acid production in children's plaque following the application of lactose or sucrose did not differ from that in adults' in minimum pH value and initial acid production activity. However, the returning time to pH 6 was shorter and the area between the pH-curve and pH 6 level in children was smaller than that in adults.
    5)There was no difference in bacterial composition between 4-day-old plaque accumulated on pH-ISFET and on enamel.

    The purpose of this study was to investigate the relationship between the Bacteroides melaninogenicus group and the Bacteroides fragilis group and the clinical findings of the periapical infected primary teeth, and to detect these Bacteroides groups from the oral cavityies of children. Bacteroides groups were determined with a fluoerscent anti-body test kit (Fluoretec-M and Fluoretec-F, Pfizer, Inc., U. S. A. )
    The results are as follows:
    1. The detection rates for the B. melaninogenicus group from closed, opened and root canals under treatment were 66.7,61.3 and 33.3%, respectively. There was no statistical relationshp between the existence of B. melaninogenicus group and the clinical findings (abscess or fistula formation, history of pre-endodontic therapy and root or alveolar bone absorption) of the periapical infected promary teeth.
    2. B. melaninogenicus group detection rate was high from the dental plaque on the primary and permanent tooth surfaces of the school chidren, but low from that of the infants. The detection rate from the coating of the tongue, cheek mucosa, gingival abscess and saliva of the chilaren was 84.6,20.0,20.0 and 9.0%, respectively.
    3. The B. fragilis group had a high detection rate from closed infected root canals of the primary teeth, the coating of the tongue and dental plaque from children. Furthermore, the B. fragilis group existed in all canals of the primary teeth with acute periapical infection.

    The subjects were a total of 33 children (4 to 8 years of age), of which 23 were kindergarteners, and 10 were school age children. They had previous dental experience.
    Changes in internal behavior were observed when the subjects were auditorily stimulated by the sound of air, vacuum, engine, turbine and crying. The change ratios of resistance in stimulation were analyzed by galvanic skin reflex (GSR).
    The change ratios to the auditory stimuli were compared with the sequence of response, sex difference and age difference.
    The results obtained were as follows.
    1) There was no significant difference between each auditory stimulation on kindergarteners and school age children.
    2) Comparing sex groups of kindergarteners and school age children, there was no significant difference on each stimulation.
    3) Comparing age groups, kindergarteners showed larger change ratios than school age children by all stimuli except the sound of crying.

  • A comparison with the 7-years-ago survey in the same district

    In order to know th relationship between the decrease in the prevalence of caries and the changes in daily habits, especially dietary habits in young infants, we conducted an investigation at the dental health examination of one and a half year old children at Nakahara Health Center in Kawasaki City in 1982. We compared the results of this survey with a similar one conducted 7 years ago in the same district.
    The results obtained were as follows.
    1. The caries prevalence rate decreased about 7 %, and mean decayed teeth pre child decreased about 0.3 teeth in these 7 years. In particular, the caries in cidence rate of labial surfaces of upper incisors decreased by half.
    2. According to the questionnaires to the mothers in this survey, there was a significant difference in the finishing time of weaning and the nursing habits in bed between the decayed teeth group and the no decayed teeth group. However, there was little difference in the eating and drinking sweets between meals habits.
    3. In the decayed group, there was about a 20 % decrease in the rate of children nursing in bed between this and the last surveys, but there was little difference in the no decayed group between the two surveys.
    4. In the decayed group, the rate of children eating sweets between meals decreased by half, to the same rate as children in the no decayed group which had had little change in these 7 years.
    5. From these two surveys, it is suggested that the main factors which contributed to caries reduction in labial surfaces of upper incisors were the continuation of breast feeding or using nursing bottles after 1 year, and especially the habits of nursing in bed, and of eating sweets between meals.

    472 oral examination papers and 573 questionnaire papers on first-visit patients who visited the Pedodontic Clinic of Tokushima University Dental Hospital during the period from October 1977 to September 1982, were investigated.
    The purpose of this study was to understand the actual condition of the patients who visited the hospital and to succeed in the prevention of dental caries in our clinic and locality.
    The results showed a greater prevalence of carious deciduous teeth than the mean df teeth numbers for Japanese children, especially 2-4 years old, higher rates of untreated deciduous teeth, the higher rates of irregular suckling during the suckling period, delayed bottle feeding habits and irregular intake of betweenmeal snacks, and that 38.4% of the mothers did not know how to prevent dental caries.
    Therefore, to effectively improve the oral health status of children, the introduction of preventive guidance, including dietary habits and oral hygiene habits, is most required.
    Most of the mothers of the parients requested long-term oral health care for children including prevention of dental caries, periodontal disease and guidance in developing occlusion.

    This histopathological study was undertaken to compare the pulpal response to one composite resin for posterior teeth from A company with that of another from B company, using the primary teeth of young dogs.
    The results were as follows:
    1. There was no significant difference between the two compostie resins. In the 3-day postoperative period, an initial inflammatory reaction elicited from cavity preparation was observed, but this response disappeared with time, and in the 21-day postoperative period, reparative dentin formation was displayed in all teeth.
    2. No significant difference between the pulpal responses of the 2 products was shown if the remaining dentin thickness was greater than 100μ. However, if it was less than 100μ, it appeared that pulpal irritation of the composite resin from A company was slightly less.

    We present a case in which a compound odontoma was found on E resion which disturbed the eruption of a permanent second premolar in a ten-year-old girl.198412251. The case was diagnosed as compound odontoma based on histopathologic and scanning electron microscopic investigations.198412252. The site of occurrence, at the maxillary premolar region, is rare for compound odontoma.198412253. After surgical removal of the odontoma, a space regainer was set and 5 erupted successfully to its normal position after eight months.

    Neurinoma is rare in the oral region. We report a case of neurinoma in the left side of the tongue observed in 11 year 7 month old girl.
    The tumor was exposed under local anesthesia and enucleated en bloc easily. The histological diagnosis was neurinoma.
    The postoperation course has been satisfactory without recurrence and with good movement of the tongue.

    1. The anthors present a case of Cornelia de Lange syndrome in a four years old girl.
    2. Since the general, facial and oral findings, and musculoskeletal alterations of this patient were found to coincided with symptoms and signs of typical Cornelia de Lange syndrome, this case was diagnosed as such.
    3. Intraorally, highly arched palate, bifid uvula, and ankylosed labial frenum were also noted.
    4. Radiographic examinations showed microcephaly with enlarged dorsum sella, smallness of jaws, hypoplasia of the first metacarpal and fifth middle phalanx, samllness of extremities, and delayed bone age.

    The purpose of this study is to investigate the real situation of handicapped persons at a private dental clinic in a province. The second Ghoke dental clinic is a general dental surgery practiced in the central part of Sendai city in the Tohoku district, in whcih two dentists work and approximately 66 patients visit in a day. The investigation was carried out on 491 handicapped patients (7.2 % of the all patients) treated at the clinic during the four years period from June 1979 to May 1983.
    The disabilities were mental weakness, physically handicapped persons, including cerebral paralysis, and visual and auditory disabilities.314 (64.0 %) were patients with mental weakness.60% of the handicapped were men. The age range of the handicapped patients was 4 to 92 years, with 358 (72.9%) of them under the age of 19 years. The average number of times to visit the clinic was 6.1. The majority visited under 5 times. The average duration for visting the clinic was 1.7 month. The treatment for approximately half of them (47.7%) was completed in a month.19% of the patients were living in facilities for handicapped persons. In linear distances from the patient's residences to the clinic,59 were within 1 km to 2 km,58 in Sendai city over 5km, and 44 outside the city, etc. This clinic had comparatively many patients coming from long distances. The average number of treated teeth was 4.0 and the treatment was composed of fillings,2.2, extraction and pulpectomy,0.8, and root canal treatment,0.2.

    There are many case reports of supernumerary teeth, but rarely in the primary dentition. We found two cases of genuine supernumerary teeth in the Upper primary incisal region.
    There were no special problems about family history, case history and present condition in the two cases.
    Dental formula of Case I

    The patient is A 4 year 1mnth girl. We consider the Upper left primary canine teeth (C) to be supernumerary teeth.
    Dental formula of Case II

    The patient is A 5 year 1month girl. We consider the Upper left primary lateral incisal teeth (B') to be supernumerary teeth.
    From the position, form, X-ray examination etc, we decided these were genuine supernumerary teeth.
    As dental problems, the necessity of oral prophylaxis and denture guideanc about the site of supernumerary teeth was suggested.

    Long term observation and oral rehabilitation were made on the case of an ectodermal dysplasia with almost all teeth missing except for two teeth, upper left deciduous 2nd molar and right permanent canine. Electromyographic evaluations of the masticatory muscles following oral rehabilitaiton were also performed.
    The results obtained were as follows:
    1. Some functional disturbances, such as tongue thrusting, abnormal mastication and swallowing, were observed in the patient. However, these were improved considerably following myo-functional training.
    2. By wearing dentures, masticatory rhythm, synchronization on clenching, and EMG activity of the masticatory muscles of the patient became much balanced.
    3. Silent periods were noticed in the electromyograph, during tooth tapping after wearing dentures.
    4. Compared with the normal children, little difference was noticed in the case on the occlusal forces after placement of the dentures.

    A 6 year 10 month-old boy was refered to the Pedodontic Clinic of Hiroshima University School of Dentistry with the chief complaint of delayed eruption of the mandibular right second deciduous molar. He was born of a normal pregnancy and delivery.
    Consultation with a pediatrician revealed that the child's development was normal and that he was very healthy at present.
    The principal observations are as follows:
    1) An impacted mandibular right second deciduous molar and incisula at gingiva on the alveloar crest of that region were found.
    2) Radiographic findings showed that the roots of the mandibular right second deciduous molar were almost formed. A radiolucent lesion due to caries was found at the mesial and occlusal of the dental crown.
    3) The roots of the diseased tooth were fractured at extraction. Caries were found at the mesial and occluasl of the dental crown.
