The monoamine deficiency hypothesis in the depression disorders (MDD) has made a significant contribution to understand the pathophysiology of MDD and to develop new antidepressants. On the other hand, there is a large time discrepancy between the pharmacological effects for monoamine hypothesis reasons and the clinical effects on a clinical field with treated antidepressants. Therefore, this hypothesis also have limitations. Brain-derived neurotrophic factor (BDNF) has an important role to neuroplasticity and neurogenesis. Several studies reported that peripheral BDNF was significantly lower in the antidepressant-naive patients than in the treated patients with MDD. In order to better understand the role of BDNF in MDD, we compared the changes in plasma BDNF levels in remission and non-responder groups of patients with depressive syndrome. Patients with depressive syndrome who underwent remission had significantly higher plasma BDNF levels (p<0.001). In contrast, non-responders had significantly lower plasma BDNF levels (p=0.029). We suggest that plasma BDNF could potentially serve as a prognostic biomarker for depression, predicting clinical outcome.

Introduction : Current symptoms of eating disorders have evolved into a free floating and non-stylized form. We would like to determine differences that exist between generations ; therefore, our study compared the pathology of cases over several decades. Purpose : We performed psycho-pathological studies for inpatient cases with eating disorders from each decade (2nd-7th). Subjects and Methods : Ten female patients, with symptoms compatible with the diagnostic criteria for eating disorders DSM-IV-TR, were admitted to our hospital between January 2012 and the end of December 2012. These patients were evaluated, and we examined the characteristics of symptoms of cases between 10 years old and 60 years old. Results : Ten additional cases involving female patients were eliminated from the study. In a majority of cases, the age of onset was in the teens. All patients' symptoms had developed from anorexia nervosa (restricted type) ; four cases remained in restricted type ; five cases evolved into the binge-eating type. Patients with anorexia nervosa (binge-eating emission type) experienced numerous hospitalizations, and the duration of their disease was prolonged. Discussion : Eating disorders were triggered by some types of stress present in modern changing lifestyles. For chronic cases, whose main concern is body style management, anorexia nervosa is a type of defense mechanism and a form of withdrawal from social interaction. Moreover, some chronic cases, whose symptoms were altered through the use of CBT or other therapeutic approaches, became increasingly worse.

Eating disorders have various psychiatric comorbidities such as anxiety disorders and personality disorders. Pervasive developmental disorders (PDD) have become to attract attention in recent years. According to previous reports, comorbid rate is 10-20%. Initially, obsessive behaviors about food are pointed out a similarity between eating disorders and PDD, but the differences between the two are discussed in neuropsychological perspectives in recent years. Since the problem of interpersonal contact and obsessive behaviors are also observed when eating disorders are severely emaciated, its clinical figures are similar to those of PDD. Therefore, a longitudinal perspective, including the history of development from childhood is necessary for the diagnosis of the comorbid case. In the treatment of comorbid cases, in addition to the treatment of eating disorders, a nurturing and educational approach based on the characteristics of PDD is required.

In recent years, women's societal roles have expanded, and there has been an increase in women working outside the home. In today's society, women have more choices for self-actualization, but, on the other hand, their psychological stress appears to have become stronger due to the fact that they are required to work flexibly and establish desirable interpersonal relationships in the workplace. Along with this, female patients with eating disorders have also increased, and, accordingly, it has become common for them to address occupational stress and the attitude toward work at clinical settings that deal with the disorder. In this study, we first examined women's current employment status and changes in their social environment. Next, we summarized the results of previous studies on the occupational stress of eating disorder patients consulting medical centers. Finally, we considered specific measures necessary for working patients with eating disorder, and challenges to their implementation from the viewpoints of both attending and employer, while reviewing the author's previous patients.

Clinically it seems to be clear that recently the age of patients with eating disorders (ED) is becoming older, but we have almost no data about it. We investigated age, clinical symptoms, behaviors and psychosocial factors with ED in Kohnodai Hospital. The number of inpatients with ED increased from 40 in 1998 to 110 in 2008, and especially anorexia nervosa binge-eating/purging type (ANbp) increased from several in 1998 to about 60 patients in 2008. The rise of mean age in inpatients also was about ten years from 1998 to 2008 in ANbp, but not in anorexia nervosa restricting type (ANr). With regard to clinical symptoms, behaviors, and psychosocial factors, elder ED patients showed some differences compared with young ED patients or non ED outpatients. These data suggest that elder severe ED patients have increased in number in recent years.
