Heart rate variability (HRV) in 7 anorexia nervosa (AN) patients, 10 thin-normal girls and 10 normal physique girls on 24-hour was analyzed by speciral analysis in MemCalc system, and autonomic nervous function was then analyzed. We calculated high frequency (LF : 0.15-0.5 Hz) which represents parasympathetic activity, low frequency (LF:0.04-0.15 Hz) which represents sympathetic activity including parasympathetic activity,and LF/HF. The power spectral value in the HF was highest, and that of in the LF/HF was lowest in the AN among 3 groups. Those findings suggest that the difference of autonomic nervous function in the AN is the cause of the disease itself and is not due to the thin physique.

How to manage the white-coat hypertension (WHT) has been at issue in recent years, and there is no concord on how to treat it. In our previous study, we reported that EEG biofeedback therapy (BF therapy) is more useful in WHT patients under anxiety state than in patients of sustained hypertension. Hmvpver, it is not clear yet whether these favourable effects of BF therapy on WHT are due to the BF therapy itself or due to the other factors, such as the amendment of life style and what the real mechanism of the effect of BF therapy is, ln this study, therefore, clinical courses of WHT were followed for about 10 months and compard between 2 groups, one is WHT with BF therapy (n=29) and the other is WHT without Bftherapy (n=25). The results showed an improvement of blood pressure in out-patient clinic, a decrease of plasma noradrenaline levels and a rise in the α-wave appearance rate in patients with BF therapy, as compared to patients without BF therapy. And also improvements of mental state based on mental tests (Cornell Medical Index, State Trait Anxiety Inventory, Self-rating Depression Scale and Tokyo University Egogram) such as inadequacy, tension, state anxiety, depression, stressful state, were observed in natients with BF therapy. Lt was concluded, from these findings, that BF therapy is useful for WHT, probably through the improvement of mental state.

In psychosomatic treatment, the improvement of clinical symptoms is related to therapeutic factors : intratherapeutic factors such as trust and reassurance towards a physician, expectation towards treatment including drugs and extratherapeutic factors. In this study we evaluated what such therapeutic factors contributed to the improvement of clinical symptoms with a short questionnaire, and how the outcomes are different depending on the experience of physicians. We surveyed all of the 336 psychosomatic outpatients who had been treated by three physicians in our hospital with a questionnaire after the consultation, all the patients were asked whether or not their symptoms improved and what factors were related to the improvement of their subjective symptoms. About 90% of patients reported on improvement in clinical symptoms inclusive of "slightly improved" and better response, which was not different in terms of physicians' experience. The major therapeutic factors regarding one physician with advanced experience were "trust and reassurance" (27%), "specific explanation" (20%), "suggestive words used by physician" (14%), "hope for recovery" (11%) and "drug" (9%). On the other hand, those regarding the other physicians with less experience were "trust and reassurance" (25%), "specific explanation" (18%). "drug" (14%), "hope for recovery" (11%) and "suggestive words used by physician" (11%). Also, the primary therapeutic factor regarding one physician with advanced experience included "trust and reassurance" (38%). "specific explanation" (23%), "events irrelevant to treatment" (11%), "suggestive words used by physician" (8%) and "drug" (8%). On the other hand, the primary therapeutic factor regarding the other physicians with less experience included "trust and reassurance" (27%), "drug" (20%), "specific explanation" (18%), "suggestive words used by physician" (8%) and "events irrelevant to treatment" (7%). In any case of physicians, "trust and reassurance" was the most important factor contributing to the improvement of patient's symptoms, and the second was "specific explanation". However, there was a different efficacy of "drug" among the physicians depending on experience. It was suggested that extratherapeutic factors, expectation and hope also influenced the improvement of symptoms.

We examined clinical characteristics of patients with Bulimia nervosa whose images of father were aggressive. Subjects were 10 females with Bulimia nervosa (purging type) whose images of father were aggressive (Aggressive Group) and 10 females with Bulimia nervosa (purging type) whose images of father were not aggressive (Non-aggressive Group). We compared eating attitudes, mood states, stress coping, self-esteem of Aggressive Group with these of Non-aggressive Group. We used Eating Attitudes Test (EAT), Profile of Mood States (POMS), Coping Inventory for Stressful Situation (CISS) and Rosenberg's Self-esteem Scale. There were no differences of scores of EAT and POMS between Aggressive Group and Non-aggressive Group. Task-oriented coping scores tended to be lower in Aggressive Group than Non-aggressive Group. The patients with Bulimia nervosa (purging type) whose images of father were aggressive tend not to make effort purposefully to solve the problems in stressful situations. Self-esteem scores were significant lower in Aggressive Group than Non-aggressive Group. It seemed that the patients with Bulimia nervosa (purging type) whose images of father were aggressive couldn't respect themselves. Furthermore, we examined experiences of childhood sexual or physical abuse and images of mother of Aggressive Group, Non-aggressive Group and the group of 10 females with other psychiatric problems whose images of father were aggressive. Only in Aggressive Group with Bulimia nervosa (purging type) 3 patients had childhood sexual abuse. And there were many patients whose images of mother were weak in Aggressive Group with Bulimia nervosa (purging type).
