    High dose hydralazine hydrochloride, isoniazid, procainamide and phenytoin sodium were respectively administered for long term and subsequently heat denatured (00°C, for 30 minutes) calf thymus DNA was added to dd-mice. Their relationship of ANF, proteinuria and renal immunohistological findings was investigated. The results are described as follows : 1) Frequency of ANF was low by drug-administration and increased with addition of DNA, and it was statistically significantly (P=2.8%) different between drug-DNA-added and drugdiscontinued-DNA-non-added groups. But ANF were produded too by administration of DNA only. 2) In renal immunofluorescent study, γ-globulin was localized in only mesangial area, although not a little, it was recognized in controls (distilled water group) too. But mesangial localization of γ-globulin was more remarkable with drug-administration and seemed to decrease after drug discontintinuation. No effect of DNA was recognized. 3) Mesangial localization of γ-globulin was not related with ANF. r) It was not particular about proteinuria and light microscopic findings.

    The effect of an angiotensin II antagonist, 1-Sar, 8-Ile-Angiotensin II, on blood pressure has been studied in 77 patients with various types of hypertension allowed ad lib intake of dietary sodium (Na excretion 200-350 mEq per day). The spectrum of responses was broad, ranging from a mild elevation to a marked fall in blood pressure. The change in mean blood pressure during this peptide infusion showed a significant correlation with the level of peripheral plasma renin activity immediately before the infusion, irrespective of the types of hypertension. This peptide reduced blood pressure in 18 responders, but not in 59 nonrespoders. Our retrospective study in 18 patients with severe cardiovascular complications showed that in 12 responders, propranolol reduced blood pressure to near normal, while in 6 nonresponders, furosemide and other antihypertensive drugs were effective in lowering the blood pressure. The present data show that the effect of this peptide on the blood pressure correlates with the level of peripheral plasma renin activity and that the infusion peptide has many advantages inscreening hypertensive patients and choosing effective antihypertensive drugs.

    We investigated the effect of administration of NaHCO3 into patients with metabolic acidosis and the experimentally produce acidotic rats on the acid-base balance both in extra and intracellular space of the skeletal muscle.
    1) The distribution space of the infused HCO3- was estimated as 40% of the body weight, using the following formulae HCO3 needed=Base Excess×Body weight×0.4. However, in severe acidosis a larger distribution space should preferably be used.
    2) The change in the distribution space of intused HCO3- was well correlated with decreases of both plasma HCO3- and muscle cell pH.
    3) Rapid correction of metabolic acidosis with NaHCO3 occasionally results in an aggravation of intracellular acidosis of the muscle.

    We tried to use a new nonsteroidal antiinflammatory drug "Azapropazone" (Cinnamin(R)) for 35 cases of various forms of renal diseases (chronic glomerulonephritis, nephrotic syndrome, chronic pyelonephritis and Alport syndrome). As a result of this new treatment, "Azapropazone" was ef-fective in 91.4 % for albuminuria and was in 38.2 % for hematuria which indomethacin was noneffective. Also, "Azapropazone" was effective in 6 cases among 7 cases for albuminuria of nephrotic syndrome which was non-effective on the steroid therapy. Consequently, hypoalbuminemia and hypercholesterolemia restored to normal value. And for hematuria of this group, "Azapropazone" was effective in 4 cases among 6 cases. However, after discontinuance of administrtion, albuminuria and hematuria became again to positive. Renal function during administration of "Azapropazone" showed increase, no change or decrease by case and by dosis. Decreased renal function restored after discontinuance of drug. Therefore, during administration of "Azapropazone" for renal diseases, Blood urea nitrogen, serum creatinine, serum potassium and renal function must be checked, necessarily. As side effect, exanthen and epigastrial discomfort were found in a few cases, but liver function damage and gastroduodenal ulcer that are caused by indomethacin, were not found.

    A 23 years old woman, diagnosed as chronic glomerulonephritis two years before by renal biopsy, showed an abrupt converersion from normotensive state to uncontrollable hypertension during hemodialysis treatment. Large amounts of sodium (approx. 4, 000 mEq/month) were removed from the patient's body by dialyses. This caused a hyponatremia accompanied by an increase in PRA and “R”; the latter change might be an important factor to provoke the malignant hypertension. Hypotensive drugs and β-blockers were intensively administered, but no improvement was obtained.Pronounced prostration and severe retinopathy supervened. To save her life, bilateral nephrectomy was done. Postoperatively, PRA rapidly returned to normal, visual disturbance improved, and prostration ameliorated. The sequence of events might be interpretted as follows: 1) Excessive removal of sodium from the body stimulates the release of renin. 2) When the stimulations are repeated for a certain period of time, the releasability of renin (“R”) is progressively increased. For some unknown reasons, the removal of sodium through the dialysis is also accelerated, establishing a vicious circle. 3) With the establishment of the vicious circle, self-perpesuating malignant hypertension arises.

    Ten patients with malignant hypertension were treated by hemodialysis in our hospital. Diagnosis of the disease was largely based on the criteria proposed by Volhard and Fahr. Six of 10 patients had the clinical features of the disease before the start of dialysis, and was called “primary malignant hypertension” (group I). On the other hand, the remaining 4 were called “secondary malignant hypertension” (group II), because they developed the malignant stage after the start of dialysis. Furthermore, 6 patients maintained by dialyses without malignant hypertension were studied as a control group (group III). Characteristic clinical features of the disease were abnormally high blood pressure with hyperreninemia, increased cardiothoracic ratio, raised serum lactate dehydrogenase levels, advanced changes of eye grounds and abnormally low serum sodium concentration. Concerning the PRA and sodium balance, they showed elevated resting PRA, exaggerated removal of sodium from the body during dialysis and abnormally high “R” (“R” = PRA ratio/ removed sodium/ watei ratio ×100 = 1/ mEq/L %). From these results, it seemed likely that the pathogeneeis of malignant hypertension might be an exaggerated removal of sodium from the body and an elevated “R”. The methods and limitations of the dialysis treatment were also studied.
