    This report concerns a 15-year-old Japanese girl with hyperinsulinism whose fasting serum immunoreactive insulin (IRI) levels were invariably within normal range. She developed frequent seizures in association with brief fasting at 9 months of age. The anticonvulsant therapy was instituted without benefit. At 5 years of age, she was admitted to our hospital for evaluation of hypoglycemia. Despite profound hypoglycemia her basal serum IRI levels remained normal, and various endocrine tests failed to demonstrate hyperinsulinemia unequivocally. Diagnosis of hyperinsulinism was suspected by consistently lowered serum non-esterified fatty acid (NEFA) and lack of ketonuria during hypoglycemia, and was confirmed by a dramatic response to diazoxide therapy. During the past 9 years, she has remained asymptomatic with fasting blood sugar of 74-87mg/dl on diazoxide (2.5mg/kg/day).

    To evaluate the medical effects induced by the cessation of cigarette smoking, the subjects who had been followed up in a financing company were analyzed epidemiologically. The duration of the observation period was 2 years. Two health surveys were performed for each subject. The number of nonsmokers, ex-smokers and smokers were 117, 38 and 379, respectively. The results based on the analysis of covariance using the age and value of each medical indicator measured at the first health surveys as the covariates showed that the obesity index, diastolic blood pressure and the serum level of uric acid were significantly higher in the ex-smokers than in the nonsmokers and the smokers (p<0.05). Fasting blood sugar, triglyceride and total cholesterol in the ex-smokers who had smoked 20 or more cigarettes per day (N=20) were higher than in the ex-smokers who had smoked less than 20 cigarettes per day (N=18) (p<0.05). There were no significant differences in the medical indicators among the subjects with different durations of cessation of cigarette smoking. The factor analyses con-cerning the dietary habits were calculated. The ex-smokers had significant negative trends in the second factor score and significant positive trends in the third factor score between the first and second health surveys when compared with the nonsmokers and the smokers. These trends showed that the ex-smokers seemed to make their dietary habits unhealthy and to increase their calorie intake. In view of these relationships between the medical indicators and the modification in dietary habits, health education concerning dietary habits should be carried out when cessation of smoking cigarette is recommended.

    The effect of hypoprolactinemia on the ovine fetal growth was investigated. Eight chronically catheterized fetal sheep were used in this study. Blood samples were taken daily for prolactin (PRL) determinations by radioimmunoassay. The animals were randomly assigned to either the control (no treatment) or bromo-criptine (BC) group. BC (1mg, BID) was infused subcutaneously into the fetus for 11 to 21 days until spontaneous delivery occurred. The length of gestation between control and BC groups was similar, 149.5±2.4 and 147.0±2.1 (mean±SEM) days respectively. The plasma level of PRL in the control group increased from 14.3±2.1 ng/ml at 21 days prior to delivery to 41.0±6.3 ng/ml at delivery. In the BC group, the PRL levels were 25.6±9.8 ng/ml before infusion and less than 3 ng/ml after infusion. The mean fetal weight of the BC infused group (3.38±0.34 kg) was significantly lower than that of the control group (4.75±0.17kg) (p<0.02). This study suggests that PRL may play a role in normal ovine fetal growth.

    By random irradiation with a gallium-aluminum-arsenide diode laser, ten cases of unexpected multiple hematoma formation under grafted skin found on the 5th to 8th postoperative day showed marked improvement of survival and color match of the skin graft.
    To present time no side effects have been noted.

    Since 1974 we have treated 118 patients suffering from unilateral and bilateral (two patients) Morbus Menière with intratympanal applications of gentamicin-sulfate. In contrast to earlier attempts using this method, we did not try to destroy the vestibular apparatus, but taking into our considerations the recent findings about the pathophysiological mechanism of Morbus Meniere, only to damage the secretory epithelium thus preventing the endolymphatic hydrops. Relief from symptoms could be obtained in more than 90% of our patients.
