    The complications caused by the implantation of pacemakers have recently deceased, but not all can be avoided yet.
    Two cases of runaway phenomena of pacemakers were found during the early stages of tachycardia, and these pacemakers were replaced by new generators successfully.
    One of the cases was found by transtelephone monitoring. The transmitter that was developed by us was used which transmits ECG and pacing signals simultaneously by a simple acoustic coupler.
    The patient was admitted immediately. After the admission, the pacing rate of the patient had gradually ince ased in a short time.
    This fact suggests that it should not be fitted to define runaway phenomenon by pacing rate only.

    The in vitro investigation of coconut charcoal adsorp tion for the substances was performed. Also direct hemo perfusions were conducted in dogs with renal or liver insufficiencies and then evaluated.
    Beads activated charcoal derived from petroleum is found to be more excellent than activated charcoal. It releases far less pulverized carbon and has adsorption 4-5times over coconut activated charcoal. Beads activated charcoal shows high adsorption for the substances of 100-5000 molecular weights, whereas coconut activated char coal has good adsorption for those of 100-1000 molecular weights.
    The artificial liver is a combination of microencapsula ted Beads Type activated charcoal and a body fluids correction system with special dialysate. Namely the system is multifunctional in that it corrects for the body fluids as well as adsorbs and eliminates toxic substances.

    The experimental replacement of the portal vein with expanded polytetrafluoroethylene tubular grafts (pore size 1 to 2μ) was examined. Twelve of fifteen (80 percent) expanded polytetrafluoroethylene vein prostheses were patent when examined 1 day to 204 days after insertion. In this graft, initial thrombus layer is very thin and a stable fibrinous lining which prevents further fibrin deposits covers the internal surface of the graft within three months. The pores of this graft are too small to permit cellelar infiltration, therefore organization of fibrin deposits through tlese pores can not be seen.

    Inner capsules of Dacron vascular prostheses were examined under both light and electronmicroscopes from 1 to 1184 days after implantation in the thoracic aorta of 60 dogs.
    At an early stage of implantation, it was noticed that inner capsules showed the same transformations as that of the organization of thrombus adhered to the glasses inserted in dog femoral arteries. This healing process of inner capsules was similar to that of a hypothesis of arterial thickening caused by the organization of thrombus on the arterial wall.
    It was observed that the first appearance of the young spherical endothelial cells were found near the sites of suture lines and next on the center region of inner caps ules especially on the bottom of the crimps sporadically. On the center region, endothelial cells covered the surface of inner capsule by increasing their number and getting large and flat in shape.
    These results indicated that the young cells on the bottom areas were originated from the endothelial cells carried by blood stream from the proximal suture line. It may be possible that similar phenomena could be observed on the small Dacron hubs suspended in the axis of knitted crimped Dacron prostheses implanted in the thoracic aorta.

    We intended to remove the free hemoglobin (Hb) directly from the blood circulating system by means of haptoglobin (Hp) which was capable to bind the free Hb.
    For this purpose, soluble Hp was immobilized to the activated sepharose which was used as affinity adsorbent of the free Hb. These sepharose linked Hp were packed in Barrier Filter and attached to the extracorporeal circulation in rabbit. Blood flow rates kept at 15-25ml/min. These results were as follows.
    1) Hb binding capacity of immobilized Hp (0.12-0.26g/g) was less than that of soluble Hp (0.60-0.65g/g). This loss might be due to the denaturation, deformation or inactivation of Hp in coupling phase.
    2) In extracorporeal circulation, free Hb level in serum of experimental group was more reduced about 40mg/dl, compared to control group. Hb adsorbent capacity of sepharose linked Hp was about 100mg/40ml of wet volume.
    3) During the circulation, leucocyte, thrombocyte and fibrinogen were abruptly decreased.
    From these data, we concluted that immobilized Hp was effective to remove the free Hb.

    A new type of capsulated activated charcoal (AC) for direct hemoperf usion was investigated.
    AC has some difficulties when used for direct hemoperfusion, i, e., release of AC powder, damages to cell elements of blood, platelet adhesion, platelet aggregation and so on. To conquer these difficulties, AC was processed as follows. (1) Powdered AC was shaped in beads, and (2) these beads were double-encapsulated by normal gelatin and modified gelatin produced for clinical use as plasma expander. Glutalaldehyde was used as across-linking agent. This modified gelatin coated AC microcapsule (MGAC) could overcome many weakpoints of uncoated AC without any reduction of its efficiency. 600ml MGA C column has very high efficiency compared to any other dialyser. This is really a success of clinically aveilable AC microcapsule.

    In the replacement of Gore Tex graft (expanded polytetrafluoroethylene) to vena cava, the effect of anticoagulants for the prevention of early thrombosis was evaluated.
    Urokinase (fibrinolitic enzyme) and Batroxobin (thrombin like enzyme) were used for anticoagulants in this study.
    In a series of 11 grafts in dogs, anticoagulants was not used. Seven of 11 dogs were died within 1 month. Three dogs that died had patent grafts at the time of autopsy. Four dogs survived over 7 months and were studied by cavography. Three grafts were patent and one grafts was occluded.
    In a series of 10 experiments, urokinase was used. One dog died within 1 month from an uncertain cause. This dog had patent graft at the autopsy. Nine dogs survived over 7 months. All grafts of survival dogs were patent. The patency rate in this group was 100%.
    In a series of 6 dogs, Batroxobin was used. Three dogs was died within 1 month. One dog that died had patent at the autopsy. Three dogs survived over 7 months with patent. The patency rate of this group was 67%.
    The anticoagulation was of value in maintaining pate. ncy of the Gore Tex graft implantation.

  • Further observations of Quinton-Scribner shunt survival

    Recently, arteriovenous fistula is more frequently used than Quinton-Scribner shunt in many dialysis centers in Japan. However, Quinton-Scribner shunt is preferable in patients with limited care dialysis and/or home dialysis.
    We investigated the external artriovenous shunt survival in patients hemodialysed more than three years. The external artriovenous shunt is Quinton-Scribner type shunt and was made in a forearm or a lower leg.
    Frequency of a shunt revision from one extremity to another was 0.36 times a year. Five of seven patients hemodialysed more than six years have had good patent functions in their shunts using the same extremity. Among shunt complications (recannulation, clotting, infection and bleeding), clotting problems were most frequently seen, 1.5 times at the venous side and 0.6 times at the arterial side, in a year.
    In conclusion, we can expect a comparable good survival to arteriovenous fistula in Quinton-Scribner shunt under a daily careful control.

    A stainless steel cored coil electrode was taken out eight years after implantation in a patient with complete heart block. The appearence and mechanical strength of the extracted electrode was unchanged from those before implantation, although minimal degree of electrolysis was suspected by histological examinations and brownishly tinged silicone rubber sheath. A stainless steel electrode was found durable for a long-term cardiac pacing when used in biphasic pulse stimulation.
