    To elucidate the causes of SFD in cases with no maternal disease, such as toxemia, the author studied on the relation of placental cyclic AMP (cAMP) system and feto-maternal gradient of blood amino acid concentrations on the view-point of active transport of amino acids in the placenta.
    The level of cAMP in the placenta was determined by the binding protein method, while adenyl cyclase activity was assayed by the modified Krishna's method and expressed as a conversion rate of 14C-cAMP formation under PG-E1 loading. The activity of placental phosphodiesterase was assayed by the method of Boudreau & Drummond, using theophylline as an inhibitor and imidazole as an activator. The maternal and fetal amino acid levels in human were determined by gas liquid chromatography. Studies were also made on the uptake of 14C-lysine in rat fetuses after loading of theophylline and dibutyryl cAMP on maternal rats.
    The following results were obtained: (1) There was no difference of cAMP levels between the maternal and fetal side of the human placenta. (2) cAMP level in the placenta of normal pregnancy was significantly higher (20.24 p mole/mg protein) than that of SFD (6.18 p mole/mg protein). (3) The rate of placental adenyl cyclase activity in normal pregnancy was 1.97, while that in SFD was 1.26 being classifiable into three types from the figures of dose response curve by PG-E1 loading. On the contrary, the rate of placental phosphodiesterase activity was not significant among normal and SFD pregnancies. (4) Maternal amino acid levels in SFD pregnancy did not differ from that of normal, but the fetus/mother amino acid ratio (f/m ratio) in SFD pregnancy was lower than that of normal, showing a positive correlation (r=0.70) between the placental cAMP level and the f/m ratio. (5) 14C-lysine uptake in rat fetus was accelerated by theophylline loading on maternal rat but no by dibutyryl cAMP loading. In the same manner, cAMP levels in the placenta by theophylline loading on maternal rat increased significantly (1.23 p mole/mg wet weight) than that of non-loaded control (0.45 p mole/mg wet weight).
    These results suggest that the weakening of amino acid transport activity in the placenta of SFD pregnancy with no maternal disease is ascribable to the decreased activity in the adenyl cyclase-cyclic AMP system

    Toxemia of pregnancy after 33 weeks are classified into five types by the following criteria.
    1. Type E : edema only.
    2. Type P: proteinuria (and edema.)
    3. Type PH: proteinuria, hypertension (and edema.)
    4. Type EH: edema and hypertension.
    5. Type H: hypertension only.
    Peripheral leucocyte counts and its analysis, HCG in urine, HPL and Cortisol in serum at 8:00 a. m. are measured. Consequently, type E and P revealed leucopenia, neutrocytopenia, lymphocytosis and low level of HCG, HPL and Cortisol in comparison with normal cases. Contrariwise, type PH, EH and H revealed leucocytosis, neutrocytosis, lymphopenia and high level of HCG, HPL and Cortisol in compari son with normal cases.
    It seems reassnable to conclude that there is some possible participation of cell mediated immunity and adrenal cortex dysfunction in any type of this classification of toxemia of pregnancy

    This presentation defines the value of lower level HCG determination and diagnostic effectiveness of hysteroscopy for ectopic pregnancy. In confirmation of pregnancy, especially atypical ones, usual pregnancy test e.g. Gonavislide, Gonavis, etc. often indicates negative pregnancy. Those negative cases must be further investigated by using a reagent for lower level determination (Hi Gonavis). That is also necessary when we decide the indication of hysteroscopic examination for pregnant patients. And with hysteroscopic findings, e.g. abscence of chorion, decidual-like endometrial hypertrophy etc., we could readily and accurately differentiate ectopic pregnancy from organic lesions and dysfunctional disorders

  • —Report of Four Cases with a Brief Review of the Literature—

    Mixed mesodermal tumor of the female genital tract is a relatively rare and highly malignant disease. In Japanese literature, 67 cases have thus far been reported. In this paper, we added 4 such cases as follows, and discussed the definition, histogenesis, classification and treatment of this disease.
    Case 1. A 54-year old female, gravida 4, para 2, was hospitalized because of a giant pelvic mass which protruded from the fundus uteri and adhered with the transverse colon, descendence colon and sigmoid colon. Supravaginal hysterectomy and left-adnexectomy were performed and followed by chemotherapy. Microscopically, the tumor was composed predominantly of spindle cell sarcoma and admixed rarely with the cartilage.
    Case 2. A 56-year old female, gravida 2, para 2, was hospitalized because of a grape-like tumor which ocuppied the uterine cavity. Radical hysterectomy with pelvic lymphadenectomy was performed and followed by chemotherapy. Microscopically, spindle cell sarcoma and adenocarcinoma were equivalently identified.
    Case 3. A 71-year old female, gravida 6, para 6, was hospitalized because of a cauliflower-like tumor which arose from the posterior fornix of the vagina. She was treated by irradiation therapy. Microscopically, both adenocarcinoma and sarcoma elements were admixed and the cartilage was also identified.
    Case 4. A 49-year old female, gravida 2, para 2, was hospitalized because of an adult head-sized cervical tumor with the metastases to the lung, vagina and external urethral orfice. Simple total hysterectomy, both side adnexectomy and the resection of the tumors at the vagina and external urethral orfice, were performed. Microscopically, undifierentiated adenocarcinoma and spindle cell sarcoma were equivalently identified

    A male baby was born at term. His mother's prenatal course had been uneventfull. His parents were phenotypically normal and there were no cases of genetic or other malformations in their families. The birth weight (2240 gr), the body length (40 cm) and the head circumstance (31.5 cm) were mesured. This baby had several congenital malformations as follows; blephaloptosis, asymetric ears, low position of the ears, micrognathia, high arched palate, short neck, broad prominent nasal bridge, simian creasis, fifth finger short, undescending testis, scrotal hernia, hip dislocation and club foot.
    Chromosomal study showed the shortened long arm in one of D-group of this baby and his parents kariotypes were normal.
    These findings indicate clinical Dq-Syndrome
