Resent advance of electrode technology has made it possible to measure blood gases and concentrations of many chemical contents directly from a whole blood sample. However it is still suspicious that measured values of electrolytes and metabolic contents might be influenced by hematocrit of the whole blood sample. Therefore we examined the effect of hematocrit of whole blood on measurement values of electrolytes(sodium, potassium, chloride and ionized calcium)and metabolites(glucose and lactate)level using ABL SYSTEM 625(Radiometer A/S, Copenhagen, Denmark). Values of sodium, potassium, chloride, glucose and lactate values were no significant difference between plasma and whole blood samples(P>0.05), while those of ionized calcium were significantly different between the both samples(P=0.0003). Since the value of ionized calcium in plasma was significantly lower than in whole blood, it is suggested that discharge of carbon dioxide from plasma may lead to the elevation of plasma pH and protein bound ionized calcium. An additional study have confirmed ionized calcium levels were not significantly different between plasma and pH adjusted whole blood(P=0.456).
