    The revision of medical fees in 2018 can be one of the policies which builds the strength of staff cooperation among different occupations and regional cooperation, meaning it can build community based integrated care systems. Accordingly, in this study, registered dieticians began participating in the Hospitalization and Discharge Support Program, and the usability of this program was verified. In the hospitalization support, a procedure of participation by registered dieticians was devised, grasping the outline of patients who used the Hospitalization and Discharge Support Center. The utilization method of information on nursing and nutrition management was considered in discharge support, and cooperative structure with the nearest hospitals and nursing and personal care facilities was organized.

    It was shown that maintaining a good nutritional state until hospitalization was useful, as well as use as a pilot study, and intervention by occupation staff eases the burden in hospitalization support. It was shown that it becomes possible to promptly manage nutrition administration for patients after discharge by sharing information before and after the transferring of hospitals. As a result, discharge support maintained and increased dietary intake among patients and it also maintained and improved nutritional state.

    These results indicated that participation by registered dieticians in the Hospitalization and Discharge Support Program, in addition to the strength of cooperation of staff inside and outside of the hospital was successful.
