    The respective actions of N-pole and S-pole of 2 kinds of permanent magnets of different gauss on the peripheral circulatory kinetics of the hamster's buccal pouch were determined by the laser doppler flowmeter (LDF), and the following conclusions were obtained:
    1) Cylindrical N-pole of 15 mm-diameter and 7 mm-height of a magnet of 1200 gauss caused an increase in blood flow of 0.44±0.07 V compared with a decrease of -0.37±0.05 V for S-pole.
    2) Cylindrical N-pole of 9 mm-diameter and 7 mm-height of the same magnet caused an increase in blood flow of 0.20±0.03 V, parallel to that of 0.19±0.03 V for S-pole.
    The above results confirmed that a permanent magnet of a constant gauss N-pole caused increased blood flow regardless of its magnitude but S-pole caused an alternation of increase and decrease according to its magnitude. It was also proved that the action of the magnet applied was exerted immediately upon the blood flow and kept this constant regardless of time, and returned to original values immediately after stopping.

    This study examined the changes in the positions of the condylar head of mandible before and after sagittal splitting ramus osteotomy surgery performed by applying the photographic subtraction method to Cephalometric laminagraph.
    Materials and Methods: This examination was carried out in twelve patients (two males and ten females, ranged 14 to 27 years old) who were diagnosed as having severe class III skeletal pattern and underwent sagittal splitting ramus osteotomy. Cephalometric laminagraphs taken before and after surgery were used for photographic subtraction. Changes in positions of the condylar head after surgery, were examined with respect to direction of rotation, degrees of vertical movement and horizontal movement.
    Results: rotation of condyle was observed in seven cases, two cases showed forward rotation (+) on one side only, two cases showed backward rotation (-) of the condyle on one side only, and three cases showed forward rotation (+) of the condyle on both sides. Vertical and horizontal movement were observed in all the cases: vertical change of the condyle on both sides in two cases, on one side in the other ten, horizontal change of the condyle on both sides in seven cases, and on one side in the other five.
    Discussion: Applying photographic subtraction to cephalometric laminagraphy was clinically useful for observing not only the small changes in the mandibular condyle but also the overall changes in the surgical position of the mandible.

    Statistical analysis was made on 26 patients who had undergone treatment with maxillary prostheses at our clinic during about 9 years from November 1977 to December 1986.
    With regard to sex distribution of the patients, 15 were males and 11 were females. Regarding age distribution, about 80% of the patients were in the 6 th to 8 th decades.
    In regard to etiology of the maxillary defect, in about 90% of the patients the causes were surgical operations for malignant tumors.
    The mode of the prostheses indicated in about 60% of the patients was a type of hollow obturator.

    Bone morphogenetic protein derived from bovine decalcified dentin matrix (d-BMP) has been solubilized with 4M guanidine hydrochrolide (GuHC1) and purified. It was insoluble in 0.01M CH3COONa containing 6M urea (pH 4.8), 0.5M GuHCl and water. The solubilized fractions were purified by 2 steps of gel filtration on Superose 12 following Sephacryl S-200, and by anion exchange chromatography on Mono Q HR 5/5. The activity of d-BMP was bioassayed by implantation in AKR mouse thigh muscle, and observed both radiologically and histologically. The final purificates indicated its molecular weight to be about 26 K daltons on SDS-PAGE and 29 K daltons after reduction with 2-mercaptoethanol. Isoelectric point was about 6.5 on isoelectric focusing. By amino acid analysis, the prominent amino acids were Asp, Tyr, Glu and Ser. The biochemical characteristics of d-BMP seemed to be different from other previously reported BMP, even BMP derived from bovine decalcified bone matrix. It suggests that possibility BMP differs in various tissues even in the same species.

    416 cases with lichen planus of the oral mucosa were treated with TCM and WM and studied.
    The results showed the following:
    1). In the reticular type TCM alone treated group of 250 cases the cure rate was 12.8%, markedly effective rate was 23.6%, effective rate was 28.4% and ineffective rate was 35.2%.
    2). A 387 case group with mixtured type by TCM and WM was significantly eff ective (65.7%), with a gross efficiency of 84%.
    Our study, therefore, strongly suggested that the more successful effect for treatment of OLP in this variety pattern utilized TCM and WM.
    This report further suggests that TCM alone might cure or be markedly effective for OLP.

    This paper reports the treatment of OLP with TH. We studied one hundred patients (45 men, 55 women) with clinical and histopathologic diagnosis of OLP, their distribution of lesion according to area and is shown in Table 1.
    Our survey of 100 cases has classified the patterns into two different groups, there were 66 reticular pattern cases and 34 mixture pattern (reticular and erosion form) cases.
    They were treated with 5-10mg Ti tri-daily for 2-8 months. After 2-8 months of treatment TH was a cure for 30 patients (24 reticular form and 6 mixture form), significantly efficacious for 54 patients (35 reticular form cases and 19 mixture form cases), 12 patients showed signs of improvement (7 reticular form cases and 5 mixture form cases), with a gross efficiency of 100 % in reticular form. The clinical effects of TII are shown in Table 2.
    TII is a TCM drug. The mechanism of action of the drug is not known, but is believed that it may possibly regulate immune reaction, espectially in III type hypersensitivity (immune complexes), therefore it may play a importment role in OLP.
    TII has same effect as steroid. Its anti-inflammatory effect is non-specific in nature, fast and less toxic.
    This report further suggests that TCM alone might cure or be markedly effective for OLP.

    Diabetes mellitus is well know for decreasing resistance to infection, but the mechanism of this phenomenon remains unkown in several points.
    Recently, opportunistic infections associated with diabetes mellitus have come under scrutiny.
    However, there are few reports on them, and there are no reports on experiments about the relationship between diabetes mellitus and opportunistic infections caused by oral bacteria.
    The purposes of this study are to decide the dose of Streptozotocin needed to induce diabetes mellitus in mice by single interperitoneal administration, to examine the relationship between diabetic states and opportunistic infections by oral bacteria in mice, and to examine the specific IgM in infected mice experimentally induced with diabetes mellitus.
    The results obtained are as follows;
    1) Single intraperitoneal administration of 300mg/kg of STZ was the most suitable for induction of diabetes mellitus.
    2) Oral bacteria caused infection in diabetic mice.
    3) There was no relationship between IgM production and infection resistance at 48 hours after infection.

    The authors made an experimental study on the healing process of freeze-dried allogeneic bone and fresh autogenous bone which were grafted in dog mandibles. It was obvious that freeze-dried allogeneic bone was as good a grafted material as fresh autogenous bone.
    But it was observed that in the freeze-dried bone graft group, inflammatory cells still remained for a long term after grafting and new bone formation was slightly delayed.
    This result suggested that freeze-dried allogeneic bone had immunological factors. So we investigated the cell-mediated immunity of freeze-dried allogeneic bone using the leucocyte migration inhibition test (LMIT).
    (Materials and methods)
    Freeze-dried allogeneic bone and fresh allogeneic bone were grafted in nine dog mandibles. Leucocytes and lymphocytes were taken at intervals of 1, 2, 3, and 4 weeks after grafting. We carried out LMIT by direct method and agarose plate method. Antigens were made by homoginizing the bone. Migration Index (M. I.) was calculated as follows: M. I.=migration zone in presence of antigen/migration zone in absence of antigen (Result)
    1 Week after grafting, many inflammatory cells and leucocyte migration were observed in both groups.
    2 Weeks after grafting, inflammatory cells still remained but leucocyte migration was inhibited in both groups.
    3, 4 Weeks after grafting, inflammation and leucocyte migration inhibition diminished gradually in the freeze-dried allogeneic bone group, but in the fresh allogeneic bone group this tendency was not remarkable.
    This investigation suggested that the antigenicity of freeze-dried allogeneic bone was weaker than fresh allogeneic bone.

    Nuclear DNA analysis of tumor cells from nine adenoid cystic carcinomas having a salivary gland origin was made in tissue sections by microspectrophotometry with special reference to the correlation between prognosis and variable histology.
    According to Philip's grading system, the nine cases were classified into three histological grades. The three categories established were: Grade I, tumors with tubular and cribriform areas but without solid components; Grade II, tumors with a predominantly cribriforom pattern; Grade III, tumors with a predominantly solid pattern. Iwai's classification of DNA histogram patterns was also used for the nine patients.
    All DNA histograms were graded as four patterns such as P-I (diploid pattern), P-II (hyperdiploid pattern), P-III (narrow aneuploid pattern) and P-IV (wide aneuploid pattern).
    The following results were obtained.
    (1) Histologically, 2 cases were Grade I, 3 cases as Grade II and 4 cases as Grade III.
    (2) The relationship between Iwai's classification of DNA pattern and Philip's histological grade was analysed. Grade I and II were the same as P-I and II of DNA histograms. Grade III, however, tended to be the same as P-III and IV of DNA histograms.
    (3) Grade III appeared to have an overall poor prognosis in terms of local recurrence, distant metastases and long-term survival.

    Infection with Methicillin·Cephem-resistant Staphylococcus aureus (MRSA) has recently attracted much attention because of its resistance to a wide range of antibiotics and high mortality rate.
    Case 1 was a boy 2 years and 5 months old. He received hospital treatment for a swelling in the left cheek. Because of no improvement, he was referred to our Department 11 days later. At initial examination, the temperature was 36.8·C. A mass, pain and reddening were found in the left submandibular area. It was clinically diagnosed as left submandibular phlegmon. Administration of CTM and CLDM resulted in exacerbation of the lesion. Staphylococcus aureus was isolated from the pus and surrounding tissue. The isolated strain demonstrated high resistance to Cephem and Penicillin preparations except CMZ.
    Case 2 was an 81-year-old woman. Clinical diagnosis of chronic osteomyelitis of the mandible was established. MRSA was isolated from the lesional tissue. This strain showed high resistance to many antibiotics (except CLDM and CMZ). The minimum inhibitory concentration exceeded 100μg/ml against an inoculation dose of 108 CFU/ml.

    The authors reported a case of metachronous triplicated primary carcinomas; multicentric squamous carcinoma in the oral cavity, malignant thecoma at the left ovarium and tubulary adenocarcinoma in the stomach, respectively. The tumors in the oral cavity occurred at the right buccal mucosa and on the alveolar crests of the bilateral premolar regions adjacent to the ipsilateral canines.
    Five cases of multiple primary cancers were obtained out of 96 cases of malignant neoplasms which were managed in our department from January 1981 to July 1987 after the definition by Warren, S. and Gates, O.
    These five cases were evaluated, and conclusions are as follows:
    1) Four female patients were included, and three cases aged more than 60.
    2) Four cases were of duplicated primary cancers and one case of triplicated.
    3) The incidence sites in the oral cavity were the buccal mucosa with or without gingival complication in four cases and the tongue in one.
    4) Three organs out of six on which another cancer (except oral cavity) occurred were in digestive system;two cases in the stomach and one in the rectum.
    5) Four cases of another cancer except the oral cavity were treated only with surgical procedures.
    6) Four cases in the oral carcinomas were managed by combined therapy with chemotherapy, irradiation and operation.
    Although the pathogenesis of multiple primary cancer has been proposed to date, conclusion has not been reached. Anyway, multiple factors are considered to affect their incidence.

    The purpose of this paper is to evaluate the applications and problems of intraoperative frozen-section diagnosis in department of Oral and Maxillofacial Surgery, Yamaguchi University hospital. Intra-operative diagnosis were performed in 35 operations, including 27 malignant tumor, one benign tumor and 7 non diagnosis during these 2 years.
    The average number of sections in one operation were 4.9, and 19 cases in most. In five of 35 cases, operative methods was altered according to intraoperative frozen-section diagnosis. In clinic, the intraoperative frozen-section diagnosis was frequently used for identification of extent of a lesion (surgical margin involved or free, lymph node metastasis or not). In all sections, intraoperative frozen-section diagnosis were accordant to final diagnosis except for one mucoepidermoid tumor.
    For more accurate intraoperative frozen-section diagnosis, we should be careful to deal with frozen sections and communicate with pathologist about differential diagnosis as well as informations of patient.

    The number of cases of tetanus has decreased in occidental countries because of the widespread use of tetanus immunization. In Japan also, the incidence of tetanus has decreased since the wide-spread use of tetanus toxoid in 1974.
    Patients most often consult general practitioners for their initial complaints, so it is rare that dentists or oral and maxillofacial surgeons are called upon to make a diagnosis of tetanus.
    Recently we examined a 65-year-old woman with severe trismus. We made a diagnosis tetanus based on her clinical symptom and condition. Prompt intensive medical intervention saved this woman's life.
    Although the etiology of trismus is rarely tetanus, all physicians and dentists treating the oral cavity should be aware of this possibility as the successful management of tetanus requires early diagnosis and prompt therapy.

    From Jan., 1986 to Mar., 1986, we used Afloqualone (Arofuto®) on 20 adult patients (5 males, 15 females) in TMJ syndrome with myofascial pain dysfunction. These patients were divided to myofascial pain group in 11 numbers, myofascial pain with anterior displacement of the disk with reduction group in 4 numbers, myofascial pain with anterior displacement of the disk without reduction group in 3 numbers, myofascial pain with irregularity of bone surface group in 1 number and myofascial pain with psychologic factor group in 1 number.
    In clinical findings, this was effective to patients with acute type of myofascial pain and with anterior displacement of the disk with reduction groups.
    Chronic type in each clinical groups spent on more time to effect and was less effective than acute type. This was especially effective on the masticatory muscles and also was effective on abnormality of jaw movement and reciprocal clicking on acute type ofmyofascial pain and with anterior displacement of the disk with reduction groups. There was the side-effect in 3 cases, which were all inappetence.

    It is important to appreciate soft tissue pathology in making a diagnosis of temporomandibular joint (TMJ) disease. For this purpose, contrast arthrography has been introduced.
    In addition, X-ray Computed Tomography (CT) which can clearly delineate both hard and soft tissues, has been used for the diagnosis of TMJ disease, especially after the introducution of direct sagittal computed tomography of the head. But up to the present, it has been difficult to demonstrate the articular disc with the use of X-ray CT.
    In this investigation, we attempted to demonstrate the articular disc by direct sagittal computed tomography with single contrast arthrography in which we injected air as a negative contrast medium into the upper joint space of the TMJ.
    As a result, soft tissues including the articular disc were shown clearly. We can obtain different images from the same scanning data by image processing. One is suitable for demonstrating bony tissusses and another is suitable for demonstrating soft tissues. W e conclude that our method is useful in the diagnosis of TMJ diseases.

    Ranula is a mucous cyst originating in major or minor salivary glands in the oral floor. Sometime it intrudes into submental and neck areas through an aperture or posterior resion of mylohyoid muscle. These are called plunging ranula.
    In this case, we reported a 17-year-old female that underwent resected sublingual gland and marspialized ranula, and prognosis was very good.

    Uptake of Ga-67 citrate by soft-tissue tumors was first reported by Edward and Hayes in 1969. Since that time, Go-67 has been investigated for use in the diagnosis and staging of many types of malignancies. The accumulation of Ga-67 in a tumor has been found to diminish after effective irradiation or chemotherapy. To explore this problem further, we investigated the change of Ga-67 scintigram following irradiation and chemotherapy for 5 cases with malignant lymphoma of the neck region.
    Ga-67 scintigrams of all cases of malignant lymphoma revealed strong positive images before treatment. However, after effective therapy the Ga-67 scintigrams became negative.
    This observation suggested that Ga-67 scintigrams can provide useful information regarding therapeutic effectiveness.
    Ga-67 scintigrams performed upon completion of therapy are useful adjuncts in assessing prognosis for tumor recurrence.

    Chemical burns of oral mucosa are relatively rare, mainly seen in the home and laboratory and limited to particular cases of accidentally taking chemicals into the mouth, misuse of medicine, suicide and so on.
    In general, the effects of chemicals on the mucosa are determined by factors such as the kind of chemicals, concentration, amount, duration of contact, temperature and injured tissue.
    We experienced two cases of chemical burns in the oral cavity from accidental taking alkali liquids into the mouth. In one case, he accidentally took ammonia water of unknown concentration.
    Because of marked erosion of the tongue, he was treated with infusion and neutralization, and pain relief. In the other case, saturated KOH was taken into the mouth during a chemical experiment. After a while, erosion and ulcer resulted and in a month frenulum linguae adhered. Swelling of the oral floor like the ranula appeared after two months.
    In chemical burns, tissue destruction begins after the latent period, and in spite of removal of chemicals, the destruction continues until the chemicals that penetrated into the tissue are either inactivated or completely disposed of. So it sometimes moves to Grade III of chemical burns as in the second case of this report.

    Four cases of dentigerous cyst occupying the whole maxillary sinus were presented and their cyst walls were observed to be lined with keratinized epithelial tissue. It could be considered rational to classify dentigerous cyst into non-keratinizing and keratinizing.

    The computer-assisted system using IC-card was designed to preserve dental examination results. The program works with the personal computer units which were composed of CPU, CRT, floppy disk unit, printer, IC-card reader/writer and IC-card. The IC-card reader/writer connects with the personal computer through RS-232C interface, and IC-card with 64kbits memory has memory capacity of about 8, 000 words.
    The input items are composed as follows ; 1) identity, 2) anamnesis, 3) findings of dental arch and gingiva, 4) O'Lery's plaque control record, 5) gingival index, 6) examination of periodontal pocket, 7) mobility and 8) contact test.
    This system has been used experimentally in the dental office of the Ministry of Posts and Telecommunications since December, 1986. It was found that this system is very effective for a dental clinic.
