    In oral and maxillofacial reconstructive surgery with a free peroneal osteocutaneous flap, preoperative evaluation of the vascular pattern of the flap donor site is desirable, for precise planning and safe harvesting of the flap. The reliability of the flap can be enhanced by preoperative knowledge about the exact location of the cutaneous perforator and osseous nutrient vessel.
    We carried out enhanced MR angiography before free peroneal osteocutaneous flap transplantation. In four patients who underwent reconstruction of oromandibular defects due to tumor resection, the vascular patterns of the donor site were preoperatively evaluated by MR angiography, with an intravenous injection of a gadolinium contrast enhancement agent. In all cases, MR angiograms clearly showed the exact position of the septocutaneous perforator vessels. This information enabled better presurgical planning and a more riskless flap elevation. Preoperative MR angiography is useful in reconstructive surgery with a free peroneal osteocutaneous flap.

  • Comparison between Histological Type and Expression of Cell Adhesion Molecules

    To investigate the relationship between tumor intercellular adhesion and ameloblastomas' variation in histology, we examined, immunohistochemically, E-cadherin (E-CD) and P- cadherin (P-CD) expression in 33 ameloblastomas (9 cases of follicular type, 12 of plexiform, 6 of acanthomatous, and 6 of granular cell type).
    Peripheral columnar / cuboidal cells in follicular, plexiform, acanthomatous, and granular cell ameloblastomas showed E-CD and P-CD co-expression, homogeneously, at the cell-cell boundaries. Stellate reticulum-like cells in plexiform ameloblastomas co-expressed E-CD and P-CD at the cell-cell boundaries, in the follicular and acanthomatous ameloblastoma expressed E-CD, but not P-CD. Squamous cell metaplasias cell in the acanthomatous ameloblastoma expressed only E-CD. In the granular cell ameloblastoma, granular cells expressed E-CD at the cell-cell boundaries, but some of them showed obscure and diffuse expression, and P-CD expression was observed in the cytoplasm. Reduced E-CD and P-CD expression in granular cells was noted in the central portion of tumor nests. We therefore concluded that both E-CD and P-CD are important regulators of intercellular adhesion, and that these molecules are also linked to the process of ameloblastoma morphogenesis.

    Alterations in secretion of salivary proteins due to surgical stress were experimentally studied. Saliva stimulated by pilocarpine was collected from the oral cavity of adult ICR male mice after surgical procedures. The simple surgical procedures, which included skin incision, development of a flap, and suturing, were performed under pentobarbital anesthesia. Serum corticosterone levels were elevated in the animals following the operation, due to surgical intervention. Salivary protein concentrations increased after surgical procedures. Kallikrein activities in the saliva in surgically treated animals were all markedly elevated. In contrast, saliva amylase activities relative to the salivary proteins were reduced in surgically treated animals. Salivary protein separation patterns on SDS-PAGE from surgically treated animals differed significantly from control mice and mice given a β-adrenergic agonist (isoproterenol), but they were very similar to those from mice given an α1-adrenergic agonist (phenylephrine). Pretreatment with prazosin (an α1-adrenergic antagonist) reduced surgically induced alterations in the separation pattern of salivary proteins. The results suggest that surgical stress enhances the secretion of salivary proteins, due mainly to α1-adrenergic receptor stimulation, in mice.

    Symptomatology for swallowing disorders has not yet been clarified, so that clinical decision-making for diagnostic testing such as videofluorography, is still unclear and difficult. This study analyzed symptoms of 11 patients who were judged to have swallowing disorders based on videofluorography. The results indicated that the following 6 symptoms were useful as indicators: food spills out of the mouth, bolusresidue in the oral cavity, like in the vestibule, floor of the mouth, and the palate; coughing during eating, and increased sputum production during and after eating. These symptoms would be helpful to establish a questionnaire and screening form for swallowing disorders.

    Recently, various sugar chains have been added to polystyrene derivantia (PV-sugar), and attempts have been made to regulate specific cell growth and adhesion. PV-sugar is a highly interesting high-molecular compound, in which the action of cells changes due to differences in sugar chains. This sugar chain addition reaction technique is promising for its potential for new clinical applications in the preparation of artificial extra-cellular matrix. We examined various actions of alteration in vitro with PV-sugar on fibroblasts from skin and oral mucosa. Six kinds of PV-sugar were coated on a dish, with no cellular adhesion processing. Cell growth was measured using WST-1, and cell adhesion was measured by lactate dehydrogenase cytotoxicity testing at 24 and 48 hours after the coating. For the morphological observation, methylene blue stain and fluorescence staining were performed. As a result of our examination, the PVsugar did not function for direct cell growth of the fibroblasts. However, improved cell adhesion was recognized with PV-GlcNAc (poly (N-p-vinylbenzyl-O-2-acetoamide-2-deoxy-β-D-glucopyranosyl-(1, 4)-2-acetoamide-2-deoxy-β-gluconamide)), PV-Lam (poly (N-pvinylbenzyl-O-α-D-glucopyranosyl-(1, 4)-D-gluconamide)). Moreover, PV-MA (poly (N-p-vinylbenzyl-O-α-Dglucopyranosyl-(1, 4)-D-gluconamide)) tended to inhibit cell adhesion and growth in the early stage. The cytoskeleton was observed to change with the type of sugar chain. From the above results, when bioartificial tissue is made, it was suggested that the sugar chain addition technique is useful to give the matrix an anchorage for cellularis variation.

    Screening for infectious diseases (hepatitis B and C, syphilis) was carried out on 703 dental outpatients. The incidence of seropositive patients was 2.6%(18/703), including 0.7%(5/703) for hepatitis B, 1.7%(12/703) for hepatitis C, and 0.3%(2/703) for syphilis (1 case was positive for both hepatitis C and syphilis). The result shows that the rate of infec-tious disease was close to that of blood donor and adult medical checkup data. Only 16.7%(3/18) of seropositive patients were detected before treatment, and it was difficult to confirm an infected patient by interview.

    To clarify the awareness and subjective symptoms of the disorder in preoperative ankyloglossia, patients were evaluated using a questionnaire survey. The following results were obtained.
    1. Disorders in swallowing and mastication were recognized in about 33.3% of the moderate cases. Oral functional disorders were checked in about 83.4% of the moderate cases, which was significantly frequent compared with the slight cases.
    2. Speech disorders were observed most frequently as subjective symptoms, and uptake disorders were observed in some cases.

    Gardner's syndrome (GS) is a disease characterized by the three symptoms of polyposis coli, osteoma, and soft tissue tumor. Moreover, intraosseous lesions in the jaw are frequently detected in this syndrome.
    A 18-year-old female with GS was referred to our clinic for a thorough jaw examination. X-ray findings revealed a compound odontoma and impacted teeth in the mandible. Taking her wish into conside-ration, surgical resection of these lesions was performed, together with total colectomy and ileo-rectal anastomosis.
    In this case, the removal of oral lesions was very useful in improving her quality of life.

    Angiokeratoma is a rare cutaneous vascular disorder of the papillary dermis, that is also associated with hyperkeratosis of the epidermis, and it typically occurs at multiple sites. A solitary angiokeratoma in the oral region is extremely rare.
    We report on a 50-year-old man who had a solitary tumorous lesion of the palatal mucosa. Excisional biopsy of the mucosal lesion in the hard palate showed features consistent with angiokeratoma. We report the first solitary angiokeratoma of the palatal mucosa, and review the clinical and pathological features of this unusual condition.

    We experienced two child patients who complained of oral symptoms from psychological stress, one of which involved school refusal due to irritable bowel syndrome and eating disorder.
    We report them as follows. Case 1: A 15-year-old male patient complained of ulcer formation in the lower lip mucosa. He had occasionally suffered from stomatitis since he was an elementary school student, which had always been treated with ointment. He was introduced to us by the pediatric department of our related hospital, which suspected hereditary Behcet's syndrome because of the ulcer formation in the lower lip mucosa. At the first examination, we suspected that mental stress was associated with this symptom, since he was refusing to go to school. We performed grinding of a tooth acute margin, applied steroid ointment, and interviewed him about his background. Later, he was interviewed by a psychiatrist at our hospital. As a result, it was suggested that the symptom was a psychogenic reaction caused by anxiety resulting from the sudden death of his younger brother.
    Case 2: A 15-year-old female patient complained of jaw tiredness while eating. Clicking was noticed in the left mandibular joint in 1996, which she neglected. In addition to the click, jaw tiredness while eating was noticed in April 1997. She consulted a nearby dentist and received splint treatment. Since no improvement occurred, she was introduced to us. At the first examination, she was 150cm in height and 35kg in weight. Since her medical history included sudden weight loss and cessation of menses, we suspected an eating disorder.
    We performed temporomandibular arthrosis treatment, and then introduced her to our psychiatric department. She was treated by both departments. Her symptom has been alleviated, and she is now gradually gaining weight.
    The above two patients had undergone treatment at verious medical departments before they consulted us. In the future, more and more people may complain of oral symptoms and consult dentists and oral surgeons. It seems that mental stress in their background should be taken into consideration, even in treatment of children.
