近年,げっ歯類を用いた研究により中枢神経系による大腸運動・排便制御機構の解明が進められてきた.その結果,脊髄排便中枢が骨盤神経を介して遠位結腸・直腸運動を制御すること,その機能が複数のモノアミンやペプチドによって調節されることを明らかとなってきた.特に,視床下部-延髄縫線核-脊髄排便中枢を介する下行性神経回路が急性の心理的ストレス時に誘発される排便に関与することが明らかとなったことは重要である.これらの知見は,腸脳相関という概念を具象化するのみならず,ストレス関連排便異常のメカニズムを理解ために有用である.本稿では,大腸運動・排便に対する階層的制御機構と,心理的ストレス時の排便制御に関わる中枢性神経回路について,私たちの最新の知見を含めてまとめる.
パーキンソン病(PD)は運動症状を中核とするが,近年はα-シヌクレイン病理が全身に及ぶ「全身病」として認識されている.特に便秘などの消化器症状は運動症状に先行して出現し,患者のQOLを著しく低下させる.本稿では,PDにおける消化器症状の病態生理を自律神経,神経伝達物質,腸脳相関の観点から概説する.Braak仮説が示すように,病理変化は腸管から迷走神経を介して中枢へ上行する可能性があり,排便中枢である脳幹の障害が早期の消化器症状を説明し得る.さらに,PDでは早期からアセチルコリン(ACh)系が障害され,これが末梢の副交感神経機能低下を招き消化器症状を引き起こす.我々の臨床試験では,コリンエステラーゼ阻害薬であるドネペジルが,認知機能に加え便秘等の非運動症状を安全に改善することが示された.今後の腸脳相関のメカニズム解明は,消化管を標的とした新たな疾患修飾療法の開発に繋がることが期待される.
The human brain oscillates in harmony with frequency-specific subcomponents of several brain regions. The heart and brain are connected bi-directionally, and heart rate variability (HRV) may reflect the activity of the coordinating system, notably brain functional connectivity, including the default mode network (DMN). As shown herein, HRV may thus inform about humans’ mind, including the well-being (e.g., satisfaction). First, the VLF component (0.003–0.04 Hz) of HRV (VLF-HRV) in healthy subjects statistically decreased by 21.9% (n=18, p<0.00001) in association with appearance of the Northern Lights, and a graded decrease in VLF-HRV was validated depending on the degree of geomagnetic activity (p<0.05). Second, we confirmed 20 astronauts’ response to space in terms of their HRV endpoints. VLF-HRV decreased by 23.2% (n=20, p<0.001) in space, and a decrease in VLF-HRV was validated depending on the degree of adaptation to microgravity in space. i.e., extent of VLF-HRV decrease was 29.7% when visiting space in 9 sufficiently adapted astronauts (p<0.05), but 12.9% in 11 poorly adapted astronauts (N.S.). VLF-HRV is known as a powerful predictor of cardiovascular disease risk, whereas it was recently noted as a representative HRV index which inversely reflects a sense of deeply impressed satisfaction related to the brain’s DMN activity. It could thus be said that an abrupt decrease in VLF-HRV could mean survival (strong happiness) or death (life threatening condition) depending on the circumstances, because from a viewpoint of functional brain’s neural networks, those outcomes are opposite sides of the same coin.
