The adsorptions of FUT was syudied nn seven adsorbents in vitro. XAD-7, 8 adsorbed almost all FUT From the FUT solution, although Anion Exchange Fiber (AEF) and IRA-938 did not. XAD-2, 4 and PNEMA coated activated charcoal (DHP-1) had a small capacity. AEF and DHP-1 were exsperimented ex vivo. DHP (AEF) succeeded safely with 20-40mg/hr infusion of FUT. DHP-1 could not be used with 15-60mg/hr FUT.

In PE, FUT-175 was safe and enough to anticoagulant for patients with normal coagulative function, had no influence on systemic blood. Infusion of FUT-175 did not increase hemorrhagic diathesis, it was showed that FUT-175 was useful for patients who had hemorrhagic diathesis.

The neutral solution of CS-570 was quite stable at 40°C for 16 weeks unlike PGI
2. The anti-platelet effects of CS-570 was about 1/10 those of PGI
2. CS-570 was also effective after oral administration. Usefulness of CS-570 for the extracorporeal circulation was strongly expected.

As a measure to overcome the problems associated with heparinization in hemodialysis, the continuous intracircuit infusion of an appropriate dose of carbacyclin derivative: CS-570 was found to be successfully used as an anticoagulating method in hemodialysis of dogs without producing marked hypotetion. In this procedure, any signs suggesting intracircuit coagulation, such as decrease in platelet count and elevation of intracircuit pressure, have never been observed. Platelet aggregation has been extensively suppressed within the circuit without being accom panied by any change in activity of plasma coagulating factors.

Surface charge effects on the newly developed penta-block copolymer and its derivatives of film-formed materials in plasma and of the glass beads column method using whole blood were examined. The results presented the mosaic-charge and the anionic materials as scarce platelet adhesion. By former Lee-White test the mosaic-charge and the cationic materials were good. These facts suggest the mosaic-charge material would become one of the ideal anti-thrombogenic materials.

To clarify the relationship among complement activation(CA), hemodialysis leukopenia (HL) and hypoxia (HH), we examined the effect of FUT-175 (multi-enzymatic blockade: FUT) on them. Although FUT inhibited cellulose membrane mediated CA dose-dependently in in vitro, FUT could not suppress HL clinically. In ex vivo experiments with dogs, very high dose of FUT inhibited HL and maintained leukocyte function during HD. Even though HL was prevented, CA was still noted. There were no correlation between HL and HH during HD with either heparin or FUT. These results indicate that not only CA but also other enzymatic pathways which can be suppressed by FUT have contribution to HL and that HL is not main pathogenetic factor of HR.

We examined adhesion, growth and morphology of mouse fibroblast (L Cell) on chemically modified Fibrinogen and Fibrin fiber coated on sulfonated-polystyrene. It was found that fibroblast response to chemically modified Fibrinogen varied drastically with net charge, and that cellular response to Fibrin fiber coated on sulfonated-polystyrene varied with surface charge density of sulfonated-polystyrene.

Hemolytic plasma acts on blood asa a coagulation activator. By gel filtration fractionation on Sephadex G-15, hemolytic plasma is separated into (1) a tissue thromboplastin-like substance containing proteins and (2) Redox potentialassociated ADP, AMP and ATP of the molecular weight of approximately 500 not containing proteins. The role of ADP in platelet agglutination was investigated using an aggregometer. Platelets distinctly showed agglutination in the presence of hemolytic plasma. In a similar experiment using concentrated (140) hemolytic plasma from the blood collected on termination of extracorporeal circulation, agglutination of platelets was also confirmed. This indicated that ADP released in hemolysis agglutinates platelets and eventually causes iutravascular coagulation.

Perfluorochemical (PFC) emulsion has developed as artificial blood, but it is limited to a maximum of 1500ml in regard to the infusion volume because of poor excretion of PFC from body. However, if it is possible to retriev PFC once mixed with whole blood, this may increase the clinical applicability of PFC emulsion. We previously reported that centrifugation is a useful method of retrieving PFC from whole blood. Then this time, we tried normothermic Whole Body Rinse-out, almost same as Total Body Washout in efficiency, retrieving PFC and transfusing donor's fresh blood after replacement of blood with PFC emulsion. As results of the experiments using three dogs containing one jaundiced dog by ligation of common hepatic duct, bilirubin clearance rate was 90.5% and the concentration of PFC in blood reduced to 1/2-1/3 of that of maximum level.

Emulsified PFC preperaticis have been experimentally and clinical utilized as artificial blood for transfusion. However, it has also been noted that administrated PFCs are extensively taken up andprolonged period within the host RES phagocytes. We conclued that i. v. injection of PFCs to mice induced severe and chronic depression of host immune functions. In vitro mitogenic response of B cells was directly suppressed by PFCs. T cell mitogenic responses were affected to much lesser extents.

Fluosol-DA cardioplegia was compared with crystalloid cardioplegia as a method of myocardial preservation during ischemic arrest.Experimental dogs were divided into two groups according to perfusates. One group received the standard crystalloid cardioplegic solution [K
+20mEq/L, 4°C] and the other received an oxygenated Fluosol cardioplegic solution [K
+20mEq/L, 4°C]. CI, SVI, LVSWI, LVmaxdp/dt were measured before and after cardioplegic arrest. The results documented significantly better CI, SVI, LVSWI in the Fluosol than the crystalloid group. But LVmax dp/dt was depressed equally and showed no significant difference in both group. We conclude that the Fluosol was better protective effect than the crystalloid group.

Radiopaque perfluoroctybromide emulsions were prepared with small particles as size as 0.113μ and effective oxygen-transferring performance and were applied for selective coronary arter iography (CAG). We developed adaptive assisted ventilation (AAV) system that was more adaptive for body. AAV can make patients assisted ventilation agree with there ventilatory demand, because it can sens there ventilatory responce. We showed the process of shifting from mechanical ventilation to spontaneous ventilation by respiratory rate, tidal volume, air way pressure, and the density of expilatory CO2. As the passage of time, mechanical respiratory rate was decreasing and spontaneous respiratory was increasing. But spontaneous respiratory rate decreased to the normal range by increasing tidal volume. The assisted ratio of ventilation increased, because the air way pressure decreased by increasing the density of expilatory CO2. Compaired with Pco2 by the old on off method in the weaning from respilator. In AAV method Pco2 showed a tendency to gather around the normal range, but in on off method Pco2 showed atendency to increase.

Gore-Tex which was reinforced with teflon rings was used as a tracheal prosthesis. None of the implanted tracheal prostheses in the 14 mongrel dogs have shown deformity or dislocation, because of strength and compliance to both longitudinal direction and horizontal direction. But this prosthesis was weak in infection and the fibril length of this graft itself was too small to form reepithelization to adhere to the adjacent tissues. Sequential bronchoscopic observation revealed overgrown granulation in the anastomotic sites which resulted in severe tracheal stenosis about 2 months later after implantation. To suppress these overgrown granulation, we used steroid and immunosuppressant and obtained relatively good results to decrease inflammatory cells and collagen fibers in the anastomotic sites histologically.

We examined an artificial esophagus with the expanded polytetrafluoroethlene (EPTFE) with beagle dogs and Australia-White rabbits. The grafts were reinforced with polyehylene rings, and were implanted 3 cm at the neck in two dogs, 1cm in three rabbits. One dog was succeeded in surviving for more than two months, another one died for the endoscopic technical failure at postoperative 6 weeks. Three rabbits could not succeeded in surviving for more than two weeks, because of IVH technical failure, leakage and poorly general condition. The inner surface of the grafts were progressively covered with regenerating mucosa in rabbits. We observed, the graft fell down into the stomach for leakage in one autopsy dog.

There is only statistical report about chronic dialysis patients issued by the Japnese Society for Dialysis Therapy i n Japan. According to it, new patients treated with dialysis therapy i n one year are over 10, 000 patients. Among these patients, the rate of diabetic nephrophathy is gradually increased by year and year. Then its rate became 13.7% in 1982 new patients. So that, it is necessary to estimate the number of patients, resulting with follow up study to each new patient for not only medical problems but also socio-economical views in Japan.
