    Ten Y-specific DNA fragments were cloned from a human Y-chromosome-rich DNA library using a newly developed plasmid vector. Mapping of the 10 Y chromosome fragments was carried out by the Southern hybridization technique using DNA samples of patients with a structurally abnormal Y chromosome. The rationale is that the more proximal the probe, the larger the number of patients expected to show positive bands. Probes mapped in the other laboratories have also been used for comparision. It is difficult to identify positively very small chromosome areas even with the combination of several different banding techniques. Detailed DNA analysis of structurally abnormal Y chromosomes has become possible by the use of Y-specific DNA probes isolated in the present study. It is now possible to analyze with precision the entire Y chromosome from the very end of the short arm to the distal end of the long arm. The series of probes isolated in the present study, proved to be of great value in analyzing DNA of clinical cases with a variety of structural abnormalities of the Y chromosome.

    In totally 1235 people who underwent hospitalized clinical survey, I studied the abnormality rate relating to health consciousness, the rate of accordance of subjective complaints with abnormal findings, and the abnormality rate by sex and the number of examination. The obtained results are as follows; 1) There was no significant difference in the frequency of abnormal findings between people who feel healthy and those who do not feel healthy. 2) The rate of accordance of subjective complaints with abnormal findings varied extensively among organs and systems. 3) There was no significant difference in the abnormality rate by sex. 4) Likewise there was no significant difference in the abnormality rate by the number of examination. Based on these results, I showed advantage of the hospitalized clinical survey system, which consists of not only examination of organs but also general health check up for the detection of occult diseases. And I expressed the value of medical interview in percentage obtained from the rate of accordance of subjective complaints with abnormal findings. By this process I showed the degree of importance of medical interview for the detection of occult diseases, depending upon various organs and systems.

    To assess the non-specific cell mediated immunity in 127 cases of esophageal carcinoma without preoperative radiation therapy, a study was made on the degree of histologic stage, gross longitudinal diameter, histologic invasion to the adventitia, lymph node metastasis, the number of metastatic lymph nodes and histologic differentiation, by means of immunological parameters. The results were as follows: 1) There were decreasing tendencies of lymphocyte rate, lymphocyte count, phytohemagglutinin (PHA) skin test, purified protein derivatives (PPD) skin test and PHA-induced blastogenesis with the advance of development of carcinoma and a most specific correlation existed between the lymphocyte rate and the development of stage. 2) There was no definite variation of T-lymphocyte rate and B-lymphocyte rate with advance of development of carcinoma, and the value as an immunological parameter for esophageal carcinoma was not admitted. 3) The decline of immunity accompanying the degree of lymph node metastasis was more strongly correlated than the degree of histological invasion to the adventitia. 4) There was no correlation between the degree of histologic differentiation and immunological parameter. 5) To assess the immunity of carcinoma-laden patients, this immunity must be judged synthetically by means of many kinds of parameters.

    In 1901, Askanazy made a report on a case of heterotopic bone formation in the laparotomy scar. Since that time, many cases of this phenomenon have been reported in Europe and in the United States. According to Piculachs et al, such cases amout to even 230 cases. In Japan, Hayashi was the first to make a report on heterotopic bone formation in 1938. Since that time, the cases accumulated over a period of 50 years only amount to 76 cases. This total includes one case experienced in our Department. The case is a 59-year-old male. A diagnosis of cholelithiasis was made; the gallbladder was extracted through an upper mid-line incision; and a T-tube drainage operation was performed. Eight years and six months after the operation, a 15 cm hardness along the surgical scar was revealed and was accompanied with pain. After various examinations, this case was diagnosed as heterotopic bone formation and underwent surgery to have the bone resected. The above case will be discussed.

    In a case of severe active Crohn's disease of colon, we tried the combined intraarterial injection therapy using a steroid and urinastatin (UST) to superior and inferior mesenteric artery, and observed rapid improvement. A fifteen year of age Japanese girl having severe continuous abdominal pain with bloody stool, high fever, and anemia on admission. An endoscopic and xray findings on barium enema revealed a longitudinal linear ulcer, pseudopolyps, and strictures of colon. After treatment of the combined intraarterial injection therapy of predonisolone and UST, Crohn's disease activity index (CDAI) was markedly improved from 473.5 to 27.8. After discharge from the hospital, the patient received salazopyrin and a elementary diet. One year later, endoscopic and Xray findings on barium enema showed marked improvement of the strictures of colon, and no relapse occured until present time. According to our experience, it is suggested that combined intraarterial injection therapy using a steroid and UST is useful for active Crohn's disease.
