The red cell esterase D among 524 blood donors in Tokyo were investigated by the starch gel electrophoresis techniques essentially similar to original methods. The distribution of phenotypes were as follows : EsD 1 (45.42%), EsD 2-1 (43.89%) and EsD 2 (10.69%). The frequencies of the alleles of <EsD^1>___- and <EsD^2>___- were estimated to be 0.6737 and 0.3263, respectively. These findings show that the frequency of <EsD^2>___- gene of Japanese is higher than those of Caucasians and Negroes reported thus far. Twins study confirmed that the above mentioned three phenotypes were controlled by the autosomal co-dominant alleles of <EsD^1>___- and <EsD^2>___-.

Two cases-the one with second-degree A-V block and the other with third-degree A-V block-were treated successfully by implantation of artificial cardiac pacemakers. A stress was made on the importance of preoperative temporary cardiac pacing and recordings of His bundle electrogram for selecting patients. The routine measurement of myocardial threshold and analysis of pulse wave-form of generators upon implantation were discussed. In addition, our methodology of following up patients with implanted pacemakers were briefly described. The application of pacemakers to various types of arrythmias is a useful armamentarium provided that they were properly handled with precise knowledge and techniques.

The incidence of poststreptococcal acute glomerulonephritis (PSAGN) in the adult has been thought to be much lower than the child. However, in almost one year of 1973 we could observe 12 cases of PSAGN in our medical department. The diagnosis was based on the following clinical criteria ; 1) No past history of renal disease, proteinuria, hematuria, hypertension or edema. 2) No extrarenal or laboratory data consisting with a systemic disease. 3) Preceding history of infection, mainly, of upper respiratory tract. 4) Acute onset of proteinuria and hematuria, hypertension and/or edema. 5) Elevation of ASO titer. The clinical data and course of these cases were precisely described. It is known that PSAGN may occur without the elevation of ASO titer or with only unrinary abnormalities without clinical manifestations. Considering these facts, the incidence of PSAGN in the adult seems much higher than previously thought. Based on our observations, the possibility is raised that PSAGN may be the main pathogenetic underlying disease in patients with asymptomatic proteinuria or so called chronic glomerulonephritis. Further studies are needed to confirm this hypothesis.

Human peripheral blood lymphocytes, when incubated with specific soluble antigen, produce a soluble factor which inhibit the migration of normal guinea pig peritoneal exudate cells out of small capillary tubes. This factor is produced only when the subject is sensitized previously by the stimulating antigen and is referrd to as MIF. The in vitro assay method for the detection of MIF by the capillary tube technique is called MIF test or MI test. This test is currently one of the most useful tool for the study of cellular immunity and has its origin in the experimental work of delayed hypersensitivity. More recently, it has been applied widely to the analysis of human immunological disorders, such as autoimmunity, transplantation, immunodeficiency, or cancer immunity. For example, we have successfully used this method for the detection of cellular sensitivity to glomerulonephritis. In this paper, the principle and practical manipulation of MIF test were introduced.
