    In recent years the ventilating bronchoscope under general anesthesia in children has been accomplished more frequently in RCU and ICU. This report describes the experimental result of air way resistance especially in low O_2 flow rate (200 ml/sec) and the energy loss of O_2 flow at the scopic orifice, as the evaluation of the function of new slit-hole bronchoscope. The checkvalue like effect of the sidehole of the standard bronchoscope is documented. The air way resistance at the side hole is higher in the same O_2 flow, by expiration than by the inspiration according to the result of the experimental simulation. The slit-hole orifice of the new type bronchoscope decreased the air way resistance in to the pediatric bronchoscope especially in low flow rate (250 ml/sec Fig. 2). The ratio of energy loss in the O_2 flow throgh the side hole orifice (-e') and the slit-hole type orifece (-e") is caluculated as -e'/e"=2.71 from the measurments of the angle between O_2 flow line and scopic tube.

    The effectiveness of magnesium sulfate (MgSO_4) on extending survival of the anoxic canine heart, lung and kidney has been evaluated in normothermic condition. Canine heart, lung and kidney preserved with MgSO_4 up to four hours restored normal function after auto-or homotransplantation. These studies demonstrate that the viability of the mammalian heart, lung and kidney at nearly body temperature can be extended using MgSO_4 as a metabolic inhibitor and suggest very practical approach for cadaver organ preservatior prior to organ recovery.

    Increased postcontraction sensory discharge (PCSD) arises from muscle reflex activation or direct stimulation through its efferent fibers. Some receptors supporting PCSD have been directly identified. A major component has been traced to Group la spindle afferents. Other reports have suggested that non-proprioceptors also contribute. A series of experiments were performed to investigate the influence of PCSD upon on-going central nervous activation of the motoneuronal pool supplying hindlimb muscles in lightly anesthetized, partially denervated cats. Sixty-two motoneurons were isolated from L_7 ventral root filaments, functionally categorized according to their response to triceps surae stretch, and monitored before and after a 5-sec, 5 times twitch threshold contraction. Of 37 units of motoneurons excited by stretch (MN-S^+ group), 26 units increased, 1 decreased and 10 exhibited no change after contraction. Of 6 units of motoneurons inhibited by stretch (MN-S^-), 3 increased, 2 decreased and 1 exhibited no change after contraction. In motoneurons uninfluenced by stretch (MN-S^O) and motoneurons exhibiting mixed or unpredictable responses to stretch (MN-S^M) groupes, 14 out of 19 units showed no change. Statistical tests showed that only the MN-S^+ group exhibited a significant change (.01 level) in mean frequencies after contraction. Furthermore, in 37 units in the MN-S^+ group, 24 units were tonic and only 5 were phasic and 8 phasic-tonic units. Overall, these results were compatible with previous results indicating that PCSD is served largely by proprioceptive discharge. Theoretically, PCSD and its motoneuronal counterpart might serve to stabilize the muscle against "length" changes in postural activities.

    From April 1970 to May 1978, 9140 patients with upper G-I complaints were examined by the fluoroscopic and radiographic procedures in the Department of Radiology, School of Medicine, Kyorin University. The radiological diagnosis of 95 percent of all the patients was made by Dr. D. Aoyama. The number of patients with gastric cancer was 485 in 9140 patients (5.3%). We termed open gastric ulcer and/or scar as GU, duodenal ulcer as DU, coexistent gastric and duodenal ulcer as GU+DU and advanced and early gastric cancer as Ca. O means no pathological findings. In this paper the problems of incidences of gastric, duodenal and coexistent gastric and duodenal ulcer are discussed, and the incidence of GU is 19.5%, that of DU is 6.6%, that of GU+DU 5.1%, that of total GU 24.6%, that of total DU 11.8%, and that of total GU+(GU+DU)+DU 31.3%. The sex difference in their incidences was also examined statistically. In GU the difference was not significant, but in DU and GU+DU it was significant (p<0.01). In the follow-up study of GU+DU, the number of patients was 271, as in the Table 13. Radiologically the scar of gastric ulcer is frequently observed or it often disappears during the follow-up studies, but the recurrence of gastric ulcer is not so rare. On the contrary, the scar of duodenal ulcer does not disappear completely in the majority of the patients in 6.5 years, the longest follow-up time in our experience.

    A newly developed device which aims to the transvenous closure of the Patent Ductus Arteriosus (PDA) without thoracotomy is presented. The device consists of double umbrella, ratchet mechanism and guide wire. These are placed in the capsule at the distal end of the catheter. The outer umbrella opens in the aorta and inner one opens in the main pulmonary artery. These two umbrella are connected with ratchet mechanism which make it possible to adjust the length corresponding to the PDA. A smooth function of the designed mechanism and a successful closure of the PDA are demonstrated in the animal experiment using dogs constructed PDA. This device could possibly be used clinically after the meticulous chronic experimental studies.

    Involuntary movements observed in a patient with multiple cerebral infarction were studied in relation to the location of cerebral lesions visualized by computerized tomography. A 46-year-old, right-handed man suddenly suffered aphasia and right hemiparesis 4 years ago which recovered in about 1 year. In the second attack occurred recently, he showed conjugate eye deviation to the right, aphasia and involuntary movements such as dystonia of the trunk, tremor and athetosis of the extremities more marked in the right side without motor paresis. A computerized tomography disclosed an old infarction in the left basal ganglia and fresh infarction with contrast enhancement in the right frontal lobe, basal ganglia and temporoparietal lobes. It was noted that the involuntary movements appeared more remarkably in the right extremities in spite of the fact that infarction newly occurred in the right hemisphere at that time. Presumably, symptoms of the left basal ganglia damaged in the first attack had not emerged because of the simultaneous motor paresis, until the second attack due to the lesions in the right hemisphere which may also have possibly influenced the blood flow of the left basal ganglia with resulting activation of their dormant derangements. The involuntary movements of this patients, especially dystonia and then athetosis, are still present about 1 year thereafter without any definite response to medication.

    Disopyramide (Rythmodan) was synthesized in 1954. Since the late 1960's, disopyramide has been successfully used to treat patients with a variety of atrial and ventricular arhythmias. The effect of orally administered disopyramide (100 to 500 mg, daily) was studied in 10 patients with a variety of arhythmias. Paroxysmal atrial fibrillation in two of 2 patients was converted to sinus rhythm, whereas no chronic atrial fibrillation was converted to sinus rhythm or atrial flutter. Frequencies of atrial premature contraction and ventricular contraction were decreased in two of 2 patients (100%) and in six of 9 patients (67%) respectively. Side effects were observed in six of 10 patients (60%) and were primarily due to transient anticholinergic activity of the drug including dryness of mouth, urinary hesitancy or gastrointestinal distress. Two patients had transient urinary hesitancy requiring discontinuation of disopyramide. Disopyramide might be particularly useful as an antiarhythmic agent in patients with a variety of atrial and ventricular arhythmias.

    The patient undergone cardiac pacemaker implantasion, needs careful long term follow-up, and must be checked up not only of his general states but also of pacemaker function, at the pacemaker clinic. The parameters reflecting pacemaker function were sought, PULSE PER MINUTE and PULSE DURATION measure-ment found to be convenient, which can be measured by simple electronic device. Using this, patient can check himself his implanted generator by quite easy way. The instrument adopts as patient-self checking system.
