    Paroxysmal nocturnal hemoglobinuria (PNH) is a disease characterized by hemoglobinuria caused by intravascular hemolysis and marked hemosiderosis in the tubular epithelium, and frequently results in renal failure. We reported an autopsy case of PNH, 57-year-old woman, having thrombosis of femoral vein and cavitated pulmonary tuberculosis with hematogenous generalizarion. The kidneys showed marked hemosiderosis in the proximal tubular epithelium and formation of small abscesses. Scarring bands with destruction of a few nephrons in the cortex, however, may be brought about by renal vasoconstriction due to repetition and prolongation of hemolytic crisis leading to chronic renal dysfunction. Proliferation of the lacis cells in the juxtaglomerular apparatus caused by chronic anemia was suspected as the probable cause for the production of erythropoietin and resultant erythroblastic formation of the bone marrow.

    An apparatus for ammonia (NH_3) determination by using gas permeation/gas chromatograph (GC) had been developed. The apparatus was composed of mainly GC and a thermoregulator including a permeation plate assembly for gas permeation. Urea and NH_3 in urine were determined at pH 6.5 by using the apparatus and the usefulness of the proposed method was also disscussed on the results from the measurement of both compounds. The precision of the measurement was within 6% as coefficient of variation and the detection limit was 4.5μg for the permeated NH_3. An analytical time required was within 20 min for both the compounds and the time was more than 60 min shorter in comparison with that by the both methods of the indophenol (IP) method and the urease-indophenol (UI) method. A correlations between the proposed method, and the IP method or the UI method, were also good, and their correlation coefficients were 0.98 and 0.97, respectively. But the determined values by the proposed method were slightly higher than those of the both other methods. Also, chromatographic patterns of NH_3 and total NH_3 with urine samples collected from normal and abnormal subjects were determined and compared. In comparison with their chromatographic patterns, the screening between diseases was easier and more precise in the proposed method.

    Herpetic neuralgia was treated by micro-cytarabine and/or cycrocytidine epidural (ganglion) injection combined with the block therapy. Improvements were observed in all patient as measured by the pain intensity rating scale. To determin the optimal stimulation parameters for pain relief, conventional transcutaneous electrical nerve stimulation was applied to the patients diagnosed as having postherpetic neuralgia. The herpetic patients preferred a high range setting of the pulse rate and the lower range of the pulse width. It is suggested that this is the treatment of choice in herpetic neuralgia.

    In order to evaluate the clinical importance of polycystic kidney disease (PCKD) in the field of clinical nephrology, we studied clinical feature of 27 patients with PCKD who were under our medical control. PCKD was diagnosed by the combination of DIP, renal echo., renal scintigram and renal CT scan. Among 27 patients, 16 were male and 11 were female. Family study disclosed 7 of them were hereditarily predisposed. The average age of the appearance of initial symptoms directly related to this disease was 43 years and the average age of confirmative diagnosis was 47 years. The most frequent initial symptoms were hypertension, gross and/or microscopic hematuria, lumbago and mild proteinuria. Abdominal mass was noticed in only one patient. The level of BUN/Cr at the time of diagnosis was 27/2mg/dl. Of 27 patients with PCKD, chronic hemodialysis (HD) was introduced in 8 patients due to decompensated chronic renal failure. Three patients died of gastric cancer, infection after renal transplantation and pneumonia, respectively. However, remaining 6 patients could be favorably controlled by HD. It was concluded that PCKD might be not so rare disease as we expected and when renal symptoms, such as hematuria, hypertension, or mild proteinuria are first encountered in middle age patients, PCKD should be suspected and differential diagnosis is essential from other renal diseases.

    The sphenoid sinus and the adjacent tissues were studied on specimens removed en bloc from 12 cadavers. Horizontal sections were made and were macroscopically and microscopically observed for surgical and anatomical studies. The optic nerve was frequently in contact with the lateral wall of the paranasal sinus which was paper-thin in sinuses with marked pneumatization. In some sinuses with marked pneumatization, the course of the internal carotid artery was in contact with the posterior wall of the sphenoid sinus. The clinical anatomy should be kept in mind to avoid complications during operative procedures. Knowledge of clinical anatomy is also useful in understanding the images of CT scanning of the paranasal sinuses and surgical aporoach of trans sohenoidal hypophysectomy.

    A rare case of thrombocytopenia due to Diphenylhydantoin (DPH) was reported. A 23-year-old woman was admitted to Kyorin University Hospital with chief complaints of palpitation and easy fatigability. On physical examination, she had no abnormal findings. The electrocardiogram showed frequent premature ventricular contraction. Roentgenogram of the chest also disclosed cardiac enlargement. Immedate use of Disopyramide did not improve her premature ventricular contraction even on the second hospital day. Therefore use of DPH was started on the 15th hospital day. After the DPH therapy, number of premature ventricular contraction were gradually decreased. 18 days later, gingiva bleeding appeared and platelets counts markedly decreased (0.5×10^4/cmm). Increase of immature Megakaryocyte was observed comparing with mature type. As drug-induced thrombocytopenia was therefore suspected, DPH therapy was switched to Prednisolone. After 4 days, platelets counts recovered to the level of 19.2×10^4/cmm for 0.5×10^4/cmm. In order to clarify the tolerance test were performed. In this case, tolerance test indicated DPH as the thrombocytopenia-inducing factor. This reports have indicated that hemotologic complications occur during relatively early during DPH therapy and they usually are associated with hypersensitivity phenomena.

    Anionic sites in the inflammatory leukocytes from the two cases on osteomyelitis were examined by transmission electron microscopy. The leukocytes were taken at operation from pus which had been formed in the infected region. The cells were washed, and then incubated with cationized ferritin (CF, 0.5mg/ml, pH 7.0) at 37℃ for 30 min before or after glutaraldehyde fixation. CF particles were distributed continuously on the entire surface of the prefixed cells. In the leukocytes incubated with CF before fixation, CF particles were clustered at the cell surface, remarkably internalized, and contained in vacuoles and multivesicular bodies even near the nucleus. The results indicate that leukocytes on osteomyelitis actively interact with CF particles and endocytose them with in the cell as well as macrophages.
