    This paper describes the treatment of a partially edentulous patient with microstomia. A 65-year-old patient's face was severely burned when he was 2 years old. His mouth circumference was about 120 mm and perioral burn scar contraction strictly limited access to the oral cavity. Upper and lower preliminary impressions were obtained by using a sectioned stock tray for an edentulous patient. A sectional custom tray was made for a maxillary final impression. A larger section encompassed an area of the denture base and retainers. A small section encompassed a remaining dental arch. This technique minimized the influence of errors resulting from sectional impression. Two halves could be assembled and disassembled in the mouth and reassembled outside the mouth by LEGO plastic building blocks.
    An outline of the deture base was simplified by partial overdeture, which an isolated and mobile tooth was served as an abutment. An arm tip of the farthest clasp from the denture base travelled mesially so as not to injure the angle of the mouth. Upper and lower partial dentures could be carefully inserted and twisted into place. Upon reexamination 1 year later, the dentures were in good condition.

    Mouth guards were used by top athletes to study the changes in muscle strength. As a result, it was found that athletes that tended to bite when exerting strength experienced an increase in muscle strength. On the other hand, the muscle strength in athletes that did not bite when exerting strength tended to decrease. The effects varied depending on to the individual and the type of athletic activity, and the results varied, i. e., some athletes experiencing an increase in maximum strength, some an increase in average strength and some an increase in work strength. Thus it was concluded that there were individual differences in terms of the strength results.

    The effect of the color tone of the abutment structure, which may be a factor influencing the color tone reproduced with the Dicor system, was examined in the present study. The color tones of Dicor crowns made on spacer-applied working casts and those tried, with a try-in paste, on vital abutment teeth with healthy dentin or on various nonvital abutment structures were analyzed colorimetrically, and the following results were obtained.
    1. The L*, a* and b* values obtained by colorimetry of Dicor crowns made on the working casts were 73.97, 1.84 and 17.51, respectively.
    2. The L*, a* and b* values of Dicor crowns on vital dentinal abutments with a YB shade tryin paste were 69.19, 2.96 and 19.82, respectively.
    3. Among various abutment structures covered with Dicor samples with a try-in paste, MM showed the maximum difference and CF the minimum difference of colorimetric values from Dicor crowns made on the working casts.
    4. When compared with dentinal abutments covered with Dicor samples, MM showed the maximum difference and BF the minimum difference of colorimetric values.
    5. It is clear that, for shade taking in preparation of Dicor crowns, the vitality of the abutment tooth and, if nonvital, the type of material used for the abutment structure should be specified.

    This study was carried out in order to statistically observe crowns and bridges which were fabricated for patients at the Prosthetic Clinic of Matsumoto Dental College during the 6-year period from January 1987 to December 1992. Some of the result were as follows 1. During the 6-year period, a yearly increase was noted. In total, there were 2, 892 patients during that time.
    2. During the 6-year period, 5, 965 crowns and 1, 244 bridges were set. There was a noted increase in the final 2 years in the number of crowns and bridges that were set.
    3. It was found that the number of both crowns and bridges decreased for patients in their twenties. Taking all patients, into account an increase in hard resin facing crown and a decrease in porcelain fused to metal crown were observed.

    The influence of occlusal vertical dimension on the muscle fiber conduction velocity (MFCV) and EMG frequency parameters were studied in the human temporal muscle in four healthy subjects (21-29 yrs. old). The occlusal vertical dimension was changed from intercuspal position (ICP) to maximum bite-raising position (22.0mm) by applying six different occlusal splints to the habitual opening-closing movement path. The EMG activities at 10, 30 and 50% maximum voluntary contraction were measured at each vertical dimension. The EMG signals were detected with a surface linear electrode array with 7 contacts placed along the muscle fibers. The MFCV was calculated from the time delay between adjacent EMG signals. The frequency parameters were 10, 25, 50 and 75% cumulative power functions and mean power frequency.The results obtained were as follows:
    1. The MFCV was increased according to the increase of the contraction level, but decreased accoding to the increase of the occlusal vertical dimension.
    2. Each frequency parameter was shifted lower according to the increase of the occlusal vertical dimension.
    3. The significant correlation (p<0.05) between MFCV and frequency parameters was shown at a high contraction level.

    The purpose of this study was to clarify the mechanical characteristics of dental implants using Bioactive Glass.
    The value of the load bearing strength determined by the breaking strength test was used as a base to observe and analyze the implant through the 3-D Finite Element Method. These results were then compared to the analytical results from an implant model and evaluated.
    Results showed that fracturing of dental implants or implants coming loose was solely dependent upon where the load was applied, load angle, and direction of load application.
    Placement of the dental implant is determined by taking into consideration the direction of the occlusal force.The direction of load bearing in relation to occlusal contact on the outer oblique plane of the cusp and midportion of the occlusal plane was found to be desirable between 90°to 135° and 60°Cto 120°C, respectively.

    A new primer for tooth-bonding was developed.This primer consists of phytic acid (PYA), which acts as both etchant and chelating agent, and SnF2 as a source of metallic ion. First, human teeth are treated with 7% aqueous solution of PYA and then with 3% aqueous solution of SnF2.It is expected that Sn2+is fixed on the tooth surface by chelation with PYA in this 2-step treatment, and then some interactions between phosphoric or carboxylic acid groups of functional monomers or polycarboxylic acid contained in cements and Sn2+contribute to bonding of cements to the teeth at cementing.
    In order to confirm this hypothesis, this primer was applied to bonding of 4 adhesive resin and one glass ionomer cements to human dentin.The treatment effects were compared with those of the conditioner (control) attached to each product.
    The following results were obtained:
    1.The early shear bond strength (after 24 hours in an atomosphere of 100% RH at 37°C) obtained from specimens treated with PYA-SnF2 primer was equal to or higher than that of the controls in all cements examined.For Panavia 21, Imperva Dual/Bond and Fuji Bond, there were statistically significant differences between treatments with PYA-SnF2 primer and the attached conditioners.
    2.After 5, 000 thermocycles, the shear bond strength to dentin treated with PYA-SnF2 primer was equal to or higher than that of the controls.For both treatments, however, the shear bond strength decreased significantly in comparison with that in early stage except for Panavia 21 with both treatments and Bistite Resin Cement with PYA-SnF2.
    3. It is suggested that PYA-SnF2 primer effectively bonds both adhesive resin and glass ionomer cements to dentin and provides durable bonding.

    The effects of age and denture-wearing on the composition of the oral microflora in molar plaque and whole saliva were investigated in 40 healthy subjects, 10 of them wore partial dentures.The 30 healthy subjects were divided into three age groups: 20-29 years (group A), 40-51 years (group B) and over 70 years (group C).The denture-wearing subjects were divided into two age groups: 40-52 years (group B′) and over 70 years (group C′).
    The total viable counts and the number of predominant organisms were similar in all age groups and denture-wearing groups. Whereas some minor groups of aerobes in the oral microflora, i.e., lactobacilli and staphylococci, significantly increased detection frequency (p <0.05) of groups C and C'. The detection frequency (p<0.05) of Candida and lactobacilli in saliva were significantly higher in denture wearers.However, the number of detected organisms was small.The findings of this study indicated that no major changes occur in the oral microflora according to age and denture-wearing.

    In order to clarify the influence of the examiner's standing position during palpation (right side of simulation model: PR, central: PC, left side: PL) on the palpable pressure speeds of fingers, 10 dentists of 10.3 year-mean clinical experience were examined for palpable pressure speeds of second fingers (F 2) and third fingers (F 3) of left (L) and right (R) sides of a simulation model.
    The following results were obtained.
    1. Palpable pressure speeds of F 2 and F 3 were influenced by the examiner's standing position.
    2. For both F 2 and F 3, pressure speed was higher for L than R in PR and for R than L in PL or PC.
    3. Pressure speeds of F 2 and F 3 in PC showed small laterality.
    4. Palpable pressure speeds of F 2 and F 3 showed greater laterality in PR and PL than in PC.

    Various attempts have been made to investigate what effects the constitutional elements of a partial denture have on the abutment teeth: how the difference in its design, retention and supporting method affects the abutment teeth. No analysis, however, seems to have been reported as to the difference in the capacity of load that the abutment teeth have and the force against the periodontal tissues, having different dental root shapes.
    When examining, under a condition related to clinical cases, what effect the design of the partial denture has on various dental root shapes of abutment teeth, we tried to measure the values of movement under a given load, having different supporting alveolar bone levels and different dental root shapes. Lower second premolar teeth were chosen as comparison teeth and the Sirognathograph analyzing system was employed as a measuring system.
    The obtained results are as follows:
    1. As the supporting alveolar bone level became lower, the value of the movement had a tendency to increase.
    2. In comparison of the partial denture with the vertical tooth root, the one with the curved tooth root under a given load showed lower values in both the mesial and distal movement and the vertical movement.
    We can conclude, from the above results, that in designing a free-end saddle denture not only the screening of the condition of periodontal tissues is necessary but due consideration should be given to the shape and the curvature of the dental root.

    Cleft lip and palate patients show several characteristic clinical and functional findings, such as:
    a. Cross bite, open bite and unstable occlusion in ICP.
    b. Tight upper lip, midface retrusion and scar contracted vestibular sulcus.
    c. Malposition of teeth, oronasal fistula and velopharyngeal incompetence.
    d. Relapse of teeth and segment.
    To consider a prosthodontic therapy to be successful, two factors must be satisfied:
    1. Reconstruction for optimum occlusion: stable occlusion in ICP and anterior guidanceproviding harmonious jaw movement.
    2. Prosthesis must satisfy requests of esthetics, comfort, simplicity and durability.
    In this study, the clinical and prosthetic findings of 10 patients before and after prosthetictreatment were compared.
    Results:
    1. All patients were satisfied with the given prosthodontic treatment.
    2. In 2 patients, both factors were established.
    In 4 patients who had anterior malocclusion, stable occlusion in ICP was established, butoptimum anterior guidance was not.In 4 patients who had severe malocclusion, optimum occlusion was not established with oralreconstruction.
    3. Many patients who had upper malpositioned teeth, oronasal fistula, relapse of teeth andsegment showed that their prostheses were more complicated and larger.
    Therefore, we reconfirm that prosthodontists, oral surgeons and orthodontists must coordinate and discuss how to accomplish a successful rehabilitation of cleft lip and palate patients.

    This study was designed to clarify the standards for functional evaluation of the anterior guidance. The rotation and translation of the mandibular head in protrusive movement were measured with a simplified condylar movement recorder using the dual axis graphic system.
    At the edge-to-edge position, a significant correlation was noted between the sagittal condylarguide inclination and the incisal guide inclination, but the rotation of the mandibular head inprotrusive movement was variable because of curves of the condylar path and incisal path. Thechanging aspects of the rotation of the mandibular head could be classified into three types.Nevertheless, changes in the rotation of the mandibular head did not go beyond a certain range, suggesting that there are some limitations on the change in the rotation of the mandibular head.
    From the above, it was concluded that there is something characteristic about the rotation of themandibular head from the intercuspal position to edge-to edge contact position in protrusivemovement. This suggests that it is important for us to understand the phases of rotation andtranslation of the mandibular head in the evaluation of the function of anterior guidance.

    Adequate bone response after implant placement is that of remodeling with gradual functional adaptation to occlusal stress. Conversely, overstressing the implant causes bone absorption.We should perform prosthetic management so that adequate occlusal stress can be generatedon the implant. We designed a new implant-bone model made from urethane foam as cancellousbone and acrylic resin as cortical bone. An Apaceram implant was placed in the center of themodel. Seven superstructures were fabricated from the following materials: complete alloy (AL), complete hard resin (HR), complete polycarbonate (PC), hard resin-veneered (HV), polycarbonate-veneered (CV), porcelain-veneered (PV), and Biotron R 70-lined (BL). A small accelerometerwas attached to the abutment and two strain gauges were placed on the surface of the model.A metal rod with an accelerometer was dropped on the superstructure. Stress-absorbing effect ofthe materials was evaluated by four signals.
    Results:
    1. Abutment acceleration of BL was the lowest of all specimens. The accelerations of BL, PC, HR, CV, and HV decreased in this order by about 70-80% when compared to that of AL. Theacceleration of PV was almost the same as that of AL.
    2. Rod accelerations of PC, HR, CV, and HV decreased in this order by about 70-80% whencompared to that of AL. Rod accelerations of PV and BL were almost the same as that of AL.
    3. Influence on strains of the model surface was lower than that on the acceleration. Strainsof BL decreased by about 20% the lowest of all specimens. Strain increased because the loading sitewas close to the strain gauge.
    4. The superstructure lined with Biotron R 70 seemed to show the highest shock-absorbingeffect of all samples.

    Recently, conical telescopic denture has been applied in many clinical treatments. However, it has many problems that need to be solved concerning retentive force of conical doublecrown.The purpose of this study was to clarify the correlation between retentive force and variousfactors affecting conical double-crown. The various factors that were considered to relate to thisproblem were fabricating material, taper angle, height of cone and load.
    The results were as follows:
    1. Retentive force was the most stable in the Plastic sheet method.
    2. Retentive force decreased as the taper angle was iocremm^d, and this effect became greateras the height of cone and load were increased.
    3. Retentive force showed an increasing tendency as the height of cone was increased, and theincreasing rate became greater as the height of cone and load were increased.
    4. As to the axis mnoa, retentive force slightly increased as the axis area was iocreammd, butunder 250 mm2, the increasing rate was little, and over 250 mm2, the increasing rate became greater.The rate became greater as the load was increased.
    5. The retentive force increased in proportion to the load, and this effect became greater as thetaper angle was decreased and the height of the cone was increased.
    6. In case of taper angle 2° and 4° the retentive force depends on the load rather than the heightof the cone. In 6°, 8° and 10°, the retentive force depends on the height of the cone rather than the load.

    Tooth mobility indicates a functional situation of periodontium and it is important to grasp tooth mobility in clinical prosthodontic treatment.We have already developed an automaticdiagnostic system for tooth mobility and an analysis of periodontium was carried out in terms ofperiodontal viscoelasticity using this diagnostic system.It is considered that viscoelasticity ofhealthy periodontium has a certain range according to each tooth group, age and sex.In this study, we attempted to establish the normal range of viscoelasticity in periodontally healthy teeth innormal subjects.
    Normal ranges were calculated with the parametric statistics on 101 upper central incisors, 105upper canines and 107 upper first molars with healthy periodontium of 57 subjects aged between 21and 28 years in both sexes, using the automatic diagnostic system for tooth mobility.
    Significant differences were found among tooth groups in all parameters with the Kruskal-Wallis test (p<0.05).Significant differences were found between sexes in c, of central incisors, c2of all tooth groups and k of first molars with the Mann-Whitney test (p <0.05).Normal rangeswere calculated separately from sexes and tooth groups.We calculated normal ranges with mean ±2 × standard deviation, after carrying out the Smirnov tests and the distribution pattern tests.

    A study was carried out on the analysis of thermal conductiveness of wax carvers by means of the two-dimensional finite element method.
    The results obtained were as follows:
    1. A comparison of the temperature at the tip of the 1 mm width and the 2 mm width waxcarvers revealed that changes in the temperature occured more rapidly in 1 mm width wax carversthan 2 mm width wax carvers.
    2. Thermal changes at the tip of wax carvers showed that as the heat moved to the tip of waxcarvers, thermal changes became faster. When the heating area is far from the tip, thermal changeswere seen to be slow.
    3. In both types of wax carvers, when heat was applied up to 10 mm from the tipof the head, the entire head was found to become uniformly the same temperature.
    4. A comparison between when both heads were heated at the same temperature andwhenthey were both heated at different working temperatures while wax covered was conducted. It wasseen that the working temperature of the 2 mm width wax carver lasted longer than that of the 1 mmwidth wax carver.
    5. According to the result we obtained, it was concluded that it is necessary to choose a waxcarver which has been heated from the 10 mm range to the tip of the wax carver and can maintainthe proper range of working temperatures for the specific purpose encountered.

    In order to establish a simple and yet reliable objective method for evaluating masticatory function, quantitative evaluation of masticatory function was attempted by analyzing the amount of glucose discharged during gumi-jelly chewing. 20 normal subjects were asked tochew gumi-jelly and the respective correlation between the amount of glucose discharge andmandibular movements as well as masticatory muscular activities were investigated. As a result, by chewing a piece of 2 g cylindrical gumi-jelly, 10 mm in both height and diameter, it was clear thatthe amount of glucose discharge was proportionally related to both the path andrhythm of themasticatory movement and the amount of masticatory muscular activity. Therefore, it was suggestedthat it was possible to establish a quantitative method for evaluating masticatory functionincluding masticatory efficiency, movement function, and masticatory muscular activity, in otherwords, a simple and reliable method for objectively evaluating masticatory function by analyzing theamount of glucose discharge.

    A thorough grasp of the dynamic correlation with the articular frames in the mandibular movements is useful for the diagnosis of craniomandibular disorders using the cine images in MRI examination. The present article was investigated in comparison with the cineimages obtained from three directions, which were lateral pterygoid muscle, mandibular ramus, andthe mandibular movements to obtain transversal images.
    The results obtained were as follows:
    1. The slice in the direction of the lateral pterygoid muscle was very advantageous forobservation of the lateral pterygoid muscle and the maxillary artery.
    2. The slice in the direction of the mandibular movements had a slight predominance over theother two slices with respect to the dynamic grasp of positional relationships in the articular frames.
    These findings suggested that the slice direction suitable for scanning in cine-MRI should beselected.

    The replacement of soft lining material is necessary for a better clinical effect. However, the changing work brings about a very unhygienic environment and demands much labor.If we can peel the material from the denture base without a cutting motion, the situation will bebetter.
    In this study, bond strength of three temporary soft lining materials (Soft-Liner, Visco-gel, trialmaterial) to a denture base (ACRON) with three separators (Vaseline, experimental separator, Visco-gel Lubricant) were examined in tension and shearing tests and observed for separationaspect.
    The results were as follows:
    1. Separation effect of Vaseline showed the highest value in all conditions, followed by that ofexperimental separator and Visco-gel Lubricant.
    2. In bond strength, the same tendency was observed in between tension and shearing tests.
    3. In the tension test, the separation aspect of Visco-gel Lubricant showed cohesive failure inall specimens, Vaseline showed interfacial failure in all, and experimental separator showed mixedfailure in half, however, the percentage of cohesive failure was only about 2.5%.
    4. In the shearing test, separation aspect of Visco-gel Lubricant showed cohesive failure and Vaseline and experimental separator showed interfacial failure in all.
    5. Experimental separator showed about 70% bond strength as compared with control.

    The purposes of this study were to describe a new procedure for temporomandibular joint (TMJ) sagittal tomography and to evaluate its standardization and reproducibility.
    One dry skull was placed into a newly constructed head holder, and TMJ sagittal tomographythereof was performed during hypocycloidal movement.
    The results were as follows:

    Soft denture liner has benefits such as improvement of comfort of the denture for the patient and reduction of residual ridge resorption by reducing impact stress and extending the loadbearingarea in the supporting structures during function. Laboratory processed materials are usedas long-term denture liners for management of supporting structures that are susceptible to trauma, pain and ulceration. The purpose of this study was to predict the effect of the lining method of thesoft denture liner on the stress in the supporting structures by using viscoelastic finite elementanalysis.
    The results were as follows:
    1. When the soft denture liners were added to the denture base by three difference liningmethods, the stress distribution in the supporting structures changed, Model 2 showed the mostuniform stress distribution in the soft tissue.
    2. When the soft denture liner B was added to the denture base, the stress intensities in thebone under dentures with a soft denture liner increased at 3.0 s after loading.
    3. The displacement of the denture of Model 2 was the least. The direction and amount ofdenture movement were related to the lining method.
