    In order to develop the health care system for the elderly we need to analyse fundamentally the phenomenon that the average age of population in each community is rapidly becoming older, as well as we have to grasp life conditions and health needs of old people. We investigated the rate of increase or decrease of population, rate of the elderly living by themselves and the index of the aging of population of each community, classified these communities into several types, and presented the points necessary to develop a community-based comprehensive health care for the elderly as to each type by considering the meanings the indices of health care, medical care and social welfare of the communities disclose.

    Recently, while the medical technology developed remarkably on the one hand, the soscial need for preventive medicine largely increased on the other. In Japan, it is an urgent problem of the medical education how to create effective methodology and technology of the preventive medicine that can attract students who generally prefer to become professionals in clinical medicine.
    Since 1978 we have practiced an educational programme in which each student looks after one family picked up from the agricultural district for one year, caring for the health of the family members and helping them out of health problems, if there are any. In this way students can learn primary health care and community health of their own will.
    Compared with the traditional method, this method is effective in making the students understand with real interest the meanings of comprehensive primary health care through concerning themselves in the health care of a family as “a socio-economical-psycho-complex”.

    In order to observe the effect of organophosphorus compound, cyanofenphos (0-ethyl 0-4 -cyanophenyl phenylphosphonothioate) (CYP) on the glucose metabolism, 20 mg/kg of CYP was repeatedly administered per os to male Wistar rats, weighing approximately 150g. The supression of the serum and brain cholinesterase activity increased with times of administration, but muscular strength of extremities was not suppressed. The concentration of serum glucose, however, was enhanced in spite of the increase in the concentration of serum insulin. The mechanism of hyperglucosaemia resulted from the supression of the incorporation of glucose into cells in peripheral tissues without the activation of glycogen phosphorylase and suppression of glycogen synthetase activity. It was suggested that the suppression of the incorporation of gulcose into cells resulted from inhibition of insulin receptors or entry sites of glucose in cells by cyanofenphos.
    The relation between disturbances of the glucose metabolism and the delayed neurotoxicity was discussed.

    Relationship between SGOT/SGPT ratio and energy intake were investigated in 1030 subjects (373 males, 657 females). These figures were taken from epidemiological survey of the Tokushima prefecture from 1975 to 1980.
    The results of the survey were as follows.
    1) Energy intakes in reversal group (SGOT/SGPT <1) was higher than that in nromal group (SGOT/SGPT >1).
    2) Body weight in reversal group was higher than that in normal group except young woman (18-39 years).
    3) Incidence of reversal of SGOT/SGPT ratio showed increasing values as energy intakes levels increased.
    It is possible to conclude from the results that SGOT/SGPT ratio was affected by energy intakes.

  • Comparison of Cancer Cases Detected by Gastric Mass Survey and Found Among Outpatients

    Since Sep. 1978, 123 patients of gastric cancer were operated in our hospital, 32 patients of them were detected by gastric mass rurvey (MS group) and 91 were outpatients (OP group).
    Clinically and histologically, we have reviewed and compared the two groups about the process of detection, the treatment and the prognosis.
    Results are as follows.
    (1) There were much more patients in MS group than OP group who had no symptom and no complaining period and who had experienced mass survey more frequently. It resulted that the ratio of early gastric cancer of MS and OP group were 68.8% and 33.0% and those who received curative resection were 96.9% and 61.5% respectively.
    (2) The cancer occupying the upper third of the stomach was rarely seen and tumorsize less than 0.5 cm was never seen in MS group, but the both were not rarely in OP group which had been examined mainly by endoscopy.
    Of course we know that we must make efforts to find these lesions from mass survey. But endoscopical method is more profitable than indirect fluoroscopy to find fine lesions, so it is suggested that endoscopy mass survey will be more useful.
    (3) According to The General Rules For The Gastric Cancer Study In Surgery And Pathology (Japanese Research Society For Gastric Cancer), we compared the prognosis of the two groups concerning 4 matters of the patients who were received absolute curative resection, whose cancer invaded to serosa, who had no evidence of regional lymph node involvement and whose histological stage was I or II.
    The result was that MS group was superior to OP group on all matters.
    The causes, we supposed, were the facts of OP group that many patients were in bad preoperative condition, so the dissections were not made successfully, and that many died of other malady.
    Five year survival rates of MS and OP group were 89.8% and 39.0% respectively.

    It was reported that steroid withdrawal therapy was beneficial in the management of patients with HBsAg-positive chronic active hepatitis and rarely led to fulminant hepatitis. We report a caseof chronic active hepatitis type B who developed severe liver dysfunction by steroid withdrawal therapy, and died one month later.
    A 37-year-old man with HBsAg-and HBeAg-positive chronic hepatitis received steroid withdrawal therapy at the time of elevated transaminase (S-GOT 952u, S-GPT 877u).
    The dosage of prednisolone was 40 mg/day at the first week, then decreased by 10 mg everyweek. Total doses were 700 mg in 4 weeks. A rebound S-GOT peak was observed at one week after the discontinuation of prednisolone. At this time, he complained of increasing general fatigue and loss of appetite. The laboratory data showed severe liver dysfunction (Prothrombin time 38%, Hepaplastin test 19%). Ascites was observed by the abdominal CT scanning. Then total bilirubin level increased gradually and urine volume decreased. Finally he died at one month after the discontinuation of prednisolone.

    A total of 55 inpatients (62 admissions) have been treated over the past one year at a clinic on an isolated island. Forty cases were dealt with in the clinic alone, whereas the remaining 22 were transferred to a supporting hospital. The former 40 included cases of mild conditions, slightly complicated moderate or severer conditions, terminal care, childbirth, minor surgery, etc.; those for whom treatment was possible because of the presence of hospital care facilities were numerous. If our clinic had had no such facilities, the number of cases treatable on the island would have decreased to 10, with about 50 cases of transfer to the mainland. The number of calls for an ambulance helicopter would also have increased markedly.
    Thus, the presence of hospital care facilities was confirmed to be important in order to perform a satisfactory level of community medicine oriented toward self-sufficiency in remote places such as isolated islands.

    In order to clarify the actual conditions and mechanism of contact dermatitis from pesticides, a field survey including patch testing with 11 pesticides and formaldehyde was carried out on 111 subjects who engaged in farming in the plastic greenhouse mainly. The relationship between skin sensitization due to pesticides and conditions of pesticide exposure was also studied. The results were as follows;
    1) About half of subjects (52%) have been suffered from health hazards due to pesticides. Among the symptoms complained, dermatitis was most frequent (32%) as compared with acute poisoning such as headache (12%), general fatigue (9 %) and so on. The causative pesticides of those dermatitis were mainly Difolatan (72%), Daconil (8 %), and Gramoxone (8%).
    2) Relatively higher rates of positive reaction of patch testing were observed in Difolatan (43%) in male, and Difolatan (25%), Topsin M (24%), Rovral (24%) in female. A positive reaction with more than one substance tested were seen in 31 males (55%) and 31 females (55%).
    3) Among 11 kinds of pesticides tested, positive correlations between the skin reaction of patch testing and the use of corresponding pesticides were found only in Orthocide (r = 0.233) and Daconil (0.203).
    4) From the Cluster Analysis, 16 kinds of using pesticides were classified into 3 clusters with similarity to patch test reactions. It was also suggested from the Dendrogram that there might be cross reactions among patch testing pesticides.
    5) By Multiple Regression Analysis, no factor which contributed remarkably to skin sensitization was clarified. And, it was suggested that many kinds of factor were complicatedly related to skin sensitization.

    For the security of water quality in rural areas, adsorption removal of phenol which is one of disinfectants for excretions by six kinds of commercial activated carbon was investigated.
    At equilibrium concentration of 1 and 10 ppm, activated carbon indicated pH 10.19 adsorbed phenol as much as 2.17 and 1.84 times compared with activated carbon indicated pH 3.06, respectively. At low equilibrium concentration of phenol, the amount adsorbed was significantly influenced by surface pH of activated carbon (p <0.05).
    These results suggest that the difference in amount of phenol adsorbed onto activated carbon is due to the interaction between phenol and surface oxygen groups on the adsorbent. Therefore, it is concluded that the activated carbon of which surface is basic suitable for the adsorption removal of phenol in the agricultural waste waters.
