  • Coronary Artery Bypass Grafting to Completely Obstructed Coronary Artery

    The effects of coronary artery bypass grafting (CABG) to conpletely obstructed coronary arteries (COCA), with angiographically demonstrable collaterals distal to the occulusion, on the lef ventricular contractility and postoperative symptoms were studied in 18 patients consisted of 10 with transmural myocardial infarction (TMMI) on ECG in the area perfused by COCA and 8 without TMMI. Of 19 CABGs including 13 to left anterior descending coronary artery (LAD), 5 to right coronary artery (RCA), 1 to left circumflex coronary (LCX), all 13 grafts to LAD and 1 to LCX were patent, whereas 2 of 5 to RCA were obstructed on the postoperative angiogram. The following evaluations were undertaken in 16 patients with patent grafts. In 8 patients with TMMI, left ventricular ejection fraction increased from 0.56±0.08 (Mean±SD) preperatively to 0.65±0.07 postoperatively (p<0.005), left ventricular segmental wall motion improved from 21.6±6.7% to 29.6±6.5%(p<0.01). Angina disappeared postoperatively in all patients but one with TMMI. All patients showed clinical symptoms of NYHA class III or IV preoperatively, which were improved to be of class I or II postoperatively. In conclusion, it was found that CABG to COCA, especially to LAD was associated with an excellent graft patency rate and with significant improvement of left ventricular contractility, even in the patients with TMMI.

    In search of reasons for regional differences in the death rate from stroke, an extensive factfinding project was carried out in Yamagata Prefecture. Out of the total 44 municipalities in the prefecture, we picked out four townships; two are noted for their high standard mortality rate from cerebrovascular disease and the other two have a low mortality from the disease. In these four townships, a thorough investigation was conducted on the incidence of stroke, along with health examination for cardiovascular disease. The findings are as follows;
    1. Despite the fact that Yamagata Prefecture has a high rate of stroke, blood pressure levels stood on a par with, or even lower than, the national average.
    2. There were no correlations between the standard mortality rates and blood pressure levels, ECG readings, funduscopic observations, and total serum cholesterol levels.
    3. By occupation, white-collar workers showed higher values than blue-collar workers and farmers in diastolic blood pressure, obesity, and cholesterol levels in the blood serum. The occupational difference was observed in each and every one of the four townships.
    4. The standard mortality rate and other fatality indices were not correlated with the incidence rate of stroke among the middle-aged and the rate of detection in the health examination for the conditions that might lead to stroke. Interesting to note is the fact that the incidence rate among the middle-aged was parallel with the detection rate. This finding suggests that further study of the incidence rate and the prevalence rate in necessary.

    The effects of the serum vitamin C levels on the activity and isozyme pattern of serum lactate dehydrogenase (LDH) in males and females in their middle and old age were examined.
    There was no siginificant correlation among the total serum, LDH activity and serum vitamin C levels in the 87 male and 83 female ranging from 45 to 84 years of age. However, high inverse correlations were found among the percentage of LDH-4 and LDH-5 and serum vitamin C levels all of age groups (45-59 years of age group, 60-69 years of age group and over 70 years of age group) of males and females.
    These results suggest that the percentage of LDH-4 and LDH-5 are influenced by the vitamin C intake in males and females in their middle and old age.

    The atmospheric pollen counts of Ceder (Cryptomeria japonica) and Cypress (Chamaecypanis obtsusa) were measured with Durham's standard devices installed on the roofs and ground in Yanai city, Yamaguchi prefecture, during the period between 1984 and 1987, and the following results were obtained.
    1) The number of atmosphreic pollen counts changed significantly every year. There was a large quantity of pollen counts recorded in 1984 and 1986, but a small quantity in 1985 and 1987. 2) Scattering period of atmospheric pollen counts also changed every year. In 1987 the atmospheric pollen began early in February. In 1985 and 1986, it began in late February and in 1984 in late March.
    3) The atmospheric pollen counts on the roofs were recorded 3 times as much as those on the ground. But there were no substantial difference between those on the roofs and ground concerning the scattering peroid of atmospheric pollen and the daily change of it's counts. There was observed significant correlation between those on the roofs and ground.(coeffecient of multiple correation=0.987)
