    The rate of mortality from paraquat poisoning is high and the principal manifestation of toxicosis is pulmonary impairment.
    In this study, we measured the concentration of paraquat in the urine, blood, and some organs in 10 patients with acute paraquat poisoning in order to study the relationships of the kinetics in the body to the concentration in the lungs and to the pulmonary impairment.
    The concentration of paraquat dichloride in the blood stream rapidly decreased in the first 24 hours. The half life was about 0.5 hour, within 2 hours (case 3) after intoxication, about 1 hour after 5 hours (case 5), about 2 hours after 10 hours (case 6) and after 24 hours (case 6) was prolonged to over 26 hours.
    Therefore, it was suggested that half life was subsequently prolonged with the passage of time.
    There were indications that renal disturbance appeared in about 5 hours after oral intake.
    Accumulations of paraquat were seen in the organs examined. The largest amount within 24 hours after exposure was recognized in the lungs, and the largest amount after 271 hours was seen in the skeletal muscle. The concentration of paraquat in the lungs was higher than the concentration levels in the blood at death in all the cases.
    Although excretion of paraquat from the lungs was very slow. it became faster with DHP and HD. However, excretion from the skeletal muscle was slow, Excretion from the other organs excepting the kidney was considered to be constant, being proportional to the blood concentration levels.
    The pathologic examination of the lungs found congestive edema in 6 cases, which became severer with the lapse of time. Partial pulmonary fibrosis was recognized only in long-term survival cases.
    The cause of irreversible pulmonary impairment was considered to be the effect of the small amount of paraquat remaining in the lungs which exceeds the concentration in the blood.

  • Effectson Free Thyroxine Index of Iodine Through Wakame Seaweed

    In this study, the effects of excessive iodine intake on free thyroixine index (FTI) were studied, and the results are presented in the following.
    FTI tended to decrease during the basal diet period without wakame seaweed and the decrease tended to continue further during the periods of TEST 1 (wakame seweed 10g/day) and TEST 2 (wakame seaweed 20g/day). The trend changed in the period of TEST 3 (wakame seaweed 40g/day), and FTI rose slightly. When the basal diet was resumed after completion of experimental diets, the level returned to the base line levels measured before the beginning of the experiment.

    A total of 1, 236 cases of gastric cancer operated at Hiraka general hospital during the 14 years from 1975 to 1988, were analyzed and the following results were obtained.
    1. There was a distinct tendency to increase in the numbers of early-staged cancer cases and patients over 70 years of age.
    Any decrease in the number of operated gastric cancers could not be obeseverd during this period.
    2. The ratio of patients aged over 70 years increased to reach 26.4% recently and the ratio of early staged cancer increased to 59.3% in 1988.
    3. The over all resection rate was 90.8% and curative resection was performed in 85.2% of resected cases.
    4. The cumulative 10-year survival rate in curative resected cases was 74.0%.
    5. The rate of the multiple cancer cases was as high as 5.0%.

    A total of 417 operated cases of colorectal cancer at Hiraka General Hospital during the 15-year period from 1974 to 1988 were analyzed and the following results were obtained.
    1. There was a distinct tendency to increase in the numbers of resectable cases. By region, cases of cancers in sigmoid colon and upper third of rectum (S+Rs) and right side colon (C+A) were on the rise.
    2. The ratio of patients aged over 70 years increased to reach 36 % recently. But the ratio of early stage cancer cases was 7.1 % on the average.
    3. The over all resection rate was 90.6 %, and curative resection was performed in 78.5 % of colon cancer cases and 82.0 % of rectal cancer cases respectively.
    4. The cumulative 10-year survival rate in curative resected cases was 74.7 % for colon cancer and 70.8 % for rectal cancer.
    5. The rate of multiple cancer cases was as high as 13.4 %.

    Habitual smoking adversely affects the functions of the respiratory system. A Survey was conducted on 100 patients with respiratory diseases in the authors' institute regarding individuals' smoking habit and the following results were obtained.
    1) Some respondents say they have difficulty abstaining from smoking though they perceive its unfavorable effects on their health. However, they feel a persuasive “recommendation to give up smoking” by the attending physicians a great help, and 2) earnest instruction and education for the patients are very important as many patients feel good after giving up smoking.
    The relationship between habitual smoking and carbon monoxide (CO) concentration in the expired air was determined in 67 healthy volunteers with an ambulatory CO analyzer and the following results were obtained.
    1) There was a significant difference in the CO concentrations in the expired air between non-smokers and smokers, 2) there were positive relationships between the CO concentrations in the expired air and daily numbers of cigarettes smoked and smoking index, 3) problems relating to passive smoking became evident, and 4) the ambulatory CO analyzer for the determination of the concentration of CO in the expired air is considered useful for the anti-smoking guidance and instructions.

    The subjects are 223 women who participated in a mass medical examination. Their ages are between 40 and 79 years. We divided them into four age groups, and studied how aging and obesity influenced their physical health. The results showed the obesity index and serum total protein level were significantly low in the 70-to-79 age group. The serum total cholesterol level was significantly high in the age group between 40 and 50, and it was thought that the elevated cholesterol levels were due to menopause. Thus in females this suggests that a great physical change occurs first in the age group between 40 and 50, and second in the age group between 60 and 70.
    Using 165 female cases whose ages were between 40 and 69 years were subdivided into four obesity index groups, we studied how the obesity influenced their physical health. The results showed that the problems include the elevation of diastolic blood pressure levels and serum uric acid levels.

    The main purpose of gastric mass screening is early detection and prompt treatment of stomach cancer and ultimately, the reduction of mortality from stomach cancer.
    Therefore, we must have to review the effectiveness of the screening program and the methods of gastric mass screening.
    Namely, it is essential to determine whether the mass screening is realy effective and whether it is appropriate to continue the program for secondary prevention of gastric cancer.
    It is most important to adjust and to analyze data after mass screening examination to evaluate the screening program and its accuracy. For example, the number of cases of stomach cancer detected by the mass screening, differences in the rate of surgical treatment, propotions of early ones to all stomach cancers detected, post-diagnostic survival rates, post-operative life, and, mortality decreasing effect of such screening.
    This time, we will report the results of gastric mass survey using the data in the Cancer Detection Center of Miyagi Cancer Society.

    A case of cancer of the urinary bladder in a 59-year-old man with incomplete L4 paraplegia is reported. He was injured in 1970. Since then, the case has been followed up on at our department. In October 1989, urinalysis showed microscopic hematuria. Urine cytology was class II -III. Endoscopically, a tumor was found contiguous to the right-side wall of the trigone and seemed to have invaded the trigone. Histopathological findings of the biopsied specimens of the tumor revealed TCC grade 1. He was treated by TUC and chemotherapy with PEP, MMC, VDR and THP-ADM. He has been free of disease for 12 months, but a solitary tumor reccurred on the left wall (TCC) ADC? G2), and treated by TUC.
    In this report we also review the referreces hitherto made to paraplegic bladder cancer.

    A 56- year-old woman visited our hospital complaining of right hypochondralgia. X-ray and endoscopic examinations revealed Borrmann 2 type gastric cancer in the anterior wall of an upper part of the stomach. Ultrasonography showed an echogenic mass in the suprarenal area. The mass which had septal formation within it, was about the same in density as fat on computerized tomography, and was hypovascular fed by the right inferior adrenal artery on angiography. These findings suggested that the mass might be adrenal myelolipoma. After total gastrectomy and right adrenectomy were done, histopathological study confirmed that the case is adrenal myelolipoma associated with Borrmann 2 type gastric cancer. In Japanese literature, only 54 cases of surgically resected adrenal myelolipoma have been reported, and this is the second case of adrenal myelolipoma associated with gastric cancer.
