    As part of the rehabilitation program for myocardial infarction patients, assisted exercise under the eye of therapists is an effective rehabilitation therapy during the period from hospital discharge to the returning to work. Regrettably, however, this part of the program is not put into regular practice in rehabilitation centers yet.
    Under the circumstances, we have conducted this type of therapy for myocardial infartion convalescents, and assessed it significance and benefits by examining therapeutic effects and changes in the endurace test results as well as changes in the cardiac output during exercise.
    In this study, we used data obtained from 25 out of the 68 acute myocardial infarction patients treated in the coronary-care unit of our hospital between Nov. 1, 1990 and May 31, 1992.
    Exercises were performed three times a week for eight to 12 weeks, kinetic intensity correspond-ing to 70-80 % of the highest heart rate in stress tests. The heart rate, blood pressure and cardiac output were measured based on ECG recordings.
    As a result, it was found that the kinesitherapy helps (1) increase the tolerability to exercise, (2) lower the heart rate, blood pressure and double product and (3) elevate the stroke volume index during exercise.
    From these findings, it is safe to say that the kinesitherapy can be given to myocardial infarction victims without causing them anxiety and is very helpful for them to restore good health and normal cardiac function and to resume their place in society.

  • K-gastroplasty

    Based upon the 8 years' experience in vertical banded gastroplasty (VBG) for the treatment of morbid obesity in our department, we analysed the clinical results after operation. As a result, the weight-reducing effect and safety of VBG were found favourvable, but some of complications developed in a few, causing serious conditions. Therefore, a modification of VBG was tried in order to utilize the characteristics of VBG and to lessen the factors of complications development as much as possible. Its essential points are to utilize the His-angle by making an oblique staple line instead of a vertical one, and to block the stomach with GIA90 and EEA28 and embed the staple line by suturing the serous membrane. Because the stomach is formed into a K-shape, it has been named k-gastroplasty. Its operative mode is discussed.

    The effect of daily physical activity on the serum lipid level was examined. This study consisted of two parts, an epidemiological study and a laboratory study.
    In the epidemiological study, we investigated the relationship between serum lipids and energy expenditure in young adults females. Significant correlations were found between energy expenditure/body weight and the levels of serum HDL-cholesterol and serum triglyceride, and atherogenic index.
    In the laboratory study, we examined the effects of exercise on the serum lipid level. Groups of subjects took part in the programs of physical activity of two different intensities for 6 weeks, and their serum lipid levels before and after these programs were compared. Exercise regimens of increasing intensity caused a significant mean increase in the serum HDL-cholesterol level. However, these regimens did not cause any significant change in serum total cholesterol and serum triglyceride values.
    These results suggest that doing even light exercise on a regular basis is effective in producing an optimum serum lipid pattern in young adult females.

    It is known that drug disposition is altered by concurrent administration of different drugs. Drug-drug interaction may also enhance a side effect that is linked to either drug. The purpose of this report is to demonstrate the usefulness of therapeutic drug monitoring (TDM) in the early detection of side effects induced by drug-drug interaction and its treatment.
    The combined use of rifampicin and mexiletine may require an increase in the dose of mexdletin by about 50% due to increased clearance resulting from enzyme induction. In addition, three days after discontinuation of the rifampicin therapy, the serum mexiletine level increased from 0.83 mcg/ml to 2.44 mcg/ml. The patient has developed a tremor. After discontinuation of mexiletine medica-tion, the symptom disappeared in two days.
    On the other hand, the patient who took theophylline and mexiletine together developed nausea, vomiting and tachycardia. Four days after initiation of the combination therapy, the serum theophyl-line level was in the toxic range of 27.3 mcg/ml. The patient's theophylline dose was decreased 25%, and side effects completely resolved. The serum theophylline concentration became normal (18.8 mcg/ml) seven days later.
    Whatever the mechanism, these drug interactions may be sufficient to necessitate the adjustment of drug dosage, preferabily in accordance with serum drug concentration levels. These results suggest that TDM is useful for the suppression of incidence of side effects by drug interactions.

    From July 1989 to May 1992. 16 cases of newly diagnosed non-Hodgkin Lymphoma (NHL) were treated with the following protocols. Patients of 69 years and under were treated with combined (1) CHVP, (2) CHOP, and (3) C-MOPP therapy. Patients of 70years and over were treated with the CHOP therapy.
    The remission rate was 87.5%. Two patients relapsed. They were both elderly patients (70>). The relapse rate was 14.3%. The relapse free survival rate was 71.3%. Now 75.2% of the patients of this study survive.
    Compared with other protocols, our strategy has been proved to be effective in terms of the remission rate.

    The amount of pesticide residues in the soil was investigated in two fields. About 0.05-0.14μg/g-dry of fungicide procymidone (SumirexTM) were detected from the soil, where fuming with procymidone had been ceased for about a year.
    Measurements have been carried out for the subsequent three years with the result that the second half-life of procymidone in the soil was estimated at about 1.5 years.
    Procymidone residues ranging from 0.005 to 0.017μg/g-raw were detected in the vegetables grown in these fields, but not in strawberries.
    These data suggested that procymidone falls under the category of long-lived organo-chlorine pesticides.
    It is necessary to study further on the environmental behavior and the toxity of procymidone, as procymidone residues in foodstuffs are reported repeatedly and the chemical is still on the market.
