      To assess the effect of pre-germinated brown rice on metabolism of glucose and lipids, blood parameters of glucose and lipids were measured before and after 3 months of intake of test rice, which was mixed with pre-germinated brown rice (PGBR) and white rice in a ratio of 1:1, in patients with diabetes mellitus type 2 (DM). Glycosylated hemoglobin A1c (HbA1c) was significantly decreased from 6.40±0.23% to 6.23±0.19 after 3 months of intake of PGBR. The fasting plasma glucose level was not changed by intake of PGBR, but serum insulin level and HOMA-IR were decreased slightly. As the decrease of LDL-cholesterol (LDL-c) and the increase of HDL-cholesterol (HDL-c) were slightly observed after 3 months of intake of PGBR, the LDL-c/HDL-c ratio was decreased significantly from 2.03±0.13 to 1.83±0.12. These changes were significantly larger in the high PGBR in take group than in the low PGBR in take group. These results suggested that the PGBR intake might have potentialities as one of therapeutic methods for diabetes mellitus type 2 and also be useful in the freatment of hypercholesterolemia.

      Background: The preferred management of localized prostate cancer is yet to be known, due in large part to a paucity of randomized controlled trials comparing benefits and risks across primary treatment options, such as watchful waiting, radical prostatectomy, radiation therapy and androgen deprivation.
      Aim: We reviewed treatment outcomes of retropubic radical prostatectomy for clinically localized prostate cancer patients.
      Methods and Subjects: We examined 63 patients with localized prostate cancers who underwent retropubic radical prostatectomy from January 2001 to December 2006. Mean age was 67.5 (range: 52-79 ages). Preoperative clinical stages were T1c (21 cases), T2a (29 cases), and T2b (13 cases).
      Results: Mean operation time was 261 minutes (range 194-340 minutes). Mean bloodloss was 1,123ml (range 135-3,500ml). As for Gleason scores of prostatectomy specimens, 32 cases had 6 or less, 25 cases had 7 and 6 cases had 8 or more. Pathological diagnoses were pT2a (31 cases), pT2b (4 cases), pT3 (28 cases), pN1 (2cases). As a result, 35 cases (55.6%) were defined as “organ confined diseases (OCD)”.
      Early complications occurred 16 cases, including rectal injuries (2 cases), wounds infections (10 cases), anastomotic stricture (2 cases) and epididymitis (2 cases). Slight and mild urinary incontinence (need of two or more pads per day) were recognized in 15cases (24%) as a late complication 3 months after operation, in 2 cases (3.2%) after 1year. Nerve sparing radical prostatectomy was performed on 28 cases. Six of them could keep erectile function with support of Phosphodiesterase type 5 (PDE-5) inhibitors.
      Biochemical recurrences were found in 12 cases, so that hormonal therapy was initiated as secondary cancer treatment. We investigated the predictors of PSA recurrence, and the OCD was a significant indicator for biochemical recurrence.

      Recently, the relevance of Helicobacter pyroli (H. pylori) infection to atrophic gastritis and gastric cancer in has been elucidated. Therefore, to diagnose H. pylori infection is important for suspecting the existence of those diseases of the stomach. We investigated the relationships of the H. pylori IgG antibody to H. pylori infection, atrophic gastritisor various stomach lesions to understand the significance and usefulness of evaluating the antibody level in the annual multiphasic health check-up participants. The subjects in this study were 148 people (male: 93, female: 55, average age: 56.4) in Toride city, which is located in the suburb of Tokyo. They had visited our hospital for medical checkups. Eighty-seven subjects underwent upper gastric endoscopy and 55 (63%) tested positive for H. pylori IgG antibody and H. pylori infection was confirmed in 41 (75%). Although 32 subjects tested negative for the antibody, 8 cases (25%) was diagnosed with H. pylori infection. The frequencies of positive antibody in atrophic gastritis, erosion and ulcer, fundic gland polyp, hyperplastic foveolar polyp, adenoma and malignancy were 17%, 70%, 61%, 77%, 31%, 62%, 0% and 100%, respectively. On the other hand of 80 subjects were who underwent barium X-ray examination, 44 tested positive for the antibody (55%). The frequency of positive antibody in no lesion, depressed lesion, elevated lesion and malignant lesion were 61%, 59%, 36% and 100%, respectively. Taken together, H. pylori antibody was a good marker for H. pylori infection and the positive antibody indicated high frequencies of atrophic gastritis and malignancy. Therefore, advices to have participants who have a positive antibody should be strongly an endscopic examination stomach lesions.

  • ——The First Attempt in Ibaraki Prefecture——

      The present health examination for five-year-old infants was implemented for the first time in 2006 at the public health center in Namegata-shi, Ibaraki prefecture. The purpose of this health project is to build up the network of a supporting system for children and their parents living in Namegata-shi by connecting guardians of children with medical facilities and the municipal administration. Moreover, it is important to control the living environment by regulating the circadian rhythm of daily activities and bodily processes, and by lessening hours watching TV. The health project included a physical examination, medical examination by interview, eye test, consultation of nutrition and oral care, music care, psychological consultation and neurological examination. Controversial children were recommended to make use of the public child guidance classroom. The examination was held ten times in 2006, and 12 times in 2007. The overall rate of attendance at the public health center for health checkups was 90% in 2006 and 88% in 2007. The attendance rate was 8.8% and 5.6% for those children with poor eysight; 5.6% and 9.7% for those with a hyperactivity tendency; 4% and 2.8% for those with obesity; 2.4% and 4.2% for those with verbal problem; and 2% and 3.5% for those with nocturnal enuresis, respectively. This health project found that the bedtime of the children who had hyperactive tendency got significantly later than those who had some problems in speech. The special feature of this group health examination was that child health professionals and officials were able to advise directly to the protectors.

      In recent years, the stress levels in the workplace have gotten worse and worse. The situation has aroused a sreat deal of public interest in mental health. Workers have become more conscious of their own stress levels. The incidences of stress-induced mental illness and overwork-related suicide have increased to the point of amajor social issue. With this knowledge, corporations are thinking in termes of improving their organizational work environment. Becoming aware that stress on the job ia a serious mental health hazard, some organizations are introducing an inhouse system to help employees, with mental problems due to workplace stress. This article reports on the 3-year-old system we have integrated into our hospital to support those employees suffering from workplace stress. First we established a consultation room. Since it opened, everything has progressed smoothly. The number of cases and number of consultations have been increasing each year. Along with the advancement of mental health care and support, we have made efforts to establish a coordination framework, both internally and externally, for bringing to light workplace cardio-health problems, stress management, and rehabilitation. So, you can see we have constructed a strong foundation of a mental health care system for the personnel of the hospital.

      A 59-year-old female was started on oral carbamazepine for her psychological disorder on April 21 2004. Four weeks later, she developed generalized erythemas on the trunk and extremities, and ran a high fever of 38°C. A physical examination showed erythroderma, and laboratory examination revealed liver dysfunction and hypereosinophilia. Three days after admission, a number of small pustules emerged on the erythrodermic backgrounds. The skin lesions were improved by the withdrawal of carbamazepine and systemic prednisolone (50mg per day). However, acute pancreatitis was also developed during the course. HHV-6 IgG was increased up to x1280 on June 17. Also, HHV-6 DNA was detected with two peaks during the therapy. DLST with carbamazepine showed a high titer of stimulation index. This case was unique in the acute generalized exanthematous pustulosis (AGEP)-like clinical appearance and the development of acute pancreatitis. HHV-6 DNA was increased with double peaks, which might suggest a “re-reactivation” of HHV-6 by carbamazepine.

      A 48-year-old man with early gastric cancer underwent distal gastrectomy and Billroth-I reconstruction using a circular stapler. On 18th POD, he complained of abdominal pain, nausea and vomit. Abdominal computed tomography and endoscopy demonstrated dilatation of the remnant stomach and edema of the anastomosis. Treatment with intravenous drip-infusion was made no improvement in clinical symptoms. When Bukuryo-in was administered on 54th POD, his complaints were dramatically reduced. He went on a fluid diet on 57th POD. On 72nd POD, he was discharged from our hospital on administration of Bukuryo-in. After 6 weeks, he was able to put an end to this therapy. At present, he is free from nausea and vomit.
