    The aim of this study is to clarify the relations between the hearing impairment of farmers and exposure to noise generated by agricultural machines. Some studies have revealed that the equivalent continuous sound level (Leq) from agricultural machines exceeds 80dB (A). But the noise -induced hearing loss experienced by farm machine operators has not received much attention, probably because they are self-employed and their working hours are irregular.
    We surveyed 1, 368 farmers, aged 17 to 76 (male 828, female 480), about working hours. We also tested their hearing levels at 1kHz and 4kHz. It was found that 22.3% of the farmers (305 of 1, 368) had some hearing impairment. In the hearing-impaired group, the mean length of working hours on tracters or other machines was 24, 689 ± 21, 155.13 hours. In the healthy group, the mean length of working hours was 16, 077. 65 ± 17, 623. 69 hours. A statistically significant difference was evident in the above results. As the operating hours increased, the incidence of the hearing impairment also increased. To avoid the biases of aging, we examined the results according to each age group. In the 40, 50, and 60-year-old age groups, the operating hours of the hearing impaired group were longer than those of the healthy group. We concluded that the amount of noise exposure probably affects farmers' hearing.

    Ten years have elapsed since the Club of Patients with Artificial Anus and Bladder was founded at the Saku Central Hospital in 1988.
    A check of numerical trends in the surgical cases of rectal cancer at the hospital from 1977 to 1996 indicated that the number of cases having undergone abdomino-perineal resection (APR) was falling despite signs of a rise in the total of cases with rectal cancer. Though there has generally been no numerical change in the cases of ileal conduit (IC) and cutaneous ureterostomy (CU), there will presumably be a rise in the number of cases with continent reservoir and neobladder, the patient's postoperative quality of life considened.
    When thought is given in such circumstances to the cancellation of registered membership by aging club members, it is surmisable that there will be a drop in the number of club members.
    We took a questionnaire survey to forecast the direction in which the club will be managed in the future amidst signs of a numerical decrease in the performance of colostomy. The findings demonstrated that the members have generally been satisfied with the club's management, suggesting the great expectations they put on a club designed for consultations on social security, among others. Even though the number of patients for whom APR, IC (ileal conduit) and CU (cutaneous ureterostomy) is performed and that of new patients registered with the club may not increase to any significant extent, it is strongly thought that there is the need for the hospital as well as the related staff to more positively involve themselves in the club's work.

    We treated 472 patients with lung cancer from 1983 though 1993. Of the total number of the cases, early lung cancer accounted for 31 caces (6.6%), 6 of the hilar and 25 of the peripheral type. Of the 6 hilar type early lung cancer cases, 3 cases were detected by mass screening (2 cases were detected by sputum cytology). On the other hand, of the 25 cases of the peripheral type early lung cancer, 10 cases were detected by mass screening. All the 10 peripheral type early lung cancer cases detected by mass screening were detected by chest roentogenography. The 5-year survival rates for the hilar and peripheral type early lung cancer were 100% and 94.4%, respectively. For the improvement of prognosis of lung cancer, every effort should be made to detect lung cancer at an early stage. Careful lung cancer screening such as sputum cytology and bronchoscopy in a highrisk group and modern techniques such as helical X-ray CT are recommended for the increase in the detection of early lung cancer.
