      Sagamihara Kyodo Hospital (hereinafter, “Kyodo Hospital”) was first opened with 20 beds in August 1945 by what was then called the Kanagawa Agricultural Association, with the aim of providing access to medical care to medically underserved village areas in the northern part of Kanagawa Prefecture. With the growth of the city of Sagamihara, the hospital shifted to become an acute care hospital, reaching a total of 545 beds in 1979. It has obtained numerous designations such as “designated cancer care hospital”, “designated clinical training hospital”, and “community medical support hospital”, and has developed into the central hospital for secondary emergency care in Sagamihara. In this Presidential Symposium, I will discuss the efforts of Kyodo Hospital to provide secondary emergency care in Sagamihara and the role the hospital has played in emergency medicine in the community.(View PDF for the rest of the abstract.)

      The purpose of this study was to clarify the factors limiting food choices of elderly people living in a mountainous region. Focus group interviews were conducted with elderly residents of Unnan City, Shimane Prefecture, to examine their attitudes toward healthy eating habits and factors limiting the variety of food intake. Results of quantitative text analysis showed that attitudes toward healthy eating could be classified into three categories: “balance of health and food”, “eating each food groups”, and “taking care of own health”. The factors limiting the intake of a variety of foods could be classified into three categories: “economic reasons”, “limiting about meat and fish”, and “limiting about potatoes”. Attitudes toward the intake of balanced meals and each food were extracted as attitudes toward healthy eating among the elderly. Economic factors (including the priority given to saving money rather than health) and social environmental factors such as family members living together were included as factors limiting the dietary habits of elderly people living in mountainous regions. There is a need to develop a strategic approach to dietary instruction based on the individual life background of community residents.

      The balance between treatment and farming work must be considered when performing hand surgery in farmers, but no studies have previously examined the level of busyness in farmers in detail. We conducted a questionnaire survey on busyness by month and desirable time for hand surgery in 20 self-employed farmers in the Tokachi region of Hokkaido who had undergone hand surgery. Busyness in crop farming peaked approximately in May for fertilizer application, sowing, and raising and transplanting seedlings and in August-October for harvesting and transporting crops. On the other hand, livestock farmers were busy with rearing management throughout the year, with a peak of busyness in May-October for fertilizer application, sowing, and harvesting and transporting forage crop. Given the approximately 3-month period of work restriction following hand surgery, all respondents wished to have this period from November to January. If a farmer is injured during the peak farming period, a treatment plan is required that supports an early return to work and allows for two-stage reconstruction and treatment of degenerative disease in the quiet period for farming. Meanwhile, almost no respondents expected to receive support from official organizations and manufacturers, indicating that there is a need to provide the government with information from a medical perspective.

      Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is useful for diagnosing mediastinal lymph node lesions. Cell blocks prepared from the needle washing fluid and flow cytometry of tissue samples are helpful in making the diagnosis, but the combination of both examinations is not routinely performed. A 77-year-old woman with fever, dyspnea, and anorexia was admitted to our hospital. Computed tomography showed enlarged mediastinal lymph nodes with calcification and left ureteral calculus; however, no focus of infection was identified. We suspected lymph node tuberculosis or malignant lymphoma, and EBUS-TBNA was performed to evaluate the mediastinal lymph node lesions. Because a cell block prepared from the needle rinse fluid was suspicious for malignant lymphoma, we changed the puncture needle from 22 G to 19 G and performed a second EBUS-TBNA. Diffuse large B-cell lymphoma (DLBCL) was diagnosed based on the results of flow cytometry of the EBUS-TBNA samples. Here we report this case of DLBCL in which mediastinal lymph node tuberculosis was suspected and cell block preparation and flow cytometry using EBUS-TBNA specimens were useful for the diagnosis.

      A 48-year-old woman with a history of recurrent alcoholic pancreatitis was found to have a pancreatic pseudocyst. In November 20XX, she visited the emergency department due to sudden abdominal pain. Contrast-enhanced computed tomography showed a pancreatic pseudocyst with a maximum diameter of 67 mm and ascites. A ruptured pancreatic pseudocyst was suspected and abdominal paracentesis was performed. The amylase level in the ascitic fluid was high (3444 IU/L), leading to a diagnosis of acute generalized peritonitis due to rupture of a pancreatic pseudocyst. Intraoperative findings revealed 500 mL of ascites mixed with pancreatic juice and turbid cyst contents, and distal pancreatectomy was performed. The postoperative course was favorable, and the patient was discharged on hospital day 14. Rupture of pancreatic pseudocyst is rare. In this case, it was promptly diagnosed and successfully treated with emergency surgery.
