    In 1992, the Third Geriatric Health Law was established to systematize colorectal cancer examination, so that an immune fecal occult blood test was applied to detect this type of cancer. The screening method for colorectal cancer through the immune fecal occult blood test has been improved on many times ever since, so far anticipated results being obtained in detecting cancer of the large intestine.
    When it comes to the precision of a kit itself, the immune fecal occult blood test still leaves much to be desired, in practice.
    This paper dwells on, why the colorectal cancer examination through the twice-screening test came to stay, while reviewing the history of chemical fecal occult blood testing in the past. Then, many unsettled problems with the colorectal cancer examination are discussed in reference to (1) the ability and method of data processing aftar close examinations, (2) the training of doctors in charge of colon endoscopic screening, (3) the way of propagating colorectal examination and (4) how to deal with detected adenomas. All these things considered. We suggest that a strategy meeting should be held for working out future colorectal cancer counterplans.

    A PGC morale scale survey was taken on the aged people in a town of Takasu. No difference was observed in the final score between sexes, but responses to some questions differed significantly between men and women. The disparities were considered due to such factors as respondents' conjugal relations, family make-up and health conditions. When compared with healthy persons, those who see doctors regularly for treatment of high blood pressure and cardiovascular disease got poor scores. Especially the scores for the woman outpatients were significantly lower than those for the healthy women. A similar tendency was noted in the results of activity analysis using the ability indexes of a geriatric research institute. Also worthy of note here is the fact that an individual ability score and moral score significantly correlated.
    Therefore, in order to help raise their moral scores, it is very important to help raise their ability to do something in everyday life after due consideration of the quality of life of the aged.

    We conducted an interview survery on the social self-sustenance of two groups of people with cervical spinal cord injuries, most difficult to cure, in a rural environment where aging, family nuclearization and depopulation are in progress. One group was composed of those staying at home (10 males) and the other, of those accommodated in a care facility (nine males, one femal). In the survery, we compared social lives of patients who associate with healthy people in their community and those who see only fellow patients in the care facility. When it comes to the basis for their livelihood, both groups were significantly reliant on pensions.
    Even those patients with so severe disorders as to need help in most part of their everyday lives could be expected to display ability of a kind which is not readily conceivable in light of the conventional criteria for the assessment of physical functions thanks to the use of office-automation equipment. Whether home care for patients with severe disorders could be provided effectively or not depends much on whether they could find their place in their homes as a respectable member of the family, aside from the degree of severity. This factor is more decisive for patients with desorders in the facilities than those at home.

    A 60-year-old man who developed a nephrotic syndrome underwent a renal biopsy, and the case was diagnosed as membranoproliferative glomerulonephritis (MPGN). Despite chemotherapy using steroid, immunosuppresive and anticoagulant drugs, the patient exhibited persistently high urinary protein levels (above 8 g/day), and the renal function deteriorated gradually.
    One and half years later, hemodialysis was started, but soon he had a high fever (above 38-40°C). Laboratory data revealed high levels of both antinuclear antibody titer and immune complex (IC) titer, and a low level of CH50.
    Considering an active collagen disease like SLE, steroid pulse therapy and plasma exchange were instituted. The therapy was very effective this time.
    It is well known that many patients with SLE and a long term history of hemodialysis develop a condition of so called “burn out” in which the activity of SLE declined to allow the withdrawal of steroid therapy. Also well known is the developement of hypocomplementemia in many patients with MPGN.
    In our case, it was difficult to clarify what caused the high fever and other clinical symptoms. We must carefully observe the clinical symptoms of SLE.

    We encountered a patient with Paget's disease. The patient was a 35-year-old-female. She visited our clinic, complaing about masticatory dysfunction. Examination revealed cranial bone outgrowth, alopecia, amenorrhea, mobile and malposed teeth, and alveolar expansion of the maxillae and mandibule. Radiographic observation of the skull and facial bones revealed a typical ground grass appearance, periapical cementum hyperplasia of the teeth. Histologic findings showed a mosaic architecture characteristic of Paget's disease.

    Cardiovascular accidental cases which occurred after the injection of an enema or evacuation are reviewed. 1) The accidents occurred not only in the aged but the youth, so care should be taken at all ages. 2) They occurred especially in the patients with underlying diseases, mainly in cardiovascular or autonomous nervous systems. 3) Therefore age and basal diseases are considered separate factors. 4) Many of the cases occurred soon or in 10 minutes after evacuation. These observations are discussed in light of literatures on changes of blood pressure, mechanism related to the autonomous nervous system, treatments, and so on.

    The HCV antibody positive rates of blood donors were studied in Yamaguchi Prefecture. C100-3 antibody was examined by the method of the 1st generation. The subjects were 146, 792 people who donated blood from Nov. 1989 to Dec. 1990. The average positive rate was 1.01% in Yamaguchi Prefecture. The positive rates of more than 3% were registered in four towns. In one town out of these four, the average positive rate was 22.0% It was extremely higher than the other three towns. Further study will be required to elucidate such regional differences in the HCV positive rate in the prefecture.
