      The purpose of this study was to shed light on the prevalence of the metabolic syndrome (MetS) in a rural community and examine whether or not serum adipocytokine levels are associated with the Diagnostic Criteria for MetS and how many MetS components laid down in the standards subjects have. The subjects were 2,858 people (1,744men and 1,114 women, mean age 49.8+/-9.9 yrs) who had participated in our health screening program between April and December, 2005. After obtaining informed consent from them, we measured serum leptin, adiponectin and high-sensitivity C-reactive protein levels, while performing routine blood tests. In the present study, these measurements were used in combination with the number of MetS components applicable to the conditions of our subjects. Two hundred men (11.5%) and 25 women (2.2%) were diagnosed with MetS. The number of MetS women was extremely small. Those women who were above the standard waist circumference of above 90cm accounted for 8.7% of the total. Thus, most women subjects were excluded as MetS candidates here. The leptin levels of the MetS group were higher than those of the non-MetS group in accordance with the number of applicable components in the diagnostic standards for MetS. Especially, the MetS group which met three MetS criteria showed significantly high leptin levels (p/5=0.0081) of 5.26+/-1.86 (ng/ml) compared to 3.31+/-1.44 (ng/ml) of the non-MetS group with waist circumference below 85cm. On the other hand, no significant difference was noted between the groups in adiponectin level. However, in contrast to leptin levels, adiponectin levels were lower in the group having three MetS components than the non-MetS group 4.37+/-1.57 (ng/ml) as against 5.52+/-3.34 (ng/ml). High-sensitivity CRP levels did not show any significant difference between the two groups.
      As regards the cutoff value of women's waist circumference, further study needs to be done. It was suggested that serum leptin levels might be closely related to the diagnostic criteria for MetS and the number of MetS components applicable.

      Our aim was to study the scan protocol of 16 DAS (Data Acquisition System) multidetector-row CT (MDCT) and to ascertain whether we could reduce radiation exposure and scanning time, while maintaing image quality in the Virtual endoscopy (VE).
      We made a mimic gastrointestinal tract and examined the result by changing a scan protocol of 16 DAS MDCT.
      We found that a reduction of radiation exposure and scanning time depended on slice width, angle of the table, rotation time, table speed, mAs/slice and resolution.
      The study confirmed that it was possible to use the scan protocol of MDCT with a radiographic dose of 40% and scanning time of 50%, without changing image quality in the VE.

      We experienced a case of severe alcoholic hepatitis. Combination treatment of continuous hemodiafiltration and steroid hormone was started immediately after admission. This treatment was very effective for severe alcoholic hepatitis, resulting in the shortening of the length of hospital stay. The reasons why this treatment was effective were; 1. The patient was younger, so the regeneration ability of liver cells was stronger. 2. The infection and bleeding of digestive organs except for acute renal failure were not found, resulting in the enhancement of the effectiveness for steroid hormone treatment. 3. The intensive blood purification treatment was started immediately. From this experience, we realized again that a fine collaboration of doctors, nurses and clinical engineers was very important in the intensive care of the severe diseases.

      With aging of inpatients, the prevention of falls in old people is an important task nurses have to take upon themselves. In our hospital, we have been tackling the task in earnest by utilizing a fall prevention assessment score sheet. Our efforts have resulted in changes in the attitude of nurses as shown by a survey. (1) By working on the nursing plan with use of the fall prevention assessment score sheet, nurses have become alert to risk factors for falling. (2) They have realized that assistance suited to the needs of each patient is vital and that collaboration between patients, their families and hospital staff contributes to the prevention of falls greatly. (3) The use of an intervention manual has made it easy for nurses to cope with falls and consequent injury. (4) To share information of every factor for falling among the nursing staff has added greatly to the prevention of the recurrence of the fall.

      It is said that retrospection therapy is effective in stabilizing emotional states of the aged and alleviating or holding in check symptoms associated with senile dementia. In recent years, the extended care facility of Asuke General Hospital has become so crowed with old people with dementia that it has become increasingly difficult to hold annual events on schedule. With the aim of improving the inmates' mental as well as psychological conditions, we more recently tried a retrospection therapy on a group of eight old people consisting of one emotionally unstable person and seven persons suffering from senile dementia.
      The group therapy was performed after ushering the old people into a private room with soft BGM played. In such a relaxing atmosphere, the session began along with a certain theme. For assessing the effectiveness of the therapy, 1) DMAS, 2) MOSES and3) NM scale were used. Apparently it produced some desired effects in every participant. Thus, the retrospection therapy proved to be useful. However, the results obtained varied depending on the kind of assessment methods. From this, we have also learned that there would be a risk of making an error of judgment with one assessment table.

      Patients who were just operated on for artificial carpal bone replacement or total artificial hip joint replacement are at high risk of luxation. They are recommended to use pillow-like cushions for propping up their hips or legs for a couple of weeks postoperatively. Even after the removal of the props, there remains a chance of dislocation. Some patients, lying upon their back and spreading their legs apart day and night, complain of pain. With a view to allaying the patients' pain, we attempted to develop a cushion such that patients could toss and turn in bed easily. A questionnaire survey was carried out on all the nurses who had learnt a lot from experience in using pillows commercially available. Based on the findings of the survey, we trial manufactured cushions and used them in one capitate bone replacement patient and three hip joint replacement patients. The cushions were well received by the patients and rated high by a orthopedic surgeon who checked X-rays of the patients.
