  • -Hemodilution with 6% Hydroxyethyl starch saline and Controlled Hypotension with Trimetaphan (TMP) of Trinitrogricelin (TNG) .-

    Hemodynamic changes and regional blood flows responses to acute hemodilution and controlled hypotension were studied in 24 mongrel dogs anaesthetized with halothane and para-lized with pancuronium. Hemodilution was produced by 20 ml/kg removal of whole blood. The infusion of 6% Hydroxyethyl starch saline was started when 10 ml/kg of the whole blood was removed. The total infusion of 6% Hydroxyethyl starch saline was 20 ml/kg. Subsequently, hypotension was produced for 60 minutes by intravenous infusion of trimetaphan (TMP) or trinitroglicern (TNG), at mean arterial pressure of 60 mmHg. The following results were obtained :
    1. In the hemodilution and TMP-induced hypotension group (the HD/TMP group), CI and renal cortical blood flow showed a significant decrease at 60 minutes of hypotension. The oxygen-carryjng capacity showed a significant decrease and oxygen extraction showed a gradual increase when hypotension was induced.
    2. In the hemodilution and TNG-induced hypotension group (the HD/TNG group) MAP was not attained at the hypotension 60 mmHg (It was induced only 68 mmHg.) . CI was increased during hypotension. Renal cortical blood flow showed a significant decrease at hypotension of 60 minutes, however it was only slightly more than the HD/TMP group. Oxygen-carrying capacity showed no changes when hypotension was induced.
    From these results, the HD/TMP group was superior to the HD/TNG group in the ability to control hypotension. And the HD/TNG group was safer than the HD/TMP group. However, the renal cortical blood flow of both groups showed a significant decrease at 60 minutes of hypotension. This method is necessary to increase the plasma volume expander.

    A 17 year old girl underwent tumor excision and reconstruction for a recurrent buccal rhabdomyosarcoma. Postoperatively, she developed a local wound infection with MRSA. Multiple antimicrobial chemotherapies and local irrigation failed to improve the symptoms. But administration of Vancomycin with Rifampicin resulted in immediate disappearance of the MRSA from the wound site and improvement of the systemic infection.
    The definition of MRSA and methods for treatment and prevention are discussed.

    From December, 1977 throught March, 1991 there were 231 cases with oral infecions hospitalized in our ward.
    A retrospective review of these patients, especially the cases with refractory infections (which we prescrived over for 3 weeks hospitalization case) was reported.
    The 231 cases were classified periostitis (103 cases, 44.6%), osteomyelitis (24 cases, 10.4%), cellulitis (67 cases, 29.0%) and others (30 cases, 13.0%) .
    The No. of cases, where hospitalization of over 3 weeks was required is highest with osteomyelitis (5 cases, 16.1%) followed by periostitis (3 cases, 2.9%) and cellulitis (2 cases, 3.0%) .
    The features of refractory infections in this study were as follows ;
    1) immunocompromised host e. g. diabetes mellitus and rhumatoid arthritis
    2) the case of wide resection of infection of infectious lesion
    3) the case of which underwent no effective therapy for a long time from onset of infection

    Calcium and phosphate contents, and alkaline and acid phosphatase (ALPase and ACPase) activities in serum of Wistar Strain rat fed with low Ca and Be diets were examined.
    Fourty 4-week-old male rats of the Wistar Strain with initial body weight of 50-60 gr were used. Those animals were divided into 4 groups and were respectively fed with a normal well balanced diet (control group), a low Ca diet (low Ca group), a Be diet (Be group) and a Be·Ca diet (Be·Ca group) for 4 weeks.
    At the end of the experiment period, serum were obtained from the carotid artery of all rats. Using this serum, we measured Ca and P with atomic absorption spectrophotomotry, ALPase and ACPase activity with Kind and King's method, and protein with Lowry's method.
    The conditions of growth and development were worse in the order of the Be·Ca, the Low Ca and the Be groups than those in the control group.
    Serum calcium decreased in the low Ca and Be groups, but in the Be·Ca group, it was similar to that in the control group.
    Serum P decreased in low Ca group and furthermore decreased in the Be group. It also decreased in the Be·Ca group, but its value was similar to that in the low Ca group.
    ALPase activity increased remarkably in the low Ca group. This acceleration was inhibited in the Be and the Be·Ca groups.
    From these facts, it was suggested that low Ca diets accelerated ALPase activity and beryllium inhibited phosphate metabolism.

    In order to evaluate the efficacy, safety and feeling of patients with a spect use of TMD-906 (a double-layer film containing Lithospermum Root extract and glycyrrhetic acid for application to the oral cavity), clinical trials were carried out at Second Department of Oral and Maxillofacial Surgery, School of Dental Medicine, Tsurumi University and First Department of Oral and Maxillofacial Surgery, School of Dental Medicine, Showa University.
    The following results were obtained.
    1) Of the total 36 cases, 27 cases were acute lesions (11 chronic recurrent aphtha, 11 aphthous atomatitis, 5 the others) and 9 cases were chronic lesions (5 Lichenqlanus, 4 the others) .
    2) One to three pieces of TMD-906 were applied to the affected part.
    Application periods were less than 5 days in 9 cases, 5 to 10 days in 22 cases and more than 11 days in 5 cases.
    3) With respect to improvement of clinical observations, improvement of contact pain was 94.4% (33/35), improvement of pain at meals 97.2% (35/36), alleviation of flare 91.2% (31/34) and alleviation of erosion or ulcer was 89.7% (26/29) .
    4) With respect to the feeling of the patients, 100.0% (36/36) of the patients felt “no pain or a little pain” for pain in application. 91.7% (33/36) had “no specific feeling” or “slight feeling of a foreign body” for feeling of adhension.
    These results suggested that the feeling of the patient in application was very preferable.
    Average duration of adhensive was more than 2 hours in 97.2% (35/36) . Adhesion to lesion for long time was superior pharmaceutical property of this product.
    5) Side effects were not reported in any of these cases.
    6) General improvement, general safety and efficacy were judged from clinical observat-tions, feeling of patients and side effects.
    For general improvement, “remarkable improvement” and “improvement” were 91.7%.
    In acute lesion of chronic recurrent pahtha, aphthous stomatitis and the others, each improvement was 100.0%. There was no difference along these diseases.
    In chronic lesion, less improvement were observed, the case of Lichen planus was 60.0 %, others were 75.0%.
    7) For general safety, all cases were “safety” .
    8) For efficacy, “very efficient”and “efficent” were 91.7%.
