    This report describes a clinical application of fibrin adhesive (Beriplast®P), which consists of high concentrated human fibrinogen, thrombin, and blood coagulation factor XIII. This fibrin adhesive was used for the purpose of hemostasis, adhesion, packing, and protection of the wound in 78 cases of various operations in the oral and maxillofacial region. The clinical results were good in 73 cases, and poor in 5 cases. The side effects were not observed in any of the cases in which the fibrin adhesive was employed.
    The results support the fact that Beriplast®P is useful for the oral and maxillofacial surgery.

    A 17-year-old male developed direct Coomb's positive hemolytic anemia, in which hemoglobin level became 6.8g/dl from 14.5g/dl, while receiving cefuroxime (CXM) therapy.
    CXM was administrated after the open reduction and fixation for mandibular fracture. Hemoglobinuria occurred with abdominal pain 24 hours after CXM administration. The patient was treated with massive fluid transfusion and forced diuresis to prevent renal failure. Twenty days later, hemoglobinuria disappeared and direct Coomb's test became negative.
    Type II allergic reaction on the red blood cell membrane to CXM might have caused severe immunological hemolysis.

    The aim of this study was to investigate the effect of incubating temperature and time on oral bacterial growth in vitro.
    Incubating time and temperature were established as follows respectively; 60°C, 70°C, 75°C, and 80°C 30 min. each, 75°C for 15 min. and 20 min, additionally.
    Twenty-nine bacterial species were tested.
    Incubating time of 20min. at 75°C was enough to pasteurize all oral bacterial growth.

    The susceptibility of cefuzonam sodium (CZON) against 99 stains of oral Streptococci which were isolated from the odontogenic infections was measured according to the standard method of Japan Society of Chemotherapy
    Ninety-seven percent of them showed that the MIC is less than 0.39μg/ml, which means the susceptibility of CZON proved quite favorable.
    Considerting the pharmacokinetic parameter of CZON, the agent satisfies the standard necessary for the transient bacteremia during and after the dental surgical procedure.

    Adjuvant chemotherapy has recently become an effective measure for treatment of malignant tumors. Sunfural® S is a slow-releasing Tegafur capsule, which is a masked compound of 5FU, and was developed for the purpose of elevating the effect and reducing the side effects of Tegafur.
    We report here side effects of Sunfural® S possibly caused by the long-term administration of this agent to 26 oral S.C.C.C patients. Side effects were seen in 9 patients (34.6 %) . Two patients experienced two different side effects. Liver dysfunction in 3 cases, loss of appetite in 2 cases, pigmentation in 2 cases, eruption, photosensitivity, decrease of WBC, or decrease of platelets in 1 case. The occurrence of these side effects showed no relevance to patients' age, sex, dosage, or duration of administration. All patients recovered in several weeks after the administration had been stopped. These results suggested the usefulness and the safety of Sunfural® S as an adjuvant chemotherapeutic agent.

    Loxoprofen sodium (Loxonin®) is one of the anti-inflammatory analgesics of the phenylpropionic acid group. Its side effects on digestive organs are less than those of other analgesics because of prodrug. In thes study, for 50 cases with postexodontic pain, the analgesic effect and safety of Loxonin® were clinically evaluated after impacted wisdom tooth extraction.
    1. The frequency of the analgesic effect was 82 % according to impressions of patients, and 80 % by the judgment of the doctor.
    2. Nobody knew the side effects of the drug.
    3. The frequency of the reduction of a pain was 76 %, and the duration of effect was 84 %, each those rates were over “effective”.
    From the results of this study, it was suggested that Loxonin® is effective and safe for treatment of postexodontic pain.

    In the case of post-operated nutritional maintenance in oral and maxillofacial region, enteral tube nutrition (T-330) was applied for a week from the day after operation. Administered cases were 6 cases for mandibular/maxillary bone fracture, 2 cases for mandibular protrusion and 1 case for temporomandibular joint ankylosis.
    The results were as follows :
    1. If slight side effects were included, side effects such as loose feces (2 cases), diarrhea (3 cases), abdomen inflation (2 cases), nausea (3 cases) and vomiting (1 case) were confirmed during the administration week. But, symptoms in each case were so slight that neither reduction in amount of administration nor interruption was performed.
    2. In the case with the side effects of loose feces or diarrhea, a large quanity of total dosage and high concentration were given at the beginning of administration of T-330.
    3. Under the unstable condition with injury and operation, a sufficient and balanced nutritional supply was realized by T-330 without remarkable side effects.
    4. The results of general clinical examination were normal after one week of administration of T-330.
    From the above-mentioned results, we concluded that T-330 could be suitable for nutritional maintenance of the patient operated for oral-maxillofacial diseases, esp., mandibular fracture.

    In the case of post operation nutritional maintenance in oral and maxillofacial region, enteral tube nutrition (T-330) mainly containing di-and tripeptide was applied for a week from the day after operation. Administered cases were 6 cases for mandibular/maxillary bone fracture, 2 cases for mandibular protrusion, and 1 case for temporomandibular joint ankylosis.
    The results concerning plasma amino acid, serum albumin, and total protein were as follows :
    1. In all cases, plasma amino acid after a week of administration of T-330 showed an increasing tendency when compared to that of preoperation. Furthermore plasma aminograms were well-balanced.
    2. Serum albumin, serum total protein and fisher ratio in postoperation (7 days) showed no significant difference when compared to those in preoperation. Each examination value was within the normal limit.
    3. Plasma 3-methylhistidines were N. D. (non-detective) but within the normal limit both in pre-and postoperation.
    4. The results in fresh fractured mandibule/maxilla (6 cases) and comparatively older fractured mandibule (1 case) had the same tendency as those mentioned above.
    From the above-mentioned results, we concluded that T-330 could be a suitable source of nitrogen for nutritional maintenance of the patient operated on for oral-maxillofacial diseases.

    All medicines have some side effects. Medicines are used when the effect is more than the side effect.
    In the cases of antibiotics, their effect for infection is very large, so usually, in consideration of the effect, the side effect does not become a subject of discussion. But drug fever is one side effect that may take a turn for the worse, if we mishandle it.
    The fever in administration of medicine is called drug fever, universally it occurs due to the method of administration, physiological function of medicine, change of system for regulation of body temperature, factor of patient, and allergy.
    This has been a report of drug fever as side effect, allergy, after administration of piperacillin sodium.
    In the course of treatment for infection, body temperature became normal, occasionally rising again. As fever is the main symptom of infection, judgment of fever, whether it is caused by a drug or resistant bacteria, is very difficult.
    Clinically, in the case of an unknown fever, according to the circumstances, not only catching the situation of patient but suspending the administration of medicine is necessary.

    The present study was undertaken to examine the DNA synthesis activity in the first lower molar tooth germs of neo-natal rat cultured under a low-calcium environment. The right and left first lower molar tooth germs were extracted from 12 neo-natal Wistar rats. The right and left molar tooth germs were used as control and low Ca groups respectively. (3H) thymidine (1μCi/ml) was added to the cultures medium at day 8 in culture and the cultures were incubated for another 24 and 48 hours respectively, and the incorporation of [3H] thymidine into the tooth germs was examined.
    The amount of incorporation of [3H] thymidine into tooth germ decreased with time in both control and low Ca groups. The degree of the decrease was higher in the control group than in the low Ca group. The amount of incorporation of [3H] thymidine was more in the low Ca group than in the control group in both 24-and 48- hour-culture periods. The degree of the increase was slightly higher in the 48-hour-culture than in the 24-hour-one.
    The ratio of the low Ca group to the control group for the incorporation of [3H] thymidine into tooth germ was slightly higher in the 48-hour-culture period than in the 24-hour-one.
    From these facts, it was suggested that this phenomenon seemed to be one of cell responses in tooth germ, keeping a normal cell function due to increase of cell differentiation and proliferation against a low calcium environment.

    Air turbine handpiece is indespensable for ordinary dental clinical works. As the air turbine handpiece system contains a retraction valve, it has been hypothesized that the feature causes the dental unit to draw or aspirate cooling water, saliva, and blood back into the water line.
    For that reason, the air turbine system has a high risk for cross infection in the dental office.
    The purpose of this study was to determine the microorganisms in the air turbine handpiece after dental clinical use, and the efficiency of simple disinfection for the air turbine handpiece with an alchoholic disinfectant.
