    It has been thought until recently that acute glomerulonephritis is the disease of children and rare in adults. However, we have found considerable large number of acute glomerulonephritic patients in our out- patient clinic and medical ward over the past several years. In this study we classified those patients into poststreptococcal and non-poststreptococal acute glomerulonephritis on the basis of serum ASO titers which were measured at the time of first visit and at least latter two occasions. Poststreptococcal acute glomerulonephritis (PSAGN) had higher titer than 125 Todd units, whereas non-poststreptoccal (non-PSAGN) had lower value than 125 Todd units. We compared these two groups from the aspects of cinical findings, histological studies (light microscopy and immunofluorecence) of renal biopsy specimens and cellular immunities to streptolysin-O. Major differences of clinical findings between them were prodromal infections, incubation periods and prognoses. PSAGN usually had typical bacterial pharyngo-tonsillitis, two weeks incubation periods and good prognoses. On the contrary, typical non-PSAGN had viral prodromal infections, shorter incubation periods and various different prognoses. Histological studies disclosed that PSAGN had more marked inflammatory cell proliferations in glomeruli than non-PSAGN on the time of renal biopsies, which were done not so much later after the onset of illness. Cellular immunities were studied by the lymphocyte blasto-genesis to streptolysin-O. PSAGN showed 3.5-20.0% blastic lymphocytes, although non-PSAGN had no observed blastic cells. These blastic cells in most of PSAGN cases had declined to the standard level of normal healthy subjects at one year later, which were lower than 5%.

    In the previous paper, we reported the efficiency of EX-01 and -03 coils on the rate of water removal. In this paper, we examined firstly the efficiency of Hollow Fiber Artificial kidney and other newly developed various cartridge type coils on the rate of removal of water, BUN and creatinine. Experimental and clinical data revealed all these coils have an excellent efficiency and can be used clinically as same as EX-series. Secondly, we compared the efficiency of single needle and double needle dialysis system on the rate of removal of water, BUN and creatinine. The results showed that single needle dialysis system has almost the same efficiency as double needle dialysis system, if conditions of dialysis, such as time, pressure and rate of blood flow, are adequately manageable.

    Eleven cases of radiation injury are reported. Three of them were relatively superficial "radiation dermatitis". They received radical excision and free skin-grafting to the cosmetic and functional satisfaction. Eight patients had deeper injury, "radiation ulcer". Six cases were treated by "local flap". The local flap technique is the simplest and the most effective way to treat the radiation ulcer. The reason is 1) it is a one stage operation, 2) it has a permanent pedicle supplying good blood flow, 3) it has very close texture and color match to the area. However, a skin-grafting performed on one patient of radiation ulcer ended up with failure. The indication of the skin-grafting and the local flap was discussed from the point of the stage or degree of radiation injuries and the recommendable method is the skin-grafting to the radiation dermatitis and the local flap to the radiation ulcer.

    From Feb. 1976 to Apr. 1977, 8 cases of patent ductus arteriosus (PDA) were operated without any mortality. Of 8 cases, one case was associated with severe pulmonary hypertension with differential cyanosis. The child underwent division of PDA with successful recovery, though the patient had stormy postoperative course. Postoperative right heart catheterization studies showed remarkable decrease of pulmonary pressure. In contrast to ventricular septal defect (VSD) with pulmonary hypertension, pulmonary artery pressure drops markedly in PDA, even in early postoperative days. Thus infant with PDA, associated with pulmonary hypertension, should be operated vigorously. Our previous clinical and animal experiences suggest PDA should divided rather than ligated to prevent recanalization and aneurysmal formation in long term follow up.

    Recently, the administration of large dosis of methylprednisolone (1g intravenous injection on three succesive days) with low dose steroid therapy was reported to be very effective to control the progression of lupus nephritis and steroid resistant primary nephrotic syndrome. This new mode of therapy is designated as pulse therapy by Cathcart et al. We tried this therapy to one patient with lupus nephritis showing nephrotic syndrome and chronic renal failure after the regular hemodialysis was performed for three month. The remarkable decrease of proteinuria was noticed, although the level of BUN and serum creatinine remained unchanged. Immunologically the titer of ANF and anti-DNA showed also a significant decrease. As a result, the interval of hemodialysis could be reduced from 3 times to once or twice a week. It is noticed that the regular hemodialysis in patients with lupus nephritis can be halted, if the steroid therapy is effective to control the renal failure. In this paper, we also discussed how to stop the dialysis in lupus nephritis.

    A cace of "Intractable Diarrhea", accompanied by Prune-Belly syndrome, is reported. This patient was successfully treated by the feeding of enzymatically digested casein milk. This milk contains no α-lactoalbmin, and no β-lactoglobin, whole casein beeing hydrated to the condition that two-thirds changed into free amino-acid and the rest peptide of moleculer weight less than 1000. It was concluded that the present "hydrated" milk was specifically effective for "Intractable Diarrhea", due to milk protein allergy.

    Osteochondromatosis is a rare condition in which cartilage is formed in the synovial membranes of joints, tendon sheath, or bursae by metaplasia of the connective tissue under the surface of the synovial membrane. The affection is monoarticular and most commonly involve the knee, but hip joint involvement is rare. Although the clinical features are usualy completely non-specific, the roentgenogram often provides important diagnostic information and arthrography is of particular value in diagnosis when the loose bodies are radiotranslucent. In this disease, synovectomy proves to bea good form of treatment. The course is not clinically malignant. The purpose of this paper is to present a cace with hip joint involvement and to discuss the radiological features concerning to the etiology of this disease.

    Recently, lysozyme levels of urine and serum are utilized for the detection of some disease processes, such as tubular protei nuria or monocytic leukemia. We tried to measure lysozyme levels in urine and serum in normal persons and in patients with various glomerulonephritis according to the "Osserman's Lysoplate Method". The level of lysozymn in urine and serum in healthy contorols were 5μg/ml and 30-40μg/ml, respectively and were very high in patients with chronic renal failure up to 150μg/ml in both urine and serum. After the handling of many samples, we could conclude that this method is very excellent in the following points ; 1) the technique is simple and reproducibility is high, 2) many samples can be treated at the same time, 3) the concentration of lysozyme from very low to very high levels can be checked. In this paper, the precise technique of our "Lysoplate Method" were reported. We believe this method should be selected as a routine clinical examination of lysozymn in urine and serum.
