Elemental diets Twinline
® (TL: Ohtsuka pharma) and ElentalR (ET: Morishita pharma) were administered via a feeding tube to patients who underwent oral surgery, and the results were as follows:
1. Prealbumin and retinol binding protein decreased on the first postoperative day but rapidly increased thereafter reaching to each preoperative level on the 14 th day. Compared to the ET group, the TL group revealed a more rapid recovery of these proteins.
2. Although free fatty acids and total choresterol levels were almost constant, the triglyceride level in the TL group increased postoperatively.
3. The middle chain fatty acid (myristic acid) as well as long chain fatty acids (arachidonic acid, li noleic acid, palmitic acid, and eicosapentanoic acid) decreased greatly for 2 days after the surgery, but they began to increase on the 6 th day and their levels became higher than the preoperative levels at 2 weeks. The increase rates of eicosapentanoic acid and linolenic acid were statistically and significantly high in the TL group.
4. However, IL-6 generation was not suppressed and the suppression of GM-CSF production was weak in both groups, generation of TNF-α and IL-β was suppressed in both groups, being severe in the TL group.
5. Generation of reactive oxygens from neutrophils decreased gradually for 2 weeks after the surgery, and the decrease was more prominent in the TL group.
These results indicated that both diets, TL being rather superior, rapidly increase serum levels of proteins and fatty acids, which are necessary decreased by oral surgery, and that both diets, especially TL, suppress lymphocyte and neutrophil function, which indicates caution should be taken against infections.
