    Cancer-induced osteolysis such as bone invasion or bone metastasis causes serious morbidity and reduces the survival rate or quality of life of cancer patients. Osteoclastic bone resorption plays an important role in cancer-induced bone diseases. Angiogenesis is an essential process in tumor growth and metastasis formation. Recently, it has been reported that a close relationship exists between osteoclastic bone resorption and angiogenic factors. Therefore, it is expected that potent osteoclastic bone resorption inhibitors or anti-angiogenesis agents would be efficacious for cancer-induced osteolysis.
    In the present study, we examined the effects of third-generation bisphosphonate as a bone resorption inhibitor, anti-angiogenesis agent TNP-470, matrix metalloproteinase inhibitor against MMP-2 and -9 (MMI-166), and an antibody against connective tissue growth factor (CTGF) as an angiogenesis inhibitor, on an osteolytic bone metastasis model in nude mice. Moreover, the effects of TNP-470 on a hypercalcemia model of nude mice transplanted with a human oral squamous cell carcinoma secreting PTHrP were examined. Consequently, bisphosphonate markedly inhibited the progression of established osteolytic lesions and prevented the development of new osteolytic lesions. TNP-470, an antibody against CTGF, and MMI-166 also inhibited cancerinduced osteolytic lesions in nude mice. TNP-470 also significantly improved the increase of blood Ca2+ in the hypercalcemia model. TNP-470 suppressed osteoclast formation and bone resorption activity in vitro. Antisense oligonucleotide of CTGF inhibited osteoclast formation, but did not affect the osteoclast activity. This indicates that anti-angiogenesis might suppress the osteoclastic bone resorption. These results suggested that inhibition of osteoclastic bone resorption has a possibility of therapeutic use for the treatment of cancer-induced bone osteolysis.

    We examined changes in bone mineral content (BMC) of the mandible and interleukin-6 (IL-6) production from bone marrow cells in ovariectomized (OVX) rats administered bisphosphonate.
    Six-week-old SD female rats after OVX or sham operation were injected with incadronate (10 or 30μg/100g body weight: BW) as bisphosphonate or estradiol (400μg/100g BW) subcutaneously 3 days a week for 4 weeks.
    Both incadronate and estradiol inhibited the increase of total body weight and the decrease of BMC of the mandible and humerus in OVX-rats. The increase of IL-6 release from bone marrow cells in the non-treated group by stimulation with LPS was suppressed in theincadronate group as well as the estradiol group. A dose-dependent effect was found in IL-6 release and BMC in the incadronate-group. However, although the increase of serum Ca level in OVX-rats was inhibited by incadronate and estradiol administration, there was a difference in alkaline phosphatase (AL-P) activity between both groups.
    These results suggest that incadronate increases the BMC through an alternative mechanism compared with estradiol.

    Numerous studies on the cross-sectional area of masticatory muscles, which are correlated to the facial shape, have been reported for normal subjects in previous articles. However to date, there have been no such studies on masseter muscles at jaw-closing and jaw-opening in patients with jaw deformities involving mandibular deviation. The MRI data sets of the masseter muscles at jaw-closing and jaw-opening in 14 female patients with mandibular deviation, who demonstrated a more than 3-mm deviation in the median line in the lower first incisors in comparison to the upper ones, were utilized. The cross-sectional areas from the origin to the insertion at jaw-closing and jaw-opening which were reconstructed perpendicular to the three-dimensional long axis of each masseter muscle, each maximum cross-sectional area (MCSA) and the ratio of the change in MCSA after jawopening were analyzed. As a result, a significant difference was observed between the MCSA at jaw-closing and jaw-opening on the same side. However, no difference in MCSA was seen between the deviated and nondeviated side of the mandible. The line chart patterns of the masseter muscles from the origin to the insertion could be classified into four types. Our results suggest that it is important to analyze cross-sectional areas of the masseter muscles in each subject while considering the three-dimensional axis of each muscle.

    The calcium distribution in tooth extraction sockets of mice was examined using 45-Calcium autoradiography (ARG) and Electron Probe Micro Analysis (EPMA).
    Mice were divided into 8 groups (n=8) according to the number of days (1, 2, 3, 4, 5, 7, 10, 20 respectively) after extraction. Frozen sections were taken from mice on each experimental day after injection of 45-Calcium (RI). The process of formation of new bone was observed using ARG. An ultimate analysis was performed by EPMA.
    Histological analysis was performed with toluidine blue- and alizarin red S-staining. In toluidine blue-staining, an osteoblast was found along the socket wall at 4 days and non-calcified periodontal ligament was recognized until 5 days after extraction. In alizarin red S-staining, new bone was recognized separated from the socket wall at 4 days after extraction.
    45 Ca-labeling was detected strongly in the periosteum of the mandible, the surface of cement and periodontal ligament in control animals. 45Ca-labeling was moved from the bottom to the top of the tooth extraction socket during the period from 1 to 5 days after extraction, but in the periodontal ligament lower than in the granulation tissue. 45Ca-labeling was detected in the socket at 7, 10 and 20 days. At 4 days, calcium phosphate was observed in the central portion of the socket using EPMA.
    45Ca -labeling showed deposition of calcium phosphate for alveolar bone and new bone.
    These results suggest that the granulation tissue may be involved in the initial calcification in the tooth extraction socket and lead to the formation of new bone in it.

    One hundred and four patients with pleomorphic adenoma diagnosed at the Maxillofacial Surgery Department, Tokyo Medical and Dental University Hospital during 15 years from January 1988 to December 2002, were reviewed.
    The results were as follows: 1. Of the 104 patients with pleomorphic adenoma, 34 were males and 70 were females, and the male to female ratio was 1:2. Regarding age, most of the patients were in their sixties, followed by those in their fifties, and those in their forties.
    2. As for the site distribution, the palate and the parotid gland were predominantly affected. The chief complaint of the patients was almost indolent enlargement.
    3. The patients with pleomorphic adenoma in minor salivary glands visited our hospital earlier than those with pleomorphic adenoma in major salivary glands since the former patients noticed their lesions earlier in the oral cavity. The sizes of tumors in minor salivary glands tended to be smaller than those in major salivary glands at the time of the first visit.
    4. We found capsular disorders (incomplete capsule or tumor cells invasion into capsule) of the lesions in major salivary glands (81.4%) more than those in minor salivary glands (49.0%) by histopathological study. Thus, capsular disorders seem to occur more easily in major salivary glands.
    5. Ninety-seven lesions were treated by surgery. Eighty cases were followed over 1 year, and the median follow-up period of those 80 cases was 71.5 months. During the follow-up period, recurrence was found in one primary case of the sublingual gland and one secondary case of the parotid gland.

    At our department, following segmental mandibulectomy, a titanium plate is used for primary reconstruction. However, plate failure occurred in eight patients soon after reconstruction. The cause of these plate failures was analyzed clinically and scientifically to devise a means to prevent such failures. Of 16 patients who underwent segmental mandibulectomy for oral cavity cancer and primary reconstruction using an A-O plate between 1997 and 1998, eight patients who experienced a plate failure served as subjects. Among these patients, clinical conditions (extent of failure and number of screws) were analyzed, and a Vickers hardness test, light microscopy and SEM were performed. The results showed that the origin of each failure was at an area where stress was concentrated, not plastic deformation caused by plate setting. The possible causes of these plate failures included occlusal force generated by remaining teeth, number of screws used for immobilization and plate composition.

    Tumors originating from peripheral nerves are relatively rare in the oral cavity. Such lesions occasionally appear as a central lesion of the mandible, and many of these are Schwannomas. However, central neurofibroma is quite rare. We present a case of intraosseous neurofibroma of the mandible in a 35-year-old Japanese woman. The lesion showed as a quail egg-sized radiolucency in the canine-first premolar region of the left mandibular body, overlapping the mental foramen. The lesion was enucleated with involvement of the inferior alveolar nerve. Histopathologically, the unencapsulated lesion comprised a proliferation of densely packed spindle cells and some collagenous tissue with irregular arrangement. Nerve fibers were scattered throughout the lesion. Immunohistochemistry showed S-100 protein in scattered nerve fibers within the tumor, while proliferating spindle cells were negative for this protein.

    Multi Image Micro CT was useful in determining the anatomical relationship between the mandibular canal and an intraosseous screw in a patient who had an operation for mandibular fracture. A 29-year-old male came to our hospital because of sensory nerve paralysis of the lower lip on the right side. He had been operated on 7 months ago, and had had sensory nerve paralysis since the operation. Multi Image Micro CT showed that the tip of the screw was in the mandibular canal. The paralysis was improved immediately after removal of the screw and returned to normal feeling six weeks later.

    A case of adenoid cystic carcinoma of the mandible advanced extensively to be treated as trigeminal neuralgia, is reported. A 60-year-old female had swelling with left lower lip paralysis. A tumor of the mandible was resected, But the tissue of the carcinoma infiltrated to the base of the skull. Therefore, adjuvant radiotherapy was performed soon after surgery. Tumor growth and distant metastasis have not occurred as of 3 years and 2 months after surgery.

    Neurofibroma is one of the peripheral nerve tumors and is derived from an admixture of Schwann cells and perineural fibroblast proliferations.
    . In this study, the case of a 24-year-old female with a solitary neurofibroma of the cheek was documented, and the appearances in the oral region were reviewed. The enucleated tumor revealed a white, smooth surface and measured approximately 20-10mm. Histopathologically, the tumor was encapsulated and composed of proliferations of spindle-shape cells and wavy collagen fibers in a myxomatous stroma. The tumor cells were positive for immunostaining of S-100 protein. Neither cafe au lait spots nor other neurofibromas were observed on physical examination. Related literatures indicated that this solitary neurofibroma of the cheek was a rare case.

    A 28-year-old male with infective endocarditis (I.E.) caused by odontogenic infection is reported. The patient was admitted to a hospital for the treatment of cerebral infarction. On admission, he had a heart murmur and vegetation of the mitral valve. He was referred to the cardiovascular division of the university hospital and consulted our division chiefly concerning I. E.
    The clinical and radiographic examination showed a maxillary cyst, some apical periodontitis, some marginal periodontitis and multiple dental carious teeth. CRP value decreased gradually into the normal range after extraction of those teeth and a cystectomy. His infected mitral valve was successfully replaced with a prosthetic valve. The patient is now free from the symptoms with a good functional value.
