    The synchronized sampling method of MRI (cine MRI) was applied to demonstrate swallowing movement while a normal volunteer repeated liquid swallows synchronized with the scan sequence. The trigger pulses can also control the scan timing and was repeated regularly 128 times at 4-second interval. Since a normal swallow runs within 2 seconds, the volunteer repeated 128 swallows, regularly, so that 70 longitudinal images with 33 ms intervals each were taken. The sequence for MRI images was fast gradient echo; FOV was 256 mm, TR was 33 ms, TE was 2.2 m, Flip angle was 30°, and NEX was conducted twice at a slice thickness of 5 mm. The cine MRI of a swallow clearly demonstrated bolus transport from the mouth to the esophagus. This technique is expected to be helpful to evaluate the movement of swallowing organs.

    To investigate how diabetes affects the progression of maxillofacial infection, viable bacterial cell counts, wet weight of exudate, and histopathological changes in an experimental buccal infection were examined for 48 hours after inoculation ofStreptococcus pyogenesS-8 into the cavity of carageenin-induced buccal abscess in diabetic rats induced by streptozotosin. Viable bacterial cell counts in diabetic rats were increasing after 36 hours following inoculation ofS. pyogenesS-8, and there were significant differences at 36 and 48 hours between diabetic rats and the control group. Weight of the exudate in abscesses in diabetic rats was significantly higher compared with the control group at 48 hours, and also in this stage, histopathological examinations in diabetic rats revealed more predominant infiltration of inflammatory cells than in the control, which indicates higher vascular permeability in diabetic rats. These results suggest that some factors, in addition to suppression of chemotactic activity of leukocytes, may be related to the bacterial growth in diabetic rats of this model.

    A review of unsuccessful reconstructive cases that required secondary surgical salvage at our department is described. Between 1988 and 2000, the total number of reconstructive cases in which various kinds of free flaps and microvascular reconstruction were carried out by the senior author, was 132. Fifteen cases (11.3%) required secondary surgical salvage due to local postoperative complication. Finally, 12 failed cases (9%) required secondary surgical salvage using pedicled flaps. Evaluation of the causes and methods of the surgical salvage in the unsuccessful cases is very important for future advances and improvements in reconstructive surgery.

    This study was carried out to retrospectively investigate the safety for infection after dental extractions in neutropenic patients with hematological diseases. The subjects were 19 patients, consisting of 5 male and 14 female, ranging in age from 20 to 74 years, with a mean of 49.7 years old. The hematological diseases were 6 cases of aplastic anemia, 5 of acute myeloid leukemia, 3 of myelodysplastic syndrome, 2 of acute lymphatic leukemia, and 1 each of autoimmunoneutropenia, multiple myeloma, and chronic myeloid leukemia. Granulocyte colonystimulating factor (G-CSF) was administered to 9 patients aiming to increase the neutrophil count. The neutrophil count increased after the treatment with G-CSF in 6 patients, but it was unchanged in 3 that had counts of less than 100/μl at the initial visit. Ten patients were not given G-CSF because of the condition of their diseases. Thirty teeth were removed, to prevent them from infection after bone marrow transplant or chemotherapy, under medication of β-lactam antibiotics. Infection of the extraction site occurred in 2 patients, in whom the neutrophil count was less than 120/μl at the time of extraction, regardless of administration of G-CSF. The 2 patients were successfully treated with administration of broad-spectrum antibiotics. These results suggested that dental extraction is safe in most neutropenic patients with hematological diseases but has a risk of local infection in patients with neutrophil counts of less than 120/μl.

    This study was undertaken to investigate the morphologic changes of the forearm flap in the intraoral environment. Histologic evaluation of biopsy specimens from 22 forearm flaps was done at 1 to 75 months after intraoral reconstruction after treatment of cancer. Hematoxylin eosin and Azan were used to stain deparaf f inized sections. The clinical features of the grafted flap seemed to depend on the period following the operation. The cornified layer of the epidermis of the flaps showed thinning histologically. After 10 months of exposure to an intraoral environment, mucosalization of the forearm flap was evident.

    A total of 44 patients with oral squamous cell carcinoma of stage III or N, treated by neoadjuvant chemotherapy followed by radical surgery were investigated, to elucidate the clinical significance of Bax expression for predicting chemotherapeutic effects. Immunohistochemistry for Bax protein was carried out on serial sections from f ormalin-f ixed, paraffinembedded tissues, which were classified into Bax positive and Bax negative. The effects of neoadjuvant chemotherapy were evaluated according to general rules for clinical and pathological studies on head and neck cancer by the Japan Society for Head and Neck Cancer, the Oboshi-Shimosato grade, and classification of variation of kinetic proliferation based on DNA contents and the mitotic index. Bax expression was associated with favorable effects of neoadjuvant chemotherapy. The five-year cumulative survival rate was 70% for the whole series, 90% for Bax-positive patients, and 52% for Bax-negative. Baxpositive patients had a statistically high survival rate compared with Bax-negative (P=0.01). From these results, it was suggested that Bax expression serves as a predictive indicator for neoadjuvant chemotherapy, and as a prognostic factor for survival.

    Verruciform xanthoma in the oral mucosa is an uncommon benign lesion characterized histopathologically by papillary or verrucous proliferation of squamous epithelium and numerous foam cells. We present a case of a 60-year-old female with verruciform xanthoma at the floor of the mouth. The lesion was clinically diagnosed as a papilloma. Histopathological findings showed a papillary proliferation of stratified squamous epithelium in association with parakeratosis and relatively uniform elongated rete pegs. Numerous foam cells were located in the connective tissue papillae between the rete pegs. Immunohistochemically, the foamy cells were positive to lysozyme, vimentin, CD-68, α-1 antitrypsin and Desmin, but negative to S-100 protein. These findings support the concept that the foam cells are derived from a monocytemacrophage lineage.

    Myelodysplastic syndrome (MDS) is a heterogeneous group of clonal hematopoietic disorders characterized by peripheral cytopenia and dysplasia of hematopoietic cells. To date, bone marrow transplantation is the only radical treatment, but this method is suitable for only the youth. Therefore, various other therapies have been applied to the management of MDS. We report the experience of a surgical procedure for a patient with MDS, who has been undergoing immunosuppressive therapy with cyclosporin.
    The patient was a 49-year-old female. She wasreferred to the Department of Oral and Maxillofacial Surgery, Kobe Steel Hospital, Kakogawa, because of gingival enlargement induced by cyclosporin. Extraction of teeth and a gingivectomy were formed after hemostatic control by infusion of HLA-class I antigen-matched platelet concentrates. There were no findings of abnormal bleeding and infection during or after the operation, and the patient did well.

    A case of aneurysmal bone cyst involving the left maxillary sinus associated with central giant cell granuloma, in a 16-year-old high school girl, is reported, with a review of the literature. The aneurysmal bone cyst is neither an aneurysm nor a true cyst, but rather a benign, non-neoplastic, osseous lesion characterized by the presence of numerous blood-filled, usually nonendothelialized cavities. The lesion is regarded as rare in the jaws, and the mandible is more susceptible than the maxilla. The giant cell granuloma is a non-neoplastic proliferative lesion that can involve either oral soft tissues, alveolar bone, or both tissues. It resembles bone lesions, such as aneurysmal bone cyst, fibrous dysplasia, and cherubism. This report also provides a suggested pathogenesis and the most differentiating points from the resembling lesions.

    Salivary duct carcinoma is rare carcinoma of high malignancy and poor prognosis. We report a case of salivary duct carcinoma, which developed five years after resection of a parotid cyst.
    A 71-year-old male visited our hospital complaining of a swelling of his right parotid gland. Physical examination, CT scanning, and ultra sonography implied that the mass was a malignant tumor. Radical surgery, including neck dissection and postoperative external irradiation, was performed. Although good local control could be achieved, he died of acute circulatory failure about one year after the operation.
    Histopathological examination of the resected lesion showed typical features of a salivary duct carcinoma, but did not show clear relation to the previously resected cvst.
