It is important to utilize the lessons learned from previous failure cases of IT projects for the success of future projects to be conducted by the relevant organization and other organizations by means of cause analysis. In an effort to achieve this, many IT corporations have performed cause analysis of actual failure cases, and have disseminated the results of the analysis and lessons learned to the organizations concerned. Consequently, the main causes of failure have become clear, and we have been able to provide useful information to other projects and organizations which are potentially subject to such failure. However, in terms of failure prevention, this information cannot always prevent the similar problems before they occur. There are some cases where it is difficult to prevent the problem event from occurring in spite of the sufficient knowledge about the causes. This study attempts to identify such cases and categorize the failure causes involved, and outlines the method of addressing them.

One of the major causes of project failure is that customer's requirement is not well understood and described. The project, in which the requirement definition, the specification change, and the scope change are mainly handled by the customer and the project team is passive, has higher possibility of failure. To derive the customer's real requirements in early phase is a key to lead the project to the success. The author discusses methods to mitigate the project risks caused by scope change, by predicting coming new requirements through the communication with the customer.

Project managers sufficiently recognize that risk management is very important for the success of projects. However in real projects, project managers can' t identify the risk, let alone manage the risk in most cases. Especially a lot of inexperienced managers tend to be as mentioned above. To solve this problem, we analyzed the general risk in the SI projects, and, as a result, we made the Risk Identifying Tool for the purpose of making up for the inexperience of a project manager. Moreover, we applied it to actual projects, and confirmed effects.
