    Bronchial responsiveness to inhaled doubling concentrations of acetylcholine using the astograph and single breath techniques was assessed in 7 males and 11 females (mean age 22.7, SD=2.05 and 21.2. SD=1.11 years for male and female respectively) healthy volunteers. Eleven (61%) of the subjects had neither a history of allergy nor wheezes. Five (28%) had allergic rhinitis and two (11%) had asthma. Seven (64%) of the normal subjects, all asthmatic and subjects with allergic rhinitis responded to inhaled acetylcholine. Respiratory conductance (Grs) and the one second forsed expired volume (FEV1) expressed as percentages of the baseline values were compared on a semilogarithmic scale against cumulative dose. In most subjects the pattern of the curves showed close similarities in the two methods. The bronchial sensitivity i. e. minimum cumulative dose of acetylcholine just sufficient to start a fall in the FEV1 and Grs (CminS and CminA respectively), showed good correlation (r=0.750, P< 0.01). The results indicate that bronchial responsiveness can be reliably assessed using the simple single breath method as well as the more complex astograph method.

    We investigated the effects of ethylene glycol (EG) on the hepatic drug metabolizing enzymes. The exposed group was given 1% EG solution and the control group was provided with distilled water for 2 weeks ad libitum. The body weight of the exposed group was the same as that of the control group. The liver and kidney weight per body weight did not change. The daily drinking volume for the exposed group on the average showed an increase of 13.5% over that of the control group. Hematologically and biochemically, anemia, liver and renal dysfunction were not seen. The content of the hepatic microsomal cytochrome P-450 in the exposed group showed an increase of 17% over that of the control group, but the contents of cytochrome b5, protoheme and the activities of NADPH-cytochrome c reductase, NADH-ferricyanide reductase did not change. The activities of the hepatic cytosolic alcohol dehyd-rogenase and glutathione redutase, glutathione peroxidase, glutathione-S-transferase also did not change. These results indicate that the hepatic microsomal cytochrome P-450 takes part in the metabolism of EG.

  • ―Biological Monitoring for Methyl Bromide Exposure―

    A head-space gas chromatography (GC) and an ion chromatography coupled with a conductive detector (IC) were used to evaluate levels of bromide ion in urine. The GC method followed by methylation with dimethyl sulfate, showed higher accuracy (27% of relative standard deviation) and lower minimum detection limit (0.4 mg/ℓ ) compared to the IC procedure of those values which were 8.7% and 1.0 mg/ℓ. The correlation coefficient between the analytical results of 15 urinary samples by the two methods was 0.793. The bromide ion concentrations detected in 36 urine samples of workers exposed to methyl bromide were averaged at 13.3±7.7mg/ℓ. The average bromide ion of the non-exposed group was 7.1 ± 2.1 mg/ℓ (n = 6). Exposed methyl bromide concentrations of workers were monitored with passive samplers during their work shifts (8 hr). No significant correlation between exposed methyl bromide and bromide ion concentrations in urine was observed.

    The vapor-liquid equilibria of a two component organic solvent was investigated experimentally. The vapor-liquid equilibria data for the ethylacetate-toluene system was in good agreement with the calculated values by Raoult's law. But in the methanol-toluene system, the vapor concentration of methanol was higher than that of the calculated ones. As vapor-liquid equilibria depend on the combination of organic solvent, and as vapor concentration is not always in good agreement with the calculated values, the measurement of solvent in its vapor phase is considered to be quite important to evaluate the human effect of mixed organic solvent.

    We present a case of a 71-year-old female with actinomycosis in the liver, which is a rare region to be primarily affected with actinomycosis. The diagnosis was done histopathologically with a partially resected liver specimen taken during surgery for choledocholithiasis. There were no clinical signs of Actinomyces infection before surgery. The hepatic lesion was a 2 cm sized nodule with histological appearances of abscess-forming suppurative inflammation with fibrosis, in which eosinophilic radiate granules with peripheral clubs were found. The Brown-Brenn stain showed Gram-positive branched filamentous bacilli, which were revealed as acid-fastness by thc Ziehl-Neelsen method of Putt's modification. These findings were considered to be consistent with actinomycosis of the liver.

    A case of arteriovenous malformation (AVM) associated with a meningioma is reported. A 28-year-old woman was hospitalized for generalized convulsion. CT scan and right carotid angiogram revealed AVM in the right parietal lobe extending to the wall of the lateral ventricle. At the time of surgery an intraventricular meningioma attached to the choroid plexus was found in the excised cavity of the AVM. It is suggested that the meningioma might have been caused by chronic irritation due to an increased blood flow in the arteries supplying the AVM, because the AVM and the meningioma exist in the same lesion and share the same blood supply.

    There are several problems in measuring the activities of urinary enzymes, such as alkaline phosphatase (ALP), leucine aminopeptidase (LAP), and γ-glutamyl transpeptidase (γ-GTP) which are derived from renal tubulus. The purpose of the present study is to clarify the basic methodological problems in measuring these enzyme activities. Our results indicate that it is not necessary to dialyze urine when determining these three enzymes, except for alkaline phosphatase activity measured by the Kind-King method. Because of contamination of urine by bacteria and cell elements, the enzyme activity is influenced by centrifugal conditions that depend on time and speed. Our results propose that it is necessary, at least, to centrifuge at 3000 rpm for 10 minutes to obtain satisfactory data. Generally, a 24 hour urine sample instead of spot urine sample should be used for measuring the enzyme activity. However if correcting γ-GTP activity by creatinine, even a spot urine sample may be used for clinical use.

    We have been performing the tracheal reconstruction using allograft in 29 canines, and have obtained favorable results. In this paper, the details of the surgical technique of tracheal reconstruction are described. Important points for achieving technical success are the following: 1) good anesthetic control during operation, 2) proper anastomoses between recipient's trachea and the graft, 3) use of the omental pedicle flap for wrapping around the anastomotic portion.

    The new Industrial Safety and Health Law in Japan, published in 1988, not only has improved the protection of workers against hazards or other risk factors at work, but also has implemented the health promotion of workers. In order to improve their health, factors on workers' daily-lifestyle, including smoking, alcohol consumption, nutrition and exercise, have to be modified to increase well-being. As exercise is one of the most important factors in health promotion, a fundamental knowledge of exercise physiology and exercise prescription is essential for the occupational physician. As various staffs for evaluation and consultation of the workers' exercise ability are necessary, different Ministries and associations have made efforts to educate several kinds of staff members, including doctors for sports medicine, and exercise and/or fitness-instructors. The state of educational courses for the teaching staffs of exercise for health promotion is discussed.
