    It has been reported that arterial stiffness assessed by pulse wave velocity (PWV) can predict cardiovascular morbidity and mortality. Metabolic syndrome and smoking are independently associated with increased risk of cardiovascular disease. The purpose of this study was to evaluate the effect of smoking on the relationship between metabolic syndrome and arterial stiffness. We retrospectively enrolled 1,530 men on whom brachial-ankle pulse wave velocity (baPWV) measurement had previously been performed as part of health screening at Kosin University Hospital. We found that PWV was positively correlated with the following variables: age (r=0.391, P<0.0001), systolic blood pressure (r=0.438, P<0.0001), diastolic blood pressure (r=0.377, P<0.0001), low-density lipoprotein (LDL) (r=0.068, P=0.008) and high-density lipoprotein (HDL) (r=0.027, P=0.287), whereas BMI was negatively correlated, although without statistical significance (r=-0.026, P=0.309). Among current smokers, PWV decreased with increasing BMI (from 1,387.9 to 1,311.6), and increased with increasing HDL (from 1,342.7 to 1,385.4). On the other hand, among never smokers, PWV increased with increasing BMI (from 1,382.3 to 1,458.8), and decreased with increasing HDL (from 1,391.2 to 1,369.7). PWV was inversely associated with BMI and HDL, and these differences are likely due to smoking.

    The Ca2+ imaging method was developed to explore changes in excitability in adrenal medullary (AM) cells in a large field in response to synaptic input and chemicals. The adrenal medullae of rats and guinea pigs were retrogradely loaded with Ca2+ indicator through the adrenal vein. Nerve fibers remaining in the adrenal gland were electrically stimulated to induce postsynaptic responses in AM cells, and chemicals were applied to the cells by adding to the perfusate. With this method, γ-aminobutyric acid (GABA) was shown to increase the Ca2+ signal in almost all and 40% AM cells in guinea pigs and rats, respectively.

    A bucket-type transpelvic socket was fabricated for a man with paraplegia from spinal cord injury, who underwent right partial pelvic amputation and left hip disarticulation. His main problem was inability to sit due to asymmetrical pelvic shape. We prescribed a transpelvic prosthetic socket to enable him to sit again. The socket consisted of a dual structure: a hard frame and soft liner. The main features of the socket were redistribution of pressure to prevent recurrence of pressure ulcer, and a slightly backward tilt to maintain a comfortable sitting position. In addition, the socket had small air holes for ventilation; a big window in the abdominal area for management of stoma and cystostomy; and two straps for donning it independently. In addition, we confirmed the internal pressure distribution in the socket by a pressure mapping system to prevent reoccurrence of skin trouble. Finally, the patient regained independence in activities of daily living, including driving a car, after two months of rehabilitative training.

    Research over recent years have shown that titanium dioxide (TiO2) nanoparticles (NPs) induce inflammation in various lung, kidney, liver and brain cells. Although the mechanism of inflammation is unclear, existing literature suggests the underlying role of oxidative stress. On the other hand, it has also been shown that nuclear factor-kappa B (NFκB) is activated in response to pro-inflammatory cytokines. In this study we investigated the involvement of NF-κB in TiO2-induced inflammation in human lung adenocarcinomic epithelial cells (A549 cells). After 24h of treatment, IL-8 protein release from A549 cells, induced by 10, 50 and 250 μg/ml of P25 TiO2 NPs, were statistically significantly raised, compared to that of the control. This finding corroborates existing literature in that TiO2 NPs induce a dose-dependent increase in the release of IL-8 protein when exposed to A549 cells. However, the binding of NF-κB DNA was not affected after 6 h of incubation with P25. Therefore, NF-κB DNA binding is not the likely transcription pathway that leads to TiO2-induced inflammation.

    An 82-year-old man visited our hospital when he developed a fever of over 38°C after having consumed 5 types of health foods. He had previously been treated for chronic obstructive pulmonary disease, hypertension and hyperuricemia. Blood examination on admission revealed renal dysfunction, marked elevation of C-reactive protein, and an elevated level of serum creatine kinase. According to the laboratory data and his clinical history, rhabdomyolysis complicated by acute renal failure was suspected, but his condition improved and his fever was reduced with fluid infusion. As a drug lymphocyte stimulation test was positive for only saw palmetto in the 5 health foods, we diagnosed the case as rhabdomyolysis induced by saw palmetto. We believe that this is the first case of a health food being the cause of rhabdomyolysis.

    Serious Cystadenoma (SCA) of the Pancreas is seldom malignant and is usually monitored over time. Here we report a case of SCA an enlarged cyst that had to be excised because it was difficult to diagnose by intraductal papillary-mucinous neoplasm (IPMN). The patient was a 58-year-old woman with thecoma of the right ovary accompanied by Meigs syndrome, who had undergone abdominal total hysterectomy and bilateral oophorectomy. Abdominal computed tomography scan (CT) showed a multilocular cyst 2.4 cm in diameter in the head of the pancreas. Fourteen months later, a periodic CT showed that the multilocular cyst had enlarged from 2.4 to 3.5 cm in diameter. Branch duct intraductal papillary-mucinous neoplasm (IPMN) with a tendency to enlargement and with mural nodule was suspected. Pylorus-preserving pancreatoduodenectomy (PPPD) was performed, and pathological findings revealed that the cyst was lined with a single layer of simple cuboidal epithelium. Periodic acid-Shiff staining, with and without diastase digestion, showed abundant glycogen within epithelial cells, yielding a definitive diagnosis of SCA. The ascites were probably due to the Meigs syndrome pressing the cyst, and the size of the cyst appeared smaller than the real size.

    This study aims to elucidate the level of achievement and the challenges in a basic nursing practicum for second-year nursing students through the association method. Analysis revealed various descriptions of learning through the practicum training experiences in terms of understanding the healthcare environment, planning and implementing daily life assistance, and involvement in building a supportive relationship with a care recipient. In this sense, we can assume that the goal of the practicum was achieved. The studentsʼ attitude toward learning showed an emphasis on knowledge, basics, reasoning, and acquisition of knowledge through experience. At the same time, however, we did not observe descriptions of developing the healthcare environment, understanding the methods of developing healthcare plans, or respect for privacy, thus not indicating whether the goal was achieved. In the future, it is necessary to additionally examine specific details of studentsʼ learning through analyzing their reports.
