    Hemolytic disease of the newborn of a -D-/-D- mother
    A case of hemolytic disease of the newborn of a -D-/-D- mother (K. G.) is reported here.
    She was examined by one of us during her third pregnancy. The first pregnancy terminated with the artificial abortion and the second yielded a female infant who died 24 hours after the birth with severe jaundice. The K. G. blood groups were tested with following results: O, C-, D+, E-, c-, e-, K-, k+, Jka-, Jkb-, S-, s+, Fya+, Fyb+, U-, Lub+, P-, Lea-. Her serum was considered to contain several antibodies among which anti-E was identified and anti Jkb was suspected. Blood grouping tests of the K. G. relatives revealed that her sister had also O, -D-/-D- cells which the other blood groups were almost the same as K. G. The cells were not agglutinated by K. G. serum and no unusual antibodies were found in the sister's serum, although she has 4 children and her husband's blood groups are O, C+, D+, E+, c+.
    K. G. was delivered of a female infant who quickly developed deep jaundice. Exchange transfusion was performed with success immediately after the birth using the sister's blood.
    The antibodies of the K. G. serum had increased in titer more than 2 months after the delivery and no change has been observed following one and a half years.

    The studies makes it possible to estimate “critical point” of blood transfusion, beyond which circuratory overloading may develope.
    The internal jugular venous pulse is recorded by photoelectric method. Changes of the amplitude of jugular phlebogram reflects accurately amount and speed of hemorrhage and blood transfusion, as well as central venous pressure does. The amplitude is usually increased during hemorrhage as central venous pressure is decreased, however, compensatory increase of the amplitude is observed in early stage. The amplitude is increased during blood transfusion but it is decreased when central venous pressure elevates markedly. The central venous pressure measurement alone is still insufficient to estimate developement of circulatory overloading due to blood transfusion.
    From the studies, the author propose to call such a condition as “critical point” of circuratory overloading due to excessive blood transfusion, that the amplitude of the jugular venous pulse begins to decrease, on the other hand, the central venous pressure begins to elevate markedly.
    The internal jugular venous pulse is a useful indicator by hemorrhage and blood transfusion.
