1) Postoperative acute renal failure incidentally developes after the administration of sodium alginate in amino-acid solution (Supermin plus) and kanamycin during the operation.
2) Additional administration of tetracyclin arouses much stronger impairment of renal function.
3) The following combinations of kanamycin and plasma expander should be avoided to prevent the developement of postoperative renal failure.
i) Kanamycin and dextran in amino-acid solution.
ii) Kanamycin and dextran.
iii) Kanamycin and low molecular dextran.
4) Incidence of acute renal failure remains low when kanamycin is administrated more than 3 hours before or 3 hours after the plasma expander is given to dogs and rats in normal renal condition.
5) No preventive effect from acute renal failure is detected when mannitol or lactated Ringer's solution is given together with the combined administration of kanamycin and plasma expander.
6) When kanamycin and plasma expander are given at the same time, one should carefully check the postoperative urinary volume, specific gravity of urine, BUN, serum electrolytes and urinary osmotic pressure daily, especially in the case of impaired renal function before the operation.
