    From the reports in the literatures of Japan, we collected and analized 201 cases of primary sarcoma of the large bowel, and studied the treatment and prognosis of cases (43.80/.). Compared with carcinoma, the prognosis of sarcoma is worse and many patients die in one month following the operation. The reason of poor prognosis is probably due to the difficulty of early diagnosis. Postoperative 12 cases (6.0% of 201 collected cases) are alive for 5 years or more, which are 6 myosarcomas, 2 fibrosarcomas, 2 reticulosarcomas, one lymphosarcoma and one melanosarcoma.
    Advancement and prognosis of tumor is so strongly influenced by the histologic pattern of sarcoma as followings. i. e. Reticulosarcoma and lymphosarcoma metastase early to regional lymphnodes and liver, and recurrence occures sometimes within about one year after operation. Myosarcoma has the tendency of hematogeneous metastasis and the prognosis is better than the others, but some cases expired with the local recurrence after 3 years or more. About melanosarcoma, distant organs are affected by metastasis and its prognosis is poor because of being sometimes resected as " hemorrhoid ".
    Radical operation is the first choice for treatment, although chemotherapy and irradiation must be used only as supplemental method.

    Fistulas were classified into 4 main groups according to their spread to perianal or perirectal spaces, i.e., I-subcutaneous or submucous fistula, II-intersphincteric (between internal and external sphincters) fistula, III-infralevator fistula and IV-supralevator fistula.
    I and II groups were further divided into H (high) and L (low), setting the dentate line as a dividing line. III and IV groups were divided into U (unilateral) and B (bilateral). Each of them was also divided into s (simple) and c (complicated).
    Eight hundred and eight cases of fistulas operated in our clinic for the past 3 years were classified according to the above-mentioned criteria and shown in the following table (denominator in each column). Also shown in the same table are re-operation cases in each types-of fistulas (numerator) and re-operation rates (%).

    The results after the operation of anal approach for prolapsus ani were described.
    1) The operative procedure employed were ligature of the rectal mucosa and simultaneous narrowing of the anal ring.
    2) Thirty-two cases in recent 6 years were investigated. Half of the cases were male. The youngest patient was 5 of age, the oldest patient was 78 of age.
    3) Twenty-seven cases were followed up for one year after the operation. Nineteen cases out of 24 cases, that could be completely followed up, showed good results.
    4) It could be emphasized that this operative method is able to apply to younger or older patients because of its good result. This operative procedure must be considered first before abdominal operation is performed.

    The prosthesis which receives the feces from the artificial anus is the most important matter for the patiences of colostomy. This should not be considered as a tool and should be treated as a Prosthesis.
    The author adopted the method of operation research (OR) to make a study of the structure for the colostomy prosthesis. Thirty items for the element of formation were selected by five panel members and then they are set out in six following items after the analysis of the element ;
    1) Sanitary
    2) Fitness
    3) Adaptability
    4) Activity
    5) Simplicity
    6) Economy
    As an analysis of value of each item after settled on the standard of evaluation, a Prosthesis consists of ; a ring of adapter, a cover of adapter, a bag for collecting the feces and a waist belt made of rubber.

    Thirty six cases of foreign bodies in the anorectum experienced in recent 6 years were investigated.
    1) Male and female were in the ratio 25 : 11. Average age of the patients was 51 : 4. These foreign bodies have tendency to be found in older patients.
    2) The foreign bodies by oral route and by anal route were analysed. Thirty three cases were by oral route and 3 cases were by anal route. Among the foreign bodies by oral route, 19 cases were bone from fish. An whisky glass, chopsticks and a safety pin were found in the rectum inserted by anal route.
    3) Complications caused by the foreign bodies were abscess formation in the anal region, passage disturbances of feces and urinary retention.
    4) All of these foregn bodies could be removed per anum. Twenty seven cases needed anesthesia. Operative removing were performed among 23 cases.

    Rubber band ligation treatment of the internal hemorrhoids as discribed by Blaisdell and modified by Barron and McGivney is introduced to Japan. A series of 2098 patients with internal hemorrhoids which are accompanied with or without external hemorrhoids is presented, in whom the chief treatment has been by the Barron ligating technic under spinal anesthesia. After the ligation of internal hemorrhoids, skin tags or/and thrombosed external hemorrhoids are treated surgically if neccesary.
    Secondary bleeding after the ligation occured in 0.5% (11 cases) between 6th and 15th day. No case of anal stricture due to the ligation is observed.
    In those cases which have fistula-in-ano or anal fissure besides the hemorrhoids, ligation of the hemorrhoids is done after surgical treatment of these anal pathological conditions.
    Sufficient time has not elapsed for an adequate follow up, but experience thus far appears to indicate that results are satisfactory as same as those of hemorrhoidectomy and injection treatment for hemorrhoid without an increase in the incidence of complications.

    Authors experienced an old patient with complicated fistula-in-ano and diabetes mellitus followed an unhappy course.
    Generally, at first, the conservative therapy is used to accept in the anal disease. In regard to the anal fistula, it is not correct, especially in old patients.
    Surgical operation should be performed to the old patient with anal fistula in early stage.
