    The early detection of colorectal cancer has become more important policy in Japan, by reason of the increasing incidence of its death rate. In the mass survey of the large bowel, the idea of selective mass survey must be indicated. That is, the screening program prior to the detailed examination is necessary in this mass survey.
    The program and the result of our mass survey tried in 1980 are as follows ;
    The screening test, used with the questionaire and the three days fecal occult blood testing, selected 295 people (19.1 %) from 1547 population of 13 companies. They had the detailed examination of fiberscopy and/or barium enema. One early cancer and 15 adenomatous polyps were detected.

  • Policy on Long-term Control

    Review was made on the policy on the treatment of long-term control of ulcerative colitis on the basis of our experience with 70 cases. Ulcerative colitis is an incurable disease of unknown etiology and of repeated relapsing and remitting type. The basic policy on the treatment of such disease was considered as to how to maintain the patients in a controllable state and how well to control them on long-term basis. Specifically, the following conclusions were obtained.
    1) In the active stage, the initial therapy by a combination of Salazopyrin and Predni-solone seemed to have a better therapeutic effect to lead to earlier remission.
    2) Since about 67% of the recurrent cases was due to the discontinued or decreased administration of drugs, the importance of long-term maintenance of Salazopyrin was strongly suggested for the policy of long-term control.
    3) Environmental factors such as the method of clinical examination and test for long-term observation of the course were also important factors to control patients.
    4) Since some cases of acute fulminating type or relapsing severe attack required no surgical measures, it was suggested that intensive intravenous regimen be tried patiently.

    At present the diagnosis of inflammatory bowel disease (I. B. D.) by examinations of X-ray, endoscopy and conventional biopsy is not fully satisfactory. For the better diagnosis of them, studies were made on 69 of ulcerative colitis (U. C.), 20 of Crohn's disease and 19 of intestinal tuberculosis. In cases of U. C., inflammatory polyposis was found prominent in cases with 1 to 3 months' history from the initial attack and more frequently seen in severe cases, and histological examination of the resected specimens or specimens obtained by polypectomy revealed marked inflammation of the mucosa with loss of the muscularis mucosae (m. m.). In cases of Crohn's disease, cobblestone appearance and inflammatory polyposis were histologically similar, and histological examination of the resected specimens and polypectomized specimens showed mild inflammation of the mucosa and the m. m. remained intact. In cases of intestinal tuberculosis, inflammatory polyps were found multiple, and the resected or polypectomized specimens revealed moderate inflammatory changes of the mucosal epithelium and frequent disappearance of the m. m.. From the present studies, it is concluded that the polypectomy examinations could be equivalent to the examinations of the resected specimens in yielding informations to make more accurate differential diagnosis of I. B. D..

    Colorectal cancer has been increasing in Japanese in contrast with the decline of gastric cancer and such a screening system simulating the stomach mass survey should be established as soon as possible. In USA and European countries, fecal occult blood test, hemoccult test, is widely applied to the colon mass survey, but it is not enough evaluated in Japan. Using both methods of guaiac test and hemoccult test, we examined 319 patients consisting of 67 with advanced cancer, 6 with early cancer, 126 with benign polyp and 120 controls without any gastrointestinal diseases. All cases of advanced cancer and 4 of early cancer (66.7%) showed a positive guaiac test, while 1.5% of advanced cancer and 50% of early cancer showed a negative hemoccult test. Positive rate in benign polyp correlated with its size and 41.3% and 23.0% were positive by the guaiac test and by the hemoccult test respectively. On the other hand, the false positive rate in the controls was 7.7% by the guaiac test and 2.6% by the hemoccult test under restriction of meat and fish, but it increased to 28.6% by the guaiac test and 7.1% by the hemoccult test without the restriction. Thus, the guaiac test was more sensitive than the hemoccult test. Although cancer could be diagnosed in its curative stage also by the hemoccult test, the guaiac test is better for the earlier screening if the diet restriction is kept.

    Lymphoid polyp is one of very rare benign non-epithelial tumors, commonly involving rectum and anal canal. In most cases, operation is indicated to rule out malignant lymphoma. We reported two cases of lymphoid polyps of large intestine, which can be removed and diag-nosed via colonoscopic polypectomy. Histologically, these two polyps showed well-differentiated lymphoid tissue covered by attenuated mucous membrane, compatible with the diagnosis of benign lymphoid polyp. Endoscopic polypectomy may be indicated for the diagnosis of non-epithelial tumors as well as epithelial ones.

    A 33 year-old woman with ulcerative colitis and chronic bronchial suppuration is reported. She developed ulcerative colitis manifested by bloody stools at the age of 30. One year later, because of a severe attack, colectomy with ascendecostomy was performed. Pre-operative chest x-ray films were normal. Nine months post-operatively, she developed a cough productive of purulent sputum. No causative bacilli were cultured from sputum. Chest x-ray films revealed shadowing of bilateral lower, lobes. Results of pulmonary function test, were consistent with mild obstructive lung disease. Despite several antibiotics, symptoms persisted. However, treatment with prednisolone led to dramatic resolution of cough and sputum. Bronchoscopy showed chronic inflammation and bronchography revealed mild dilatation of bronchial trees.
    The time relationship of these disorders, the absence of causative factors of bronchial lesions and the presence of other extraintestinal complications (liver dysfunction and arthralgia) suggest the possibility of more than a coincidental relationship. Chronic bronchial suppuration, as observed in this patients, may be an uncommon complication of ulcerative colitis.

    A 69-year-old housewife was admitted to the hospital because of rectal bleeding. She denied any other G. I. tract symptoms. The physical examination was negative except for the rectal examination which revealed a 1.3 cm tender, round polypoid tumor in the right posterior rectal wall, just above the dentate line. A biopsy of the lesion, suggested basaloid carcinoma.
    On May. 31, 1982. the patient underwent abdominoperineal resection. Pathological findings showed fairly well differentiated squamous cell carcinoma with focal basaloid pattern invading to the submucosal layers. Regional lymphnodes were free from metastasis. Her postoperative course was uneventful.

    Metronidazole was given to five cases of Crohn's disease. The group consisted of two males and three females (age range 20-37 year. median age 24.6 year). Three patients had ileal and colonic involvement and two patients had only colonic involvement. A favorable response was obtained in two cases. One was a 21-year-old man with colonic involvement, who received daily doses of 0.5g of metronidazole for 137 days. The other one was a 20-year-old man with ileal and colonic involvement, but main lesion was in colon. With the dose of 1.0g daily, for 89 days, paresthesia of lower extremities developed in the second case and the drug was withdrawn despite of a favorable response for Crohn's disease. One of three non-responders showed abdominal pain as an adverse reaction.
    We recommend the metronidazole for the treatment of Crohn's disease in which the colon is involved such as Crohn's colitis and Crohn's enterocolitis.

    New suture and ligation treatment (compression therapy) was developed in our hospital for the treatment of internal haemorrhoids and the 100 cases have been treated since May 1978 to August 1980.
    The theory of the method is that the blood vessels in the rectum are running longitudinally, so that, when a haemorrhoid is multiply tied in horizontal position, i. e., parallel to the pectinate line, the ligation of the vessels is effective and the internal haemorrhoid becomes atrophied due to obstruction of the circulation and finally slougshs off. No drugs are needed.
    The operative procedures are that the patient lies in Sim's position, the anus is relaxed and dilated with local anesthesia. Allis' clamp is used to catch the outer third of the internal haemorrhoid. Around needle holder threaded with silk is passed from the right side at the base of the internal haemorrhoid to the left, the silk is pulled through the haemorrhoid and a tight knot is made over the surface of the mucous membrane.
    With the above-mentioned method, 2 silk ligatures are applied for a small haemorrhoid, 3 to 4 ligatures are applied for a larger haemorrhoid.
    The procedure is simple, less painful, and with good results. Ninty one cases were followed up from 2 months to two years, with 90 cases cured and 9 cases improved, with curing rate of 99 %.
